미혼여성의 건강위험행위 예측모형

Abstract
[한글]본 연구는 미혼여성의 건강위험행위에 영향을 주는 요인들을 파악하여 이를 설명, 예측하기 위한 모형을 구축하기 위하여 시도되었다.

연구의 가설적 모형은 문헌고찰과 선행연구를 토대로 구성하였다. 외생변수로는 내적 건강통제위, 타인의존 건강통제위, 성역할 정체성의 남성성, 여성성과 인지된 생활스트레스가 있으며 내생변수로는 자아존중감, 건강관련 자기효능, 친구영향과 건강위험행위로 구성하였고 총 25개의 가설을 설정하였다.

연구의 자료수집은 2000년 10월 9일에서 2000년 10월 28일까지 실시하였으며, 서울과 수도권, 충청지역의 직장 미혼여성을 대상으로 자료를 수집하였다. 자료수집방법은 자가보고식 설문지로 하였으며 총 375개의 자료가 분석되었다. 모형검증에 이용된 총 7개 연구도구의 Cronbach' Alpha 값의 범위는 0.64에서 0.95였다.

대상자의 일반적인 특성과 연구변수의 특성은 SPSS 9.02 Win Program을 이용하여 분석하였으며, 가설적 모형과 수정모형의 검증은 LISREL 8.12 Win Program을 이용하여 공변량 구조분석을 하였다.



본 연구의 결과는 다음과 같다.

1. 모형의 부합도 분석결과

1) 가설적 모형의 전반적인 부합지수는 높게 나타나 모형과 실제자료가 잘 부 합하는 것으로 나타났다(χ²/df=1.46(p=.10), GFI=.98, AGFI=.94, RMR=.01, NNFI=.98, NFI=.98, CN=505).

2) 모형의 간명함과 부합도를 높이기 위해 세부지수를 이용하여 3개의 경로를 제거하여 수정모형을 확정하였다. 수정모형의 부합지수는 가설적 모형의 부 합지수보다 조금 더 나은 것으로 나타났다(χ²/df=1.28(p=.18), GFI=.98, AGFI=.94, RMR=.01, NNFI=.99, NFI=.98, CN=549).



2, 가설검증의 결과

1) 자아존중감은 내적 건강통제위가 높을수록(γ11=0.29, t=6.57), 성역할 정체 성의 남성성(γ13=0.23, t=7.40)과 여성성(γ14=0.12, t=4.08)이 높을수록, 그리 고 인지된 생활스트레스가 낮을수록(γ15=--0.17, t=-3.08) 높게 나타났다.

2) 건강관련 자기효능은 내적 건강통제위(γ21=0.12, t=2.44)와 타인의존 건강 통제위가 높을수록(γ22=0.16, t=3.22), 성역할 정체성의 여성성이 높을수록 (γ24=0.08, t=2.46), 자아존중감이 높을수록(β21=0.25, t=4.54) 높게 나타 났다. 성역할 정체성의 남성성은 직접효과(γ23=0.04, t=1.22)는 유의하지 않 았으나 간접효과(효과계수=0.06, t=3.85)가 유의하게 나타나 총효과(효과계 수=0.10, t=2.84)가 통계적으로 유의하게 나타났다. 또한 인지된 생활스트 레스는 직접효과는 없으나 간접효과(효과계수=-0.04, t=-2.68)가 추가되어 유의하게 나타나 건강관련 자기효능에 영향을 미치는 것으로 나타났다.

3) 친구영향은 인지된 생활스트레스가 높을수록(γ35=0.22, t=3.95), 건강관련 자기효능이 낮을수록(β32=-0.23, t=-4.41) 많이 받는 것으로 나타났다. 내적 건강통제위와는 직접효과(γ31=-0.05, t=-1.38)는 유의하지 않았으나 간접효과(효과계수=-0.04, t=-2.97)가 추가되어 총효과(효과계수=-0.09, t=-2.61)가 유의하게 나타났다. 성역할 정체성의 남성성과 여성성은 친구영 향에 대한 직접효과는 없으나 간접효과(남성성; 효과계수=-0.03, t=-2.34, 여성성; 효과계수=-0.03, t=-2.58)가 유의하게 나타나 영향을 미치는 것으로 나타났다.

4) 건강위험행위는 성역할 정체성의 남성성이 높을수록(γ43= 0.06, t=2.75), 여성성이 낮을수록(γ44=-0.05, t=-2.11), 건강관련 자기효능이 높을수록

(β42=-0.21, t=-4.37), 그리고 친구영향이 많을수록(β43= 0.21, t=3.80) 높게 나타났다.

건강위험행위에서 직접효과는 유의하지 않았으나 간접효과가 추가되어 총효과가 유의하게 나온 경로는 내적 건강통제위(효과계수=-0.10, t=-2.614), 인지된 생활스트레스(효과계수=0.11, t=2.39)와 자아존중감(효과계 수=-0.07, t=-2.12)이다.

건강위험행위에 가장 총효과가 큰 경로는 건강관련 자기효능에서 건강위험 행위로 가는 경로였다(효과계수=-0.26, t=-4.68).



