암환자의 영성 개념분석

간호학은 총체적인 인간을 대상으로 하는 학문이며, 인간은 신체·마음·영이 서로 유기적인 관계를 형성하여 서로 상호 작용하는 독특한 존재이다. 특히 암환자와 같이 신체·정신적으로 위기를 경험할 때 영적 차원은 보상기전으로서 더욱 중요하고 핵심적인 인간의 본질로서 작용한다. 그럼에도 불구하고 간호이론, 연구, 실무에서 영적 측면이 간과되어 왔다. 암환자들의 영적 고뇌를 이해하고, 그들의 영적 요구를 정확히 사정하여 적절한 영적 간호를 제공하기 위해서는 영적 간호에 있어서 가장 기본적인 개념이라 할 수 있는 ''영성''에 대한 분명한 개념정립이 요구된다. 이에 본 연구는 암환자에 있어서의 영성의 차원, 선행요인, 속성, 지표를 파악하고 암환자 영성을 재 정의하기 위한 개념분석 연구이다. / 본 연구는 Schwartz-Barcott, Kim(1986)이 제시한 혼종모형을 이용하여 기독교, 불교, 무교를 가진 암환자를 대상으로 영성 개념분석을 시도하였다.

연구대상자는 암을 진단 받고 투병중인 30세 이상의 암환자로 항암요법을 받거나 질병의 악화로 병원에 입원한 환자 중 각 종교를 대표한다고 암병동 수간호사가 추천한 기독교 2명, 불교 3명, 무교(無敎) 3명을 대상으로 하였다./ 자료수집은 해당 병원 관계자의 승낙을 얻은 후, 2002년 4월초부터 6월초에 걸쳐 이루어졌으며, 연구자가 직접 대상자들에게 연구자의 연구의도를 밝히고 대상자들의 동의를 구한 후 진행하였다. / 자료분석은 이론적 단계에서는 광범위한 문헌고찰을 통해 암환자 영성의 차원, 속성, 지표를 제시하고 작업적 정의를 내렸으며, 현장작업단계에서는 Schatzman, Strauss(1973)의 기록체계인 관찰기록, 이론적 기록, 방법론적 기록을 이용하여 현장 노트를 작성하여 자료를 조직화하였다. 이 자료를 토대로 사례별로 참여관찰기록과 심층면담내용을 삽입하여 현장관찰을 통한 종교별, 사례별 암환자의 영성 차원에 따른 속성, 지표를 제시하였다. 또한, 최종분석단계에서는 이론적 단계에서와 현장작업단계에서 파악된 암환자 영성의 차원에 따른 속성과 결과를 비교, 분석하였다. 또한, 종교를 비롯한 각 개인의 특성에 따른 영성 속성의 공통점과 차이점을 비교, 분석하였으며 최종적으로 암환자 영성의 차원, 속성, 지표를 제시하고 암환자 영성을 재 정의하였다.

연구결과는 다음과 같다.

4. 암환자 영성의 정의 / 모든 인간이 가진 본질인 ''영''의 외적 발현으로서 암투병의고통 속에서 더 찾고 강화될 수 있는 역동성과 개인의 종교, 연령, 과거경험, 가족에 대한 부담감에 따라 독특하게 표현되는 절대자, 타인·이웃·외부환경, 의미추구와 관련된 차원에서 나타나는 절대자와의 관계성 또는 자신에 대한 의지와 믿음이며 현실 초월, 미래지향, 삶의 의미와 목적을 추구하게 하는 내적 에너지원이다.

이상의 이론적 단계와 현장작업 단계를 통한 암환자 영성 개념분석을 통하여 암환자의 영성은 각 종교, 연령, 과거 경험, 가족에 대한 부담감에 따라 각기 다르게 표현될 수 있으며 또한, 암진단과 전이라는 생의 고통 속에서 더욱 강화될 수도 있음을 확인할 수 있었다. 그러므로, 간호사는 암진단과 질병의 악화는 영적인 위기인 동시에 영적 성장을 위한 좋은 계기임을 인식하고 독특하게 표현되는 각 대상자들의 영성을 정확히 사정하고 그에 적절한 간호중재를 제공하기 위한 노력을 기울여야 할 것이다

All nurses should provide spiritual care for their clients as holistic persons consisted of body, mind, and spirit. It is especially important to care spiritually for cancer patients facing the crisis of their life. Therefore, the purpose of this study is to analyze the concept of spirituality which is one of the basic concept for spiritual care in cancer patients. The subjects of this study were eight persons; 2 Christians and 3 Buddhists, and 3 persons who do not have any religion. They were cancer patients, and in-patients recommended by the head-nurse of an oncology department because they represented adherents to these different religious faith. The data was collected from April 9, 2002 to June 4, 2002 through using the method of in-depth interview and participant observation with the participants'' consent. The interviews were recorded on audio tapes and then transcribed. There were 3 phases for data analysis- Theoretical phase, Fieldwork phase, Final analytical phase- according to the Hybrid Model suggested by Schwartz-Barcott, and Kim(1986).

In the Fieldwork phase, the data of participant observations and the content of the in-depth interviews were presented and then enumerated as dimensions, attributes, and outcomes of spirituality in the cases. Also in final phase, the attributes and outcomes of the Theoretical phase and the data of fieldwork phase were compared and analyzed according to the dimensions of the spirituality of cancer patients and then, the commonalities and differences according to religion, age, past experiences and burden of family were discussed.

The final results of study were as follows :

Therefore, the new definition of spirituality in cancer patients based on above outcomes was as follows; Spirituality is an expression of ''spirit'' which is entity of human being, having a dynamic process strengthened in suffering due to struggle with cancer and having a unique characteristic according to the

individual''s religion, age, past experiences, burden of family. Spirituality is internal energy manifested in 3 dimensions- related to the absolute being, other person·neighbor·environment and seeking of meaning- connectedness with the absolute being or will and believe oneself, transcendence of reality, future oriented, and seeking meaning and purpose of life.

The result of this study shows that an individual spirituality is manifested differently by his(her) religion, age, past experiences and burden of family, and able to be strengthened in suffering due to struggle with cancer. Therefore, nurses should recognize that diagnosis and deterioration of cancer is not only a spiritual crisis but a good chance for spiritual growth and should try to assess the spiritual problems of their clients.
