비관혈적인 P파 신호 평균화 심전도(Signal-averaged P Wave ECG)를 이용한 발작성 심방 세동 발생과 예측에 관한 연구

심방 세동은 임상에서 흔히 관찰되는 부정맥으로 지속적으로 발생하는 경우 심장의 구조 및 혈류 역학적

변화를 유발하여 뇌 및 심장 혈관계 질환의 발생 요인이 된다. 심방 세동의 발생과 재발을 예측하기 위하여 여러 가지 임상 자료와 심장 초음파도 지표 등을 이용하거나 관혈적인 방법인 전기 생리학적 검사를 통해 이루어 졌다.

이에 본 연구에서는 비관혈적인 P파 신호 평균화 심전도가 발작성 심방 세동의 발생과 관상 동맥 우회술 등의 개심술 후에 발생하는 심방 세동을 예측 할 수 있는 독립적인 예측 인자로서의 임상적 유용성을 고찰하고자 하였다.

발작성 심방 세동이 발생한 환자(Group 2 , n=66)를 대상으로 하였고, 심방 세동 소견이 없었던 환자를 정상 대조군(Group 1 , n=45)으로 설정하였다. 또한 같은 기간에 개심술을 시행 받은 환자를 대상(Group 3, n=32, Group 4, n=30)으로 임상적 지표와 심초음파도상 지표 및 P파 신호 평균화 심전도상 지표를 비교 분석하였다. 대상군의 심장 초음파도 상의 좌심방 직경과 총 여과 P파 기간의 증가는 유의하지만 낮은 상관 관계를 보였다(r2=0.169, p<0.05). 발작성 심방 세동 환자군(Group2)과 정상 대조군(Group1)사이에 P파 신호 평균화 심전도 지표 중 P파 간격(비여과 P파 간격 : 109.9±14.8 vs 128.8±29.5ms, p<0.01, 총 여과 P파 간격 : 122.4±11.8 vs 146.2±22.8ms, p<0.01)은 의미 있게 Group 2에서 증가 되었다. 총 여과 P파 간격의 절단치(cutoff value)를 130ms으로 할 때 후전위(late potential) 양성인 경우가 Group 2에 유의하게 많았으며(12/45 vs 50/66, p<0.001) 발작성 심방 세동 발생의 양성예측도는 74.03%이고 음성예측도는 76.04%이며, 예민도와 특이도는 각각 77.0%와 73.0%로 측정되었다(p<0.001).개심술 환자에서 심방 세동 발생군 (Group4)과 발생하지 않은 군(Group3)에서 P파 신호 평균 심전도 지표 중 P파 간격(비여과 P파 간격 : 108.0±12.6 vs 120.6±13.4ms, p<0.001, 총 여과 P파 간격 : 117.4±11.0 vs 136.8±17.2ms, p<0.01)은 의미 있게 Group 4에서 증가 되었다. 총 여과 P파 간격의 절단치를 130ms으로 할 때 후전위 양성인 경우가 Group 4에 유의하게 많았으며 (4/25 vs 20/29, p<0.001) 개심술 후 심방 세동 발생의 양성예측도는 81.07%이고 음성예측도는 73.04%이며, 예감도와 특이도는 각각 69.0%와 84.0%로 측정되었다 (p<0.001). 또한 개심술을 받은 대상군 전체에서 개심술 전후의 총 여과 P파 간격은 유의하게 증가된 소견을 보였다(127.9±16.9 vs 136.4± 24.5ms, p<0.01). /P파 신호 평균화 심전도를 이용한 P파 후전위의 측정은 비관혈적인 방법으로 발작성 심방 세동의 발생을 예측할 수 있는 독립적인 예측 인자로서 임상적 유용성을 확인 할 수 있었다. 또한 관상 동맥 우회술 및 개심술 후의 심방 세동의 발생을 예측 가능하게

함으로써 심방 세동에 따른 혈류 역학적 불안정과 합병증을 예방하기 위한 치료를 선택 할 수 있는 임상적 근거를 마련 할 수 있다.

[영문]

Paroxysmal atrial fibrillation(PAF) is a very common cardiac arrhythmia that cause electro-physiologic and hemodynamic changes in the atrial myocardium. Recurrent PAF and chronic AF causes stroke and cardiovascular disease. For evaluation of risk of PAF are several methods: clinical parameters, left atrial diameter measured by echocardiographic study and invasive electro-physiologic study.

The present study was designed to evaluate prospectively the accuracy of defined signal-averaged P wave ECG(SAECG) for prediction of PAF and AF after coronary artery bypass graft surgery(CABG).



The study groups are divided into 4 subgroups: normal control(Group 1 , n=45), PAF patients(Group 2 , n=66),

patients who haven’t AF after open heart surgery(Group 3, n=32), and patients who have AF after open heart surgery (Group 4, n=30). We compared with group 1 and 2, group 3 and 4 for clinical characteristics,echocardiographic parameters and SAECG.



The relation of left atrial diameter and total filtered P wave duration(tFPd) are weakly positive correlation(r2=0.169, p<0.05). The unfiltered P wave duration (UnFPd) and tFPd on the SAECG was significantly longer in the group 2 than in those group 1(UnFPd : 109.9±14.8 versus128.8±29.5miliseconds, p<0.01, tFPd : 122.4±11.8 versus 146.2±22.8miliseconds, p<0.01). A cutoff

value of tFPd over 130miliseconds predicted PAF with sensitivity 77.0% , specificity 73.0%, positive predictive value 74.0% and negative predictive value 76.0%(p<0.001). The unfiltered P wave duration (UnFPd) and tFPd on the SAECG was significantly longer in the group 4 than in those group 3(UnFPd : 108.0±12.6 versus 120.6±13.4miliseconds, p<0.01, tFPd : 117.4±11.0 versus 136.8±17.2miliseconds, p<0.01). A cutoff value of tFPd over 130miliseconds predicted post-operative AF with sensitivity 69.0% , specificity 84.0%, positive predictive value 81.1% and negative

predictive value 73.0%(p<0.001). The effect of cardiac surgery to tFPd are revealed by significantly prolonged(127.9±16.9 vs 136.4± 24.5, p<0.01).



A prolonged tFPd as measured by SAECG technique may be a noninvasive independent predictor of risk for the development of PAF. Since tFPd are able to predict AF after open heart surgery, it makes possible that we could choose preventive anti-arrhythmic drug therapies for decreasing the incidence of atrial fibrillation causing hemodynamic compromise and the risk of post-operative complications.
