Abstract
[한글]본 연구는 노인요양병원 서비스 질을 향상시키는 정책적 접근 전략을 모색하기 위해 노인요양병원 서비스 질에 미치는 영향 분석을 위해 조직요인과 환자특성요인의 특성을 모두 고려하여 다수준 분석(multilevel analysis)을 적용한 조사연구이다.

본 연구 자료는 2007년 1월 이전에 개설되어 2008년 5월까지 개설된 전체 요양 병원 중 환자 평가표를 확보할 수 있었던 서울 및 6대 광역시와 전국 9개도의 총 253개 기관을 대상으로 2008년 1월에서 5월까지 계속 입원한 환자 17,234명을 대상으로 하였다. 본 연구에서 수집된 자료는 SAS 9.1을 이용하여 기술통계방법, 상관분석(correlation analysis), 다수준 분석(multilevel analysis)을 이용하여 분석하였다.

본 연구에서 노인요양병원 서비스 질은 일상생활수행능력 감소여부, 욕창발생여부, 요실금 및 변실금 발생여부, 유치도뇨관 삽입변화, 요로감염, 체중감소로 측정한 노인요양병원 서비스 질으로 측정하였으며, 연구결과에 따르면 노인요양병원 서비스 질과 관련된 환자특성요인 변수는 연령, 인지기능, 환자분류군, 초기질지표 점수이며, 집단수준 변수로는 설립구분, 병원규모, 간호사 이직률로 나타났다.

노인요양병원 서비스 질의 노인환자 개인수준 분산에 대한 예측변수들의 설명력은 20.79%이었고, 노인요양병원 서비스 질의 집단수준 분산에 대한 예측변수들의 설명력은 23.72%이었다. 노인요양병원 서비스 질의 집단수준 분산에 대해서 환자특성 변수만을 가지고 집단간의 차이를 설명했을 때 설명력이 10.63%였으나, 조직요인 변수를 추가하였을 때 집단간의 차이를 23.72%로 설명력이 늘어났다. 이는 노인요양병원 서비스의 질의 집단간 차이를 환자특성 변수 외에 조직요인을 추가하였을 때 설명력이 늘어남을 보여준다.

결론적으로 우리나라 노인요양병원 서비스 질에 미치는 영향을 알아보기 위해 시행된 본 연구를 통해서 노인요양병원 서비스 질에는 노인환자 특성 이외에 노인요양병원의 조직요인이 작용함을 알 수 있었다. 따라서 노인요양병원 서비스 질을 향상시키기 위한 정책적 접근은 기존의 노인환자 개인에 대한 교육이나 훈련뿐만 아니라, 상위 수준인 간호사 복리후생 및 노인요양병원 시설기준 관리 등의 정책적 접근이 필요함을 시사한다고 하겠다.





[영문]The purpose of this research is to identify factors related to quality of service using multilevel analysis which considers both patient's characteristics and oganizational factors. And the results of the study can help to suggest strategies to improve quality of service in long term care hospitals.

The data was collected from Health Insurance Review and Assessment Searvice(HIRA) data base. The sample was selected from 17,234 patients of 253 long term care hospitals of six metropolitan cities with seoul and nine provinces which were opened from January, 2007 to may, 2008 and had quality indicator score of patients. The selected patients were hospitalized from on January to may in 2008. The data was analyzed by SAS 9.1 using descriptive statistics, correlation analysis, multilevel analysis.

Quality Indicators of long term hospital in this study included the change of activity of daily life, decubitus ulcer, urinary incontinence, fecal incontinence, urethral catheter insertion, urinary track infection, weight loss.

The results indicated that individual level variables related to quality of service were age, cognitive ability, disease classfication, and Initial quality scores. the organizational level variables related to quality of service were ownership, the number of beds, turnover rate.

According to multilevel analysis, the explanatory powers of variables, which is related to individual level variances of quality of service was 20.79%. The explanatory powers of variables, which is related to oganizational level variances of quality of service was 23.72%. The explanatory powers of patient characteristics, which is related to oganizational level variances of quality of service was 10.63%. But, The explanatory powers of variables including patient characteristics and organizational factors, which is ralated to oganizational level variances of quality of service was 23.72%

In conclusion, this study showed that there were differences in the quality of services came from organizational factors as well as patient's characteristics. It is necessary to consider not only individual factors but also higher-level organizational facotrs such as nurse' wellfare, standard of facility to improve the quality of service in long term care hospitals
