알코올 의존 환자의 알코올에 대한 암묵적 정서 평가

Abstract

[한글]알코올 의존 환자들의 알코올 관련 자극에 의한 음주 동기 유발은 자동적, 무의식적인 만큼, 이에 연관되는 정서에 대한 연구 역시 외현적 평가만으로는 한계가 있다. 따라서, 알코올 의존 환자에게 알코올에 대한 외현적 정서 평가, 각성도 평가, 암묵적 연상 검사 전후의 알코올 갈망감의 변화 등을 함께 평가하여 자동적으로 유발되는 특징적인 인지와, 관련되는 정서를 알아보고자 하였다.

본 연구는 일 정신병원에서 입원 치료 중인 알코올 의존 환자 19명과 25명의 고위험 음주군, 20명의 사회적 음주군을 대상으로 하였다. 대조군은 사회적 음주 이상의 음주를 하면서 알코올 사용 장애가 없는 경우 고위험 음주군에, 사회적 음주 수준인 경우 사회적 음주군으로 삼았다. 알코올 관련 시각자극과 대조 자극을 목표 개념으로, 긍정 표정 자극과 부정 표정 자극을 기여 개념으로 하여 암묵적 정서 평가를 위해 암묵적 연상 검사(Implicit association test)를 시행하였다. 암묵적 연상 검사 전후로 주관적 갈망감의 변화를 Visual Analogue Scale로 측정하였고, Self assessment of Manikin을 이용하여 알코올 관련 시각 자극에 대한 외현적 정서 평가를 시행하였다. 세 군의 알코올 관련 시각 자극에 대한 암묵적, 외현적 정서 평가 결과를 비교하였다. 그리고, 이러한 평가가 임상적 특징과는 어떻게 관련되는지를 알아보았다.

알코올 의존 환자들은 알코올 관련 자극에 대해 암묵적, 외현적으로 부정적인 정서를 연관시킨다. 그러나, 부정적 평가에도 불구하고 음주를 계속하는 것은 알코올과 긍정 정서가 암묵적인 차원에서 어떤 세기로 연관되었는가에 의해 영향을 받기 때문인 것 같다.

[영문]Alcohol craving is induced by alcohol-related stimuli. The objectives of this study is to study implicit emotional evaluation of alcohol craving in alcohol dependence.

The subject group was composed of 19 alcohol dependent patients, 25 high risk drinkers and 20 social drinkers. An implicit association test measured differential association of 2 target concepts(alcohol versus beverage) with 2 attributes(positive emotion versus negative emotion). When instructions oblige highly associated categories to share a response key, performance is faster than when less associated categories share a key. Visual analogue scale for alcohol craving and self-assessment of Manikin were examined.

Patients with alcohol dependence showed negative emotional evaluation of alcohol on both explicit and implicit levels. Nevertheless, it seems that they were not able to control their destructive drinking behaviors. In alcohol dependence, alcohol use was closely related to the degree of positive implicit evaluation of alcohol.
