본 연구는 경피적 관상동맥 중재술(PCI)을 받은 환자의 적대감·분노 및 Serum Homocysteine level이 심질환 재발에 미치는 영향을 알아보기 위한 전향적 연구(Prospective study)로서, 6개월의 추적 기간(follow-up)이 소요되었다.

연구의 대상은 경기도 S시 소재 A대학 부속병원에 내원하여 협심증 진단을 처음 받고 스텐트 삽입술을 성공적으로 시행받은 86명의 환자를 대상으로, 퇴원 후 매월 1회의 전화방문과 외래통원시 직접면담 및 의무기록지를 통해 심질환 재발의 발생유무를 확인하였다. 자료수집 기간은 2002년 10월부터 2003년 6월까지로 연구기간 중 탈락한 2명을 제외한 최종 84명의 환자를 대상자로 확정하였다.

연구도구로는 Costa 등(1986)의 적대감 24문항 4점 척도와 Spielberger(1988)의 상태­특성 분노표현 척도(STAXI)의 한국판(Chon, Hahn, & Lee, 1998) 중 특성 분노 10문항 4점 척도를 사용하였다. Homocysteine 농도는 12시간 이상 공복상태에 있는 대상자로부터 채취한 혈청을 HPLC를 이용한 형광측정법으로 분석하였으며, 수집된 자료는 SPSS/WIN version 11.0을 이용하여 전산통계 처리하였다.



본 연구의 결과는 다음과 같다.

1. 대상자 전체의 적대감·분노·Homocysteine 각각의 평균은 적대감이 60.67 (표준편차 8.77), 분노 22.15(표준편차 6.04), Homocysteine 11.51 mol/L(표준편차 6.42)이었고, 시술 후 심질환 재발은 16명이 발생하여 19.0%의 재발률을 보였다.

2. 적대감·분노 및 Homocysteine과 심질환 재발간의 관계에 대한 가설검증 결과, 1) 가설 1 "적대감 수준에 따른 심질환 재발률에 차이가 있을 것이다."는 통계적으로 유의한 차이를 나타내어(=10.02, p=.02), 지지되었다.

2) 가설 2 "분노 수준에 따른 심질환 재발률에 차이가 있을 것이다."는 통계적으로 유의한 차이가 없어(=1.60, p=.69), 기각되었다.

3) 가설 3 "Homocysteine level에 따른 심질환 재발률에 차이가 있을 것이다."는 통계적으로 유의한 차이를 나타내어(=9.77, p=.00), 지지되었다.

4) 가설 4 "적대감 수준에 따른 Homocysteine level에 차이가 있을 것이다."는 통계적으로 유의한 차이를 나타내어(=11.78, p=.00), 지지되었다.

5) 가설 5 "분노 수준에 따른 Homocysteine level에 차이가 있을 것이다."는 통계적으로 유의한 차이가 없어(=5.82, p=.12), 기각되었다.

3. [가설 1], [가설 3], [가설 4]의 연구결과를 바탕으로, Homocysteine이 적대감과 심질환 재발간에 매개변수로 작용하는지를 로지스틱 회귀분석으로 검증하였다. Homocysteine을 보정(Adjustment)하기 전, 적대감 수준이 Q4인 사람의 Odds ratio가 14.97로 통계적으로 유의하였으나(p=.02), Homocysteine을 보정한 후에는 적대감 수준 Q4의 Odds ratio가 8.30으로 크게 낮아져 통계적으로 유의하지 않았다(p=.07). 이러한 결과는 적대감과 심질환 재발간에 Homocysteine이 매개변수일 가능성을 시사하고 있다.

4. 전통적 위험요인을 통제한 상태에서 심질환 재발에 영향을 미치는 요인을 파악하기 위해 로지스틱 회귀분석을 실시한 결과, 적대감(p=.03)과 Homocysteine (p=.03)만이 심질환 재발에 대해 통계적으로 유의한 변수로 나타났다. 적대감의 경우 Q1(24­-54점)이하에 비해 Q2(55-­61점 이하)는 7.76배, Q3(62­-67점 이하)는 2.30배, Q4(68­-96점이하)는 15.77배로 심질환 재발의 위험이 높았으며, Homocysteine은 10 mol/L 이상인 사람이 10 mol/L 미만인 사람에 비해 심질환 재발의 위험이 7.83배 높았다.



