임상간호사와 보건간호사의 건강지각과 건강증진행위 비교

Abstract

[한글]본 연구는 임상간호사와 보건간호사의 건강에 대한 지각정도와 건강행위이행 수준을 파악함으로써 두 군간의 차이를 분석하고, 이들의 건강지각과 건강행위에는 어떠한 상관관계가 있는가를 규명하여 간호사를 위한 효율적인 건강증진전략을 구축하는 기초자료를 제공하는데 그 목적이 있다.

연구대상은 서울시소재 보건소 6곳과 350-400병상규모의 종합병원 2곳에 근무하는 보건간호사 150명, 임상간호사 150명을 임의로 추출하여 설문에 응답한 보건간호사 131명과 임상간호사 139명을 합하여 270명을 대상으로 하였다.

연구도구는 일반적 특성 20문항 건강지각 20문항 건강행위 42문항으로 총 82문항으로 이루어진 설문지를 사용하였다.

통계분석방법으로는 일반적 특성, 건강지각과 건강증진행위 실천정도는 기술적 통계방법을 이용하였고, 두 군간의 차이는 t-test와 x²test로 산출하였으며, 건강지각과 건강증진 생활양식과의 관계를 보기 위해서 피어슨 상관계수(pearson correlation coefficient)를 산출하여 분석하였다.

본 연구의 결과는 다음과 같다.

첫째, 임상간호사와 보건간호사간의 일반적 특성을 비교한 결과, 연령, 근무경력의 평균은 보건간호사가 임상간호사보다 높았고(p=.000), BMI에서는 보건간호사가 임상간호사보다 과체중의 분포가 높았으며(p=.002), 결혼상태에서는 보건간호사에서 기혼인 경우가 높게 나타났고(p=.000), 종교는 임상간호사에서 기독교의 분포가 높게 나타나서 두 군간에는 통계적으로 유의한 차이가 있었다(p=.000).

둘째, 임상간호사와 보건간호사간의 건강지각 분석 결과, 전반적인 건강지각 정도에 대한 평균값은 임상간호사가 보건간호사보다 높게 나타났으나 두 군간에 통계적으로 유의한 차이는 없었다.

셋째, 임상간호사와 보건간호사간의 건강행위 분석 결과, 건강행위 이행 수준에 대한 평균값은 보건간호사가 임상간호사보다 높게 나타났으나 두 군간에 통계적으로 유의한 차이는 없었다.

넷째, 임상간호사와 보건간호사간의 건강지각 및 건강증진행위간의 상관관계를 분석한 결과 두 군 모두 건강지각 (p=.000)과 건강행위(p=.000)와는 통계적으로 유의한 상관성이 있었다.

이상의 결과에서 임상간호사와 보건간호사의 건강지각 및 건강행위에는 두 군간에 유의한 차이가 없었으며, 건강지각 정도가 높으면 건강행위 이행수준은 높아 두 변수간에는 상관관계가 있는 것으로 나타났다. 또한 대상자의 인구사회학적 특성은 건강지각과 건강행위에 영향을 미치는 요인으로는 크게 작용하지 않는 것으로 나타났다.

본 연구의 결과를 통해서 볼 때 보건간호사는 임상간호사보다 지역주민들의 건강증진관리를 중점적으로 수행해야 하는 역할이 더 큼에도 불구하고 임상간호사와 건강행위에도 큰 차이가 없을 뿐 아니라 비만도는 도리어 높게 나타났다. 이에 보건간호사에 대한 건강행위향상 및 건강증진교육이 강화되어야 함을 제언한다.

[영문]The objective of this study is to find out the differences in health perceptions and health promotion behaviors between the selected hospital nurses and public health nurses.

The study subjects were 139 hospital nurses and 131 public health nurses.

The data were collected by questionnaire which included the items on health perceptions and on health promotion behaviors.

The descriptive statistical method is two-sample t-test, x² test, and correlation analysis were used for data analysis.

The followings are the results of this study:

1. According to the analytical results of health perception between hospital nurses and public health nurses, a higher mean value of health perception was shown among the hospital nurses group than the public health nurses group, however, there were no significant differences between the two groups.

2. According to analytical results of health promotion behavior between the hospital nurses and the public health nurses, a higher mean value of health promotion behavior is shown among the public health nurses group than the hospital nurses group, however, there were no significant differences between the two groups.

3. A significant correlation is found between health perception and their health behavior in both groups.

In conclusion, public health nurses, who have more responsibilities on community people's health promotion, have similar health perceptions and health promotion behavior compared to the hospital nurses.

Also, public health nurses were more obese than the hospital nurses. Therefore, more intensive health promotion education should be give to the public health nurses.
