고객관계관리(Customer Relationship Management, 이하 CRM라고 함)는 의료기관이, 의료라는 전문적 지식을 바탕으로 국민건강 유지와 경영 효율화의 두 가지 과제를 해결하고자 최근 주목하고 있는 전략 중 하나이다. 따라서 의료에서의 CRM은 단순한 IT 인프라의 구축이 아니라, 환자를 위해 전문적인 의료서비스를 제공한다는 고객지향적인 관리 철학을 바탕으로 한 고객지향적인 조직의 개혁활동으로 인식해야 한다.

본 연구는 당뇨병 환자의 CRM 적용모델을 개발하기 위한 방법론적 연구이며 크게 네 단계로 진행 되었다.

첫 단계인 분석환경 구축을 위해 1997년 3월부터 2003년 2월까지 서울시내 일개 대학병원에 내원한 외래환자를 대상으로 OCS 데이터베이스(처방, 원무, 보험청구 등)를 연관 쿼리하여 외래 진료과 전체의 진료과별 내원 환자수 및 상병군 분포, 진료과별 수입 분포 및 지정진료비 감액 현황, 진료과별 예약부도율 등을 분석하였다. 그 결과를 바탕으로 CRM의 적용 대상군을 당뇨병으로 선정하였다.

두 번째 단계로, 선정된 당뇨병 환자를 이해하고 분석하기 위하여 정량적(인구사회학적 특성과 진료 특성), 정성적 자료(당뇨병 환자 및 접점부서의 인터뷰, 사례, 내부 조사자료 등)를 수집하였다. 인구사회학적 특성과 진료 특성은 MS SQL과 MS Access, MS Excel을 이용하여 분석하였고 당뇨병 환자 및 접점부서의 인터뷰, 사례, 내부 조사자료 등을 바탕으로 요구사항을 분석하였다.

세 번째 단계로, 당뇨병 환자의 정량 및 정성적 자료를 바탕으로 개선 사항과 개선사항을 달성하기 위한 핵심과제를 도출하였다. 핵심과제인 ‘업무 프로세스의 개선’과 ‘접점부서의 효과적인 활용’, ‘시스템 통합’을 바탕으로 당뇨병 환자를 위한 통합관리 전략을 수립하였다.

마지막 단계로, 본 연구자는 통합관리 전략에 따른 예비 IT 모델링을 수행하고 전문가 타당도를 바탕으로 최종 IT 모델링을 완성하였다. CRM 적용모델의 기본적으로 정보와 프로세스로 구성되어 있으며 이때, 정보는 당뇨병 환자의 원무정보, 진료정보, 진료지원정보, 의료기관과의 관계정보 등을 의미하며, 프로세스에는 당뇨병 환자의 발견 및 합병증 발생 예측, 진단 관리, 환자의 의뢰 관리, 접점 부서의 의료서비스 관리, 캠페인 활동 및 홍보 관리 등이 포함된다.

CRM 적용 모델의 운영 목적은 당뇨병 환자에게 단순한 의료서비스를 제공하는 것이 아니라, 개별적인 의료서비스를 제공할 수 있도록 접점부서를 효과적으로 활용하는 것이며 업무별 접점에서 공유되는 정보와 업무 프로세스, 그리고 인적 자원을 통합 관리함으로써 조직의 목표를 달성하기 위함이다.

따라서 CRM 적용모델의 효과적인 운영을 위해서는 조직의 내, 외 환경에 적합한 전략적 목표와 통합 관리 전략을 뒷받침할 수 있는 IT 모델링이 수행되어야 한다.

Customer relationship management(CRM) is one of the strategies that healthcare organizations are recently aiming to achieve the maintenance of public health and financial efficiency. Since CRM in healthcare is not simply an establishment of Information & Technology(IT) infra, it should encompass customer-centered management philosophy in order to provide patients with quality medical services. Thus CRM is not a kind of it solution, but a reengineering activity in healthcare organizations.

The purpose of the present study is to develop a CRM model for patients with diabetes mellitus. The subjects were outpatients who visited a university hospital form March, 1997 thru February, 2003. the distributions of outpatients, types of disease, revenues, exemption of designated doctor''s fee, and no-show rate were obtained for each department by utilizing the OCS database. Quantitative(demographic date, medical characteristics, etc) and Qualitative date(interviews, cases, survey, etc) were collected from the diabetes patients chosen based on the distribution. While the quantitative data were organized by using MS SQL, MS Access, MS Excel, the qualitative data were represented by using tables and flow charts.

The goals of applying the CRM model are not provide diabetes patients with general medical services, but to provide individually-tailored medical services by utilizing the information obtained form the departments of moment of truth(MOT) effectively and integrating the management of work process and human resources. For an effective application of the CRM model, it modeling needs to be performed to support the strategic objectives and incorporate management strategies that are appropriate for internal and external environments. That modeling for diabetes patients includes campaign activities, customer management process, and the functions of predicting and diagnosing the patients and managing the medical services of MOT departments and referral system.