본 연구는 심리사회적 요인과 관상동맥 질환간의 관계를 설명하는데 있어 중

요한 기초자료가 될 뿐 아니라, 적대감 수준을 감소시키기 위한 다양한 심리사회적 중재프로그램의 개발이 요구됨을 시사하고 있다. 향후 추적기간과 연구대상자를 늘려 반복연구를 실시해 볼 것과 Homocysteine의 매개변수로서의 역할은 경로분석을 통해 다시 검증해 보는 후속연구가 필요함을 제언하는 바이다.

[영문]

This prospective study examined the effects of hostility, anger and serum homocysteine level on recurrent cardiac events over a 6-month follow-up period for patients who had undergone a percutaneous coronary intervention.

The present study initially enrolled 86 patients with a diagnosis of angina pectoris who underwent stent insertion at A University Hospital in S city, Korea. To monitor patients'' recurrent cardiac events, each patient was interviewed by telephone once a month, and interviewed directly with a questionnaire at each clinic visit. A chart review was done after discharge. Between October 2002 and June 2003 baseline data for each of the 86 patients were collected. Two patients were dropped for a final total of 84 patients (n=84) in the present study.

Research tools used in this study were a 24 question 4­point scale by

Costa et al(1986) and a 10 question 4­point scale specifically on anger from the Korean version(Chon, Hahn, & Lee, 1998) of State-Trait Anger Inventory by Spielberger(1988). Serum homocysteine was quantified with fluorometry using High Performance Liquid Chromatography in patients who had fasted for over 12 hours. Statistical analysis was performed using SPSS

WIN version 11.0.



The results are as follows;



1. Mean scores for hostility, anger and serum homocysteine level were 60.67(SD: 8.77), 22.15(SD: 6.04), and 11.51 mol

L(SD: 6.42), respectively. Recurrent cardiac events were occurred in 19.0% of the patients(16

84).



2. Hypothesis testing on the correlation of hostility, anger and serum

homocysteine level with recurrent cardiac events showed the following;

1) Hypothesis 1 "The recurrence rate will be different according to the score for hostility" was accepted with statistical significance(=10.02, p=.02).

2) Hypothesis 2 "The recurrence rate will be different according to the score for anger" was rejected with no statistical significance(=1.60, p=.69).

3) Hypothesis 3 "The recurrence rate will be different according to level of homocysteine" was accepted with statistical significance(=9.77, p=.00).

4) Hypothesis 4 "The homocysteine level will be different according to the score for hostility" was accepted with statistical significance(=11.78, p=.00).

5) Hypothesis 5 "The homocysteine level will be different according to the score for anger" was rejected with no statistical significance(=5.82, p=.12).



3. Based on the test results for , and

, logistic regression analysis was performed to test whether serum homocysteine level was a mediator between hostility and recurrent cardiac events. Before adjusting for homocysteine level, patients with a hostility level of Q4 showed an Odds ratio of 14.97, with statistical significance(p=.02); after adjusting for homocysteine level, patients with a hostility level of Q4 showed an Odds ratio of 8.30, with no statistical significance(p=.07). These results indicate that serum homocysteine level may be a mediator between hostility and recurrent cardiac events.

4. Under controlled traditional risk factors, logistic regression analysis was performed to elucidate the factors affecting recurrent cardiac events and showed the following; Hostility(p=.03) and serum homocysteine level(p=.03) were statistically significant parameters for recurrent cardiac events.

In the risk of recurrent cardiac events, hostility levels of Q2(55-61 points), Q3(62-67 points) and Q4(68-96 points) were higher than that of Q1(21-54 points) by 7.76, 2.30 and 15.77 times, respectively and patients with a homocysteine level of more than 10 mol/L had 7.83 times higher rate for recurrent cardiac events than those with homocysteine level of less than 10 mol/L



This study presented baseline data to elucidate whether psychosocial factors were associated with the progression of coronary artery diseases. Accordingly, various programs for psychological intervention are required to diminish hostility. Henceforth, further studies should be conducted on a larger patient population for a longer follow-up period. In addition, path analysis is required to repeatedly test the role of serum homocysteine level as a mediator for recurrent cardiac events.
