| dc.description.abstract | 총담관결석의 진단은 환자의 임상 양상과 여러 혈액, 영상 및 내시경검사 등을 통해 이루어진다. 총담관결석이 의심되는 환자에서 초기검사로 간기능검사와 복부초음파 혹은 복부 CT를 시행할 수 있으며, 이러한 초기검사 결과를 바탕으로 총담관결석의 저위험군, 중등도위험군 및 고위험군으로 분류할 수 있다. 총담관결석의 저위험군은 간기능검사가 정상이고 영상검사에서 총담관의 확장이 나타나지 않는 경우로, 이러한환자에서는 총담관결석을 시사하는 증상이 지속되지 않는 한, 추가적인 검사는 필요하지 않다. 총담관결석의 중등도위험군은 간기능검사에서 이상을 보이거나, 영상검사에서 총담관 확장을 보이는 경우로, EUS나 MRCP 검사를 추가로 시행할 수 있으며, 추가 검사에서 총담관결석이 확인되는 경우 ERCP의 시행이 필요하다. 총담관결석의 고위험군은 임상적으로 담관염이 있거나 영상검사에서 총담관결석이 확인되는 경우이며, 다른 추가 검사 없이 진단 및 치료 목적으로 ERCP 시행이 권고된다. | - |