이상의 모형검증의 결과 본 연구에서 구축한 미혼여성의 건강위험행위 예측모형은 실제자료와 잘 부합하여 미혼여성의 건강위험행위를 적절하게 예측하는 모형이라고 생각된다. 본 연구의 모형검증 결과 미혼여성의 건강위험행위에 가장 영향을 미치는 매개변수는 건강관련 자기효능으로 나타났다. 건강관련 자기효능은 내적 건강통제위, 타인의존 건강통제위, 성역할 정체성의 여성성과 자아존중감에 의해 영향을 받는다. 자아존중감은 내적 건강통제위, 성역할 정체성의 남성성과 여성성 및 인지된 생활스트레스의 영향을 받으나, 건강위험행위에 직접적인 영향을 주지는 않는다. 친구영향은 인지된 생활스트레스와 건강관련 자기효능으로부터 직접적인 영향을 받으며, 건강위험행위에 직접적인 영향을 미친다.

이러한 연구결과를 바탕으로 미혼여성의 건강위험행위를 예방하기 위하여는 건강관련 자기효능을 높이고, 자신의 성역할 정체성의 여성성에 대한 인지를 확고히 하며, 건강위험행위와 관련되어 친구영향을 적게 받도록 강조하여야 할 것이다.

따라서 이러한 내용을 바탕으로 한 프로그램의 개발과 적용 및 교육 등의 중재방안들을 강구해야 할 것으로 보며, 건강위험행위에 직접적인 영향은 없으나 간접적으로 영향을 미치는 내적 건강통제위와 자아존중감을 증가시키고 스트레스를 줄이는 간호중재 전략의 개발도 미혼여성의 건강위험행위를 줄이기 위해 필요하다고 본다.

[영문]The Purpose of this study is to identify the effects of the psychological factors on health risk behaviors and to develop a predictive model that explains health risk behaviors of unmarried women in Korea.

A hypothetical model was constructed on the basis of extensive review of literature on health risk behaviors.

The exogenous variables of the hypohtetical model were internal health locus of control, powerful others health locus of control, masculinity and feminity of sex-role identity, and perceived life stress. The endogenous variables included self-esteem, health related self-efficacy, peer influence, and health risk behaviors such as drinking, smoking and sexual risk taking behaviors.

The data was collected by self-reported questionnaire from 375 unmarried career women in Seoul, Kyunggi-Do, and Chungchung-Do from October, 9 to October, 28, 2000. Reliabilities of the seven instruments were tested Chronbach's alpha score which ranged from 0.64 to 0.95.

Data analysis was done using SPSS 9.02 Win Program for descriptive and correlational statistics. LISREL 8.12 Win Program was used for covariance structural analysis to find the best fit model which assumes causal relationship among variables.



The results are as follows;

1. The results of covariance structural analysis indicated:

1) The overall measure of fit of the hypothetical model to the data was shown to be good. 〔χ²/df=1.46(p=.10), GFI=.98, AGFI=.94, RMR=.01, NNFI=.98, NFI=.98, CN=505〕

2) Paths of the model were modified by considering theoretical implication and statistical significance of parameter estimates. A modified model was constructed by deleting three paths. Compared to the hypothetical model, the revised one had a better fit to the data.

2. The results of hypothesis testing were as follows:

1) Internal health locus of control(γ11=0.29, t=6.57), masculinity(γ13=0.23, t=7.40) and feminity(γ14=0.12, t=4.08) of sex-role identity, and perceived life stress(γ15=-0.17, t=-3.08) had significant direct effects on self-esteem. 2) Internal health locus of control(γ21=0.12, t=2.44), powerful others health locus of control(γ22=0.16, t=3.22), feminity of sex-role identity(γ24=0.08, t=2.46), and self-esteem(β21=0.25, t=4.54) had significant direct effects on health related self-efficacy. The direct effects of masculinity of sex-role identity(γ23=0.04, t=1.22) and perceived life stress on health related self-efficacy were insignificant but their indirect effects were significant.

3) Perceived life stress(γ35=0.22, t=3.95) and health related self-efficacy(β 32=-0.23, t=-4.41) had significant direct effects on peer influence. The direct effects of internal health locus of control(γ31= -0.05, t=-1.38), and masculinity and feminity of sex-role identity on peer influence were not significant, while their indirect effects were significant on that.

4) Masculinity(γ43= 0.06, t=2.75) and feminity(γ44=-0.05, t=-2.11) of sex role identity, health related self-efficacy(β42=-0.21, t=-4.37), and peer influence (β43= 0.21, t=3.80) had significant direct effects on health risk behaviors.

Internal health locus of control, perceived life stress, and self-esteem had significant both indirect and total effects statistically, but not direct effects on health risk behaviors.



The above findings showed that health related self-efficacy as a mediating variable had the most significant direct effect on health risk behaviors of unmarried women. Also peer influence had a significant direct effect on them.

Internal and powerful others health locus of control, feminity of sex-role identity, and self-esteem had significant direct effects on health related self-efficacy. Self-esteem had not a significant direct effect on health risk behaviors and internal health locus of control, masculinity and feminity of sex-role identity, and perceived life stress had significant direct effects on self-esteem. Peer influence had influenced on health risk behaviors directly, and perceived life stress and health related self-efficacy had significant direct effects on peer influence.

Finally, to increase health related self-efficacy, to identify sex-role identity, and to reduce peer influence, an effective intervention program and education should be developed to prevent health risk behaviors of unmarried women.

Also it is necessary to develop nursing intervention strategies for health risk behaviors of unmarried women by increasing internal health locus of control and self-esteem, and reducing perceived life stress which had significant indirect effects on health risk behaviors.
