관찰법을 이용한 산업간호사의 업무분석

산업간호사는 건강문제를 가진 대상자에 대한 직접간호의 제공뿐 아니라 사업장과 근로자의 건강을 위한 다양한 보건사업의 계획 ·수행 ·평가를 담당함으로써 보건관리자로서의 역할을 수행하고 있다.

산업간호사가 전문성을 보장한 질 높은 간호서비스를 제공하기 위해서는 산업간호에 대한 업무표준이 설정되어야 하고 업무의 표준화를 위해서는 업무의 분석이 선행되어야 한다.

이에 본 연구에서는 사업장 건강관리실에서 근무하는 산업간호사가 실제로 수행하고 있는 업무활동을 관찰법을 이용하여 조사 분석하였으며 이를 바탕으로 산업간호사의 업무내용의 틀을 제시하였다.

연구의 진행과정은 Conn 등(1996)이 제시한 업무분석의 진행단계를 따랐으며 관찰기록 지침서를 작성하여 관련 전문가에게 의뢰하여 1차 수정하고 예비자료수집을 통해 2차 수정하였다.

자료수집은 제조업, 운수업, 운수서비스업, 유통업의 사업장에 근무하는 산업간호사 4인을 대상으로 5일 또는 6일 동안의 업무활동을 관찰하고 기록하였으며 자료수집기간은 1998년 4월 6일부터 5월 12일 까지었다.

자료분석은 관찰 기록된 내용을 새로 작성한 업무활동 분석표에 옮겨 기록한 후 업무활동을 범주화하고 업무활동의 빈도와 소요시간을 산출하고 총 관찰기록시간에 대한 비율을 분석하였다.

본 연구의 결과를 요약하면 다음과 같다.

1. 4개의 사업장에 각각 근무하는 산업간호사의 업무활동을 관찰 분석한 결과 치료 및 검사(18.58%)가 그 수행시간 비율이 가장 높았으며 상담촉진 대화(7.03%), 통계(6.89%), 내소자 기록(5,09%), 건강관리실 정리(3.21%) 등이 주요업무활동으로 나타났다. 이어 전

문직개발 활동(3.16%), 건강유소견자 관리(3.00%), 업무일지 작성(2.60%), 상담(2.41%) 등의 활동이 많았으며 국민연금 업무는 한 개의 사업장에서 13.42%로 높은 활동비율을 보였으나 다른 사업장에서는 관찰되지 않았다.

2. 이 밖에도, 타부서간 협조, 업무보고 및 결재, 물품 구매 및 지급, 구급함 관리, 보건교육 준비, 법적 서류작성, 작업장 순회, 소속부서관련 업무, 동료간호사와 논의, 보건사업 홍보, 연금·보험 업무, 각종 정보의 검토, 학생실습준비, 전화업무 등의 업무활동을 관찰할 수 있었다.

3. 치료/검사/상담/기록에 소요되는 시간비율은 26.07%이고 내소 1회당 평균소요시간은 4.04분, 관계형성 대화시간을 포함하여 내소자 관리에 소요되는 시간은 32.16%이며 내소 1회당 평균 4.52분이 소요되는 것으로 나타났다.

4. 관찰 기록된 4개 사업장의 산업간호사 업무활동을 근거로 하여 산업간호사의 업무내용의 틀을 8개 업무영역 ; 건강증진, 교육 및 상담, 건강진단 업무, 치료관련 업무, 작업환경관련 업무, 관리 및 행정 업무, 기록, 확대된 간호활동으로 제시하였다. 제시한 8개

의 업무영역에 따라 본 연구에서 관찰된 산업간호사의 업무활동의 비율을 살펴보면, 관리 및 행정 업무(33.59%), 치료관련업무(24.91%), 교육 및 상담(15.18%), 기록(11.15%), 확대된 간호활동(7.88%), 건강진단업무(4.28%), 작업환경관련업무(1.74%), 건강증진(1.27%)

의 순으로 나타났다.

이상의 연구결과를 볼 때 전문성이 보장된 산업간호사로서의 위상정립을 위해서는 근로자에 대한 상담과 교육의 비중을 높이고 질병예방 및 건강증진을 위한 건강프로그램의 개발 및 실행에 많은 관심을 가져야 할 것이다. 그리고 다양한 연구방법을 이용한 업무분석 의 반복연구를 통해 산업간호사의 업무표준을 수립하여야 할 것이다.



[영문]

Occupational health nurses(OHNs) perform the role of health managers not only through direct care to clients but also through various health managing activities. In order to offer high quality nursing service guaranted by the OHN's specialty, a work stand for be established and job analysis must be done.

This descriptive case analysis study was done to investigate the daily work performance activities of the OHN and to present a frame for a job description. Direct observation method was used far data collection.

The study process fallowed the steps of job analysis described by Conn et al.(1996). A guideline far observation and recording was constructed by the researcher, It was modified f77t by experts, and the? secondly, through pilot data collection and discussion.

Data were collected on four nurses who worked in industrial settings located in Seoul and Inchon. It was done between April 6 and May 12, 1998 by two observers over five or six days for each OHN.

Descriptive statistics, frequency and percentage distribution were used to analyze the data.

A summary of the study results is as follows:

1. Major work activities were found to be treament and

testing(18.58%),conversations to promote counseling(7.03%), statistics(6.89%), recording about client care(5.09%), office management(3.21%), activity to develop OHN professional speciality (3.16%), management work for health problems(3.00%),

work diary recording(2.60%), and counseling(2.41%).

2. Other activities included cooperative work with other departments, reporting and settling accounts, purchasing goods and supplies, first-aid kit control, preparation for health education, drawing up legal documents, work site rounds, other duties in one's department, discussion with colleague nurses, public information about health projects, penison and security duties, reviewing various information, preparation for student training, and telephone work

3. The amount of time spent for treatment and testing/counseling/recording was26.07% of the total and on an average the OHNs spent 4.04 minutes for each client. The amount of time far client management(conversations included) was 32.16%

and on an average the OHNs spent 4.52 minutes far each client.

4. This investigator presented a frame far the job description which consisted of eight job domains including their apropriate activities; health promotion education and counselling, health examinations, tratments, work environment management, management and administration recording, and extended nursing activities. The rates of time spent were; management and administration(33.59%),treatments(24.9177), education and counselling(15.18%), recording(11.15%), extended nursing activities(7.88%), health examinations(4.2875), work environment management(1.74%), and health promotion(1.27%).

In conclusion, in order to contribute to the development of OHN as a professional speciality, it is important to increase the amount of time spent in education and counseling, and to increase the concern far the development and practice of health programs in prevention and health promotion.

Although the results of this study provide basic information to set up work standards for OHNs, investigation is needed to further clarify the framework.
