제주도 주민에서 브루셀라 감염에 관한 혈청역학적 조사

브루셀라증은 호기성 그람 음성 구간균인 브루셀라에 의해 발생하는 인수공통 질환이다. 전세계적으로 발생하나 특히 지중해 연안국가, 중동국가, 중남미에서 많이 발생하고 있다. 주요 전파양상은 경구감염이며 동물과의 접촉이 많은 사람에서는 직접 접촉에 의해서도 감염된다. 국내에서의 브루셀라증에 대한 발생보고는 1939년 경성 거주 일본인에서 1예 보고된 이후로는 전혀 없는 실정이나 1986년 목축업자 등을 대상으로 한 혈청학적 조사에서 일부 항체양성자들이 발견되어 국내에도 브루셀라증이 존재할 가능성이 제시되었다. 그러나 국내 가축에서의 브루셀라증은 1980년대 중반 이후로 제주 지역을 중심으로 소에서 급격히 증가하였고 최근에는 경기지역에서도 많이 발생하고있다. 이에 본 연구자는 국내에서 감염의 위험성이 가장 높은 지역인 제주도 주민을 대상으로 브루셀라에 대한 항체 보유상태를 조사하여 브루셀라 감염실태를 알아보고자 하였다.

제주도 각 지역의 총 2,372명으로부터 채취한 혈액을 대상으로 신속평판응집시험(rapid slide agglutination test)을 시행하여 항체역가 1:80에서 양성을 보이는 검체를 선별하였다. 양성혈청은 다시 표준시험관응집시험(standard tube agglutination test)을 시행하여 항체역가 1:80 이상에서 양성을 보인 검체만을 양성으로 판독하고 이들의 항체역가를 측정하여 다음과 같은 결과를 얻었다.

1. 전체 조사대상자 2,372명중 응집항체가 1:80 이상을 보인 검체는 모두 14건으로 전체 항체양성률은 0.59%이었다.

2. 남자의 항체양성률은 조사대상자 1,158명중 4명으로 0.34%이었고, 여자의 항체양성률은 조사대상자 1,214명중 10명으로 0.82%이었다. 성별 항체양성률의 차이는 통계학적으로 유의하지 않았다(p>0.05).

3. 연령별 항체양성률은 40대 이후의 중장년층에서 높은 경향을 보였다.

4. 지역별 항체양성률은 남제주군이 722명중 7명으로 0.97%이었고 북제주군이 625명중 4명으로 0.64%, 제주시가 659명중 3명으로 0.46% 서귀포시는 366명 모두 항체음성 이었다.

5. 항체양성자 14명의 항체역가는 1:80에서 7명, 1:160에서 4명 그리고 1:320에서 3명이 양성이었다.

6. 항체양성자 14명의 직업은 농업 7명, 축산업 3명, 서비스업 1명, 승마장 1명, 식품제조업 1명이었고 1명은 확인할 수 없었다.

7. 직업이 확인된 1,760명중 축산업자, 농부, 수의사, 유제품 가공업자 등 감염노출의 위험이 밝혀진 군에서의 항체양성률은 1,167명중 11명으로 0.94%였고 노출 위험이 밝혀지지 않은 군에서의 항체양성률은 593명중 3명으로0.51%였다. 노출위험에 따른 항체양성률

의 차이는 통계학적으로 유의하지 않았다(p>0.05).

본 연구결과로 가축과의 접촉이 많은 제주지역에서 생각보다 많은 브루셀라 감염이 존재함을 알 수 있었고, 따라서 실제 임상 환자의 발견을 위한 노력이 필요하리라 생각된다. 또한 국내 다른 지역의 주민을 대상으로 한 광범위한 조사가 필요할 것으로 사료된다.

Brucellosis is a zoonosis caused by gram-negative coccobacilli of the genus Brucella. It exists worldwide, especially in countries of the Mediterranean basin, the Arabian gulf, the Indian subcontinent, and parts of Mexico and Central and South America. Transmission to human usually occurs by ingestion of unpasteurized milk or dairy products from infected animals or by direct contact with infected animals. In Korea, Japanese who lived in Seoul in 1938 was reported as a first human brucellosis patient. After then there has been no reported case of human brucellosis in Korea. In 1986, a serologic surrey was performed among several people including stock breeders and veterinarians. Some seropositive persons were found in that study and presence of human brucellosis in our country was suggested. While human brucellosis is very rare, there has been much increase in bovine brucellosis especially in cattle of Cheju island since mid 1980's. So we performed this study to examine the prevalence of anti-Brucella antibody among People in Cheju island who were in relatively high risk of acquiring Brucellosis in Korea.

Total of 2,372 residents in Cheju island was tested. All sera were screened by rapid slide agglutination test to detect anti-Brucella antibody and serum having antibody titer of 1:80 were considered positive. Then standard tube agglutination test was performed to confirm the positive reaction and final antibody titer was determined.

1. Among 2,372 sera tested, total of 14 sera showed positive reaction at the antibody titer of 1:80 or more. The overall seropositive rate was 0.59%.

2. Positive rate in males was 0.34% and positive rate in females was 0.82%. There was no significant difference between males and females in seropositive rate(P>0.05) .

3. Seropositive rate was slightly higher in persons with 40 years old or older.

4. Seropositive rates by area of Cheju island were as follows: South Cheju-gun 0.97%, North Cheju-gun 0.64%, Cheju city 0.46% and Sogwipo city 0.0%.

5. The final antibody titer of seropositve sera was determined: 7 sera were positive at 1:80, 4 sera at 1:160 and 3 sera were positive at 1:320.

6. Occupation of the seropositive persons showed that 7 persons were farmers, 3 Persons were stack breeders, 2 persons were engaged in service trade and 1 person was working at horse-riding house. Occupation of one person was unknown.

7. Seropositive rate among people who were known to be in relatively high risk of brucella infection was 0.94%. Se개positive rate of people who were not Proved to be in risk of Brucella infection was 0.51%. There was no significant difference between two groups in seropositive rate(P>0.05).

These findings suggest that Brucella infection exists in Cheju island and seropositive rate was higher than we expected. So we should try hard to find the clinical case of human brucellosis. Also, seroprevalence of brucellosis in other part of the country should be investigated.
