Abstract
[한글]

위이형성은 위의 다단계 발암과정의 가장 진행된 점막의 비침습 병변이며 위암의 전구 병변으로 알려지면서 그 중요성이 높아지고 있다. Helicobacter pylori(H. pylori) 감염이 위암 발병의 일차적 원인이라는 사실이 역학적으로 보고되었다. H. pylori 위염은 세포의 증식과 DNA 손상을 일으키며 p53 돌연변이를 포함한 유전자 불안정을 유도하는 것으로 알려져 있다. 본 연구는 위암의 전구 돌연변인 위이형성에서 H. pylori 감염과 p53 단백의 발현과의 관계를 조사함으로써 위암 발생에 있어서 역할을 연구하였다.

199년 1월부터 1998년 6월까지 연세대학교 의과대학 세브란스 병원에서 내시경적 용종절제술, 점막절제술과 수술을 시행 받고 위이형성으로 진단 받은 환자를 대상으로 위이형성의 여러 임상 및 병리적 특징과 H. pylori 감염 및 p53 감염의 발현과의 관계를 비교

분석하였다.

1. 위이형성 환자 82명과 H. pylori 감염의 대조군 192명의 연령 분포는 각각 평균 61.9±10.4세와 60.6±9.7세이었고, 남녀 수는 위이형성 환자에서 각각 49예, 33예이었고 대조군은 93예, 99예이었다.

2. H. pylori 감염률은 위이형성 환자에서 67.2%, 대조군에서 52.6%로 위이형성 환자에서 유의하게 높았다(p<0.05).

3. p53 단백의 발현률은 위이형성에서 67.1%이었고, H. pylori 감염 양성에서 62.8%, 음성에서 81.0%로 두 군간의 유의한 차이는 없었다.

4. 고등급 및 암종변화된 이형성에서 p53 단백의 발현률은 79.3%로 저등급 이형성의 60.4%에 비하여 높은 경향을 보였다.

5. p53 단백의 발현률은 위이형성의 크기가 1.5 cm 미만일 때 54.1%, 1.5cm이상일 때 77.8% 위이형성이 1.5 cm보다 큰 경우 유의하게 높았다(p<0.05).

6. H. pylori 감염과 이헝성의 분화정도, 크기, 위치 및 다발성 사이에는 통계적으로 유의한 차이는 없었다.

이상의 결과로 H. pylori 감염의 유무에 따른 p53 단백의 발현과 위이형성의 여러 임상 및 병리적 특징과 유의한 차이는 없었다. 위이형성 환자에서 대조군보다 H. pylori 감염률이 유의하게 높았고, 고등급 및 암종변화된 이형성의 p53 단백의 발현률이 높은 경향이

었으며, 위 이형성이 1.5 cm보다 큰 경우 p53 단백의 발현률이 유의하게 높았다.

Gastric dysplasia is considered the most advanced pre-invasive lesion in the multistep morphogenesis of intestinal type gastric cancer. Helicobacter pylori(H. pylori) infection and mutation of p53 tumor suppressor gene has recently been debated to be important factors of gastric carcinogenesis. There has been many studies to uncover the precise mechanism of how H. plori infection and p53 immunoexpression is related to gastric dysplasia but it is still unknown until now. I had studied the relationship of H. plori infection and p53 immunoexpression and

tried to discover some significance in clinicopathologic factors such as grade, size, site, multiplicity in gastric dysplasia. 82 patients who were diagnosed as gastric dysplasia underwent endoscopic polypectomy or mucosal resection or operation in Ihe Severance Hospital, Yonsei University College of Medicine, Seoul,

Korea from January, 1994 to June, 1998 were examined by endoscopid biopsy of normal gastric tissue and the rapid urease test(CLO**TM; DeltaWest, Melbourne, Western Australia) for H. pylori positivity and p53 immunohistochemical stain for p53

positivity. These data were analyzed for the comparison with the clinicopathologic characteristics of the gastric dysplasia.

1. The sex distribution of the dysplasia Patients were 49 males and 33 females, and the mean age was 61.9 years old.

2. H. pylori positivity of gastric dysplasia and control was 67.1% and 52.6% respectively (P<0.05). H. pylori positivity was significantly high in gastric dysplasia compared with the control group.

3. p53 positivity of gastric dysplasia was 67.1%, and was not significantly different in the H. pylori positive patients (61.4%) compared with the H. pylori negative patients (81.0%).

4. p53 positivity in low grade dysplasia us high grade and carcinoma transformation were 60.4% us 79.3%, P53 positivity of high grade and carcinoma transformation showed the tendency for high positivity.

5. p53 positivity of the dysplasia greater than 1,5 cm in diameter was significantly higher than the dysplasia lesser than 1.5 cm in diameter(p<0.05).

6. Age, sex, location and multiplicity of gastric dysplasia were not related to p53 positivity and H. pylodri positivity.

H. pylori infection was significantly high in gastric dysplasia compared with the control group , and p53 immunoexpression was more frequent in high grad dysplasia with or without carcinoma transformation. p53 immunoexpression in the dysplasia greater than 1.5 cm in diameter was significalltly higher. However, there was no significant relationship of H. pylori infetion and p53 immunoexpression in gastric dysplasia. Further study is necessary for investigating pathogenesis of H. pylori related gastric carcinogenesis of these factor.
