Abstract
전립선암의 표준치료로 사용되는 호르몬 박탈요법은 신체적 측면에서 많은 부작용을 야기한다. 특히 혈당장애나 체지방의 축적 등의 대사성 변화를 유발하며, 이러한 현상이 지속될 경우, 대사증후군으로 이어지며, 이는 당뇨 또는 심혈관 질환과 관련성이 깊다. 그러므로 이를 예방하기 위해서는 지속적으로 건강한 생활습관을 유지하고 실천하는 것이 무엇보다 중요하다. 따라서, 본 연구의 목적은 호르몬 박탈요법을 받고 있는 전립선암환자들 중 대사증후군 위험요소를 가진 대상자들의 건강한 생활습관 개선을 위한 간호사주도 온라인 프로그램을 개발하고 그 효과를 확인하기위함이다. 본 연구는 프로그램 개발과 효과평가의 두 단계로 나누어 수행되었다. 프로그램 개발 단계에서는 교수 설계과정에 많이 사용되는 ADDIE 모형과 행동 변화 촉진에 이용되는 정보-동기-행동기술 (IMB) 모델을 활용하였다. 전문단체의 가이드라인, 체계적 문헌고찰, 포커스그룹인터뷰를 통한 대상자의 요구도를 사정하여 프로그램 초안을 완성하고, 각 분야의 전문가 집단의 검토를 통해 최종 프로그램을 확정하였다. 프로그램 효과평가단계에서는 무작위 대조군 전후 실험설계 방법으로 연구를 수행하였다. 단일기관에서 비뇨의학과 외래를 방문하는 48명의 전립선암환자가 참여했으며, 실험, 대조군에 24명씩 무작위로 배정되었다. 선정기준은 6개월 이상 호르몬박탈요법을 받고, 대사증후군 위험요소중 1개 이상 비정상 범위에 있으며, 온라인 교육이 가능한 대상자를 선정하다. 실험군은 4주간 운동, 식사, 생활습관, 호르몬 박탈요법 및 처방약에 대한 부작용관리방법에 대해 온라인 교육을 받았으며, 그 후 8주의 유지기간 동안 연구자는 대상자에게 메신져(매주) 및 전화(6, 8주째)로 건강한 생활습관을 유지하도록 상담 및 격려하였다. 교육자료는 소책자, 동영상, 온라인 교육자료 링크를 제공하였다. 대조군에게는 연구기간이 끝나는 싯점에 소책자를 제공하였다. 효과평가는 총3회(연구등록 당일, 중재시작후 6주, 12주)에 걸쳐 진행되었고, 주요 결과변수는 생활습관의 변화, 대사증후군 구성요소, 체성분, 건강관련 삶의질, 신체기 능이었다. 이중 생활습관과 건강관련 삶의질은 세 번 측정하었고, 나머지는 두 번 측정하였다. 통계분석은 SAS의 linear mixed model을 사용하였다. 그 결과, 시간 경과에 따라 대조군 보다 실험군에서 건강한 생활습관의 변화 (B=29.23, p=<.001), 일부 대사증후군 구성요소의 긍정적인 변화 (공복혈당: B=-12.0, p=0.05; 복부둘레: B=-2.49, p=0.04), 일부 체성분의 긍정적인 변화 (체중: B=-1.52, p=<.001; BMI: B=-0.55, p=<.001), 건강관련 삶의질 영역중에 비뇨기계 자극 및 폐색증상의 완화 (B=14.63, p=0.00)를 확인하였다. 본 프로그램은 전립선암환자의 건강한 생활습관의 변화를 유도하였다. 그로 인해 일부 대사증후군 구성요소 및 체성분의 긍정적인 변화를 확인할 수 있었다. 간호사는 본 프로그램의 적용으로 대상자의 생활습관의 변화를 유도 할 수 있으며, 대상자들의 자가관리 향상에 도움을 줄 것으로 기대한다. 또한 본 연구는 시,공간 제약이 적은 온라인 교육의 개발을 위한 근거 자료로 활용할 수 있다.

Background: Androgen deprivation therapy (ADT), used as standard treatment for prostate cancer (PC), causes many physical side effects. In particular, it causes metabolic changes, such as fasting glucose abnormalities or accumulation of body fat, and its continuation can lead to metabolic syndrome (MetS), which is closely related to diabetes or cardiovascular disease. Therefore, it is important to maintain and practice a healthy lifestyle in patients with PC. Objectives: The purpose of this study was to develop a nurse-led online program to improve healthy lifestyles in patients with PC under ADT who had risk factors of MetS and to evaluate the effectiveness of the program. Methods: This study was conducted in two phases: program development and effectiveness evaluation. In the program development phase, the analysis, design, development, implementation, and evaluation (ADDIE) model, widely used in instructional design processes, and the information-motivation-behavioral skills (IMB) model, used to promote behavior changes, were used. The draft program was completed by assessing the needs of the target population through the guidelines of related organizations, systematic review and meta-analysis, and focus group interviews. The final program was confirmed through a review of an expert group in each field. In the effectiveness evaluation phase, randomized control trial was conducted. A total of 48 patients with PC who visited the urology department at a single institution were recruited, and 24 were randomly assigned to the experimental and control groups each. Two participants dropped out for personal reasons in the experimental group. The inclusion criteria were those who had received ADT for more than six months, had at least one MetS component in the abnormal range, and were able to receive online education. The experimental group received online education on exercise, diet, lifestyle, and side effects management for prescription drugs, which included ADT, along with counseling and encouragement to maintain a healthy lifestyle. For educational materials, a booklet, video files, and links to online training materials were provided. The control group was provided with a booklet at the end of the meeting. The main outcome variables were lifestyle, MetS components, body composition, health-related quality of life (HRQoL), and physical function to evaluate the effectiveness of the program, which was measured three times (at baseline, sixth weeks, and twelveth weeks after initiation of the intervention). Lifestyle and HRQoL were measured three times, and the remaining variables were measured two times. A linear mixed-model in SAS was used for the statistical analysis. Results: Gradually, there were more healthy lifestyle changes in the experimental group than in the control group over time (B=29.23, p=<.001). The experimental group had more positive changes in some MetS components (fasting blood sugar: B=-12.0, p=0.05 and abdominal circumference: B=-2.49, p=0.04), body composition (body weight: B=-1.52, p=<.001 and BMI: B=-0.55, p=<.001), and urinary irritative and obstruction symptoms in HRQoL (B=14.63, p=0.00) over time than the control group. Conclusions: A nurse-led online health coaching program was developed to promote a healthy lifestyle in patients with PC under ADT. The research findings confirmed that health outcomes including the MetS components, body composition, ADT side effects symptoms showed partial improvement along with lifestyle changes. Therefore, nurses can induce changes in patients’ lifestyles and improve self-management of patients who are starting ADT for the first time by applying for this program. In addition, this study can serve as evidence for the development of online education.
