가정간호수가의 적정성 평가 연구

1994년부터 시작된 병원중심 가정간호 시범사업의 수가는 방문당 수가로서 기본방문료, 교통비 그리고 개별행위료로 구성되어 있으며 보험적용에 있어서는 입원요율을 적용하고 있다. 이에 가정간호 수가구조가 가정간호의 특성을 옳게 반영하고 있고 수가산출이 타당한지 그리고 가정간호수가가 실제 소요비용을 잘 반영하고 비용-효과가 있는지의 적정성을 검증하고자 본 연구를 시도하였다.

연구 대상은 1994년 9월 1일부터 1996년 8월 31일까지 일대학병원 가정간호 사업소에 등록되어 관리된 가정간호대상자 239명 전수와 가정간호사 2명을 대상으로 하였다. 자료수집은 가정간호사 일일활동업무내용과 환자별 간호행위 제공시간, 가정간호대상자의 의

무기록분석, 사업소의 총수입과 지출, 항목별 진료비용을 수집하였다.

연구의 결과를 요약하면 다음과 같다.

1. 우리나라 가정간호사업의 특성은 병원중심 가정간호사업으로 1년간의 직무교육과정 이수후 자격증을 취득한 가정간호사가 뇌혈관질환, 만성호흡기질환등 장기적이며 복합적인 진단명을 가지고 있으며 의료기구나 구체적인 처치가 필요한 환자에게 간호서비스, 위임된 진료서비스, 교육서비스 그리고 사회사업 서비스의 포괄적인 서비스를 제공하는 것으로 나타났다. 그리고 가정간호업무 구성은 일일근무시간중 환자방문 23.3%, 교통시간 20.3%로 총근무시간의 43.6%를 사업소밖에서 사용하며 사업소운영업무, 환자관련업무, 학생교육 및 집단교육 등 사업소내 활동이 53.6%를 차지하고 있었다.

2. 현행의 가정간호수가구조는 가정간호업무에 소요되는 시간구성에 대한 특성은 반영되었으나 가정간호서비스 특성에 대한 반영은 미흡한 것으로 나타났다. 가정간호서비스에서 교육서비스는 환자와 가족의 독립적 기능 유지를 위해 꼭 필요하고 많은 시간이 요구되므로 기본방문료에 내포되어 있는 교육에 대한 수가 부분은 분리하여 별도로 수가화하는 것이 필요하다. 그와 함께 가정간호 수가산출 전제조건 검증 결과 기본방문료 산출을 위한 인건비와 교통비는 원가를 제대로 반영하지 못하는 것으로 나타났다.

3. 가정간호수가의 적정성 검증에서 가정간호수가가 가정간호사의 인건비와 교통비의 원가를 제대로 반영하지 못하고 있는 문제점을 가지고 있지만 가정방문횟수가 증가하면 가정간호운영에 소요되는 비용을 보상하고 입원비용에 비해 부담이 적은 것으로 나타났다.

결론적으로 현행의 가정간호 수가구조는 교육서비스에 대한 수 반영이 요구되며 병원과 환자측면에서 비용-효과가 있는 것으로 볼 수 있다.

그러므로 교육서비스를 수가화하기 위하여 교육에 소요되는 시간, 교육내용 및 수준 등에 관한 연구가 요구된다. 그와 함께 가정간호사업이 진료기관에 국한되지 않고 공공기관이나 간호사단체중심의 가정간호기관으로 확대될 것에 대비하여 이러한 형태의 가정간호사업에 대한 가정간호수가 적용의 연구가 필요하며 가정간호행위의 독립성을 위하여 정부에서 추진하고 있는 행위별 상대가치체계에 맞추어 가정간호행위의 상대가치체계개발을 위한 연구수행을 제언한다.

[영문]

The first demonstration project of hospital-based home health care service in Korea started in September 1994. The payment system for home health care was composed of three kinds of fee ; service fee, travel fee and fee-for-service.

The purpose of this study was to evaluate the cost structure and cost-effectiveness based on the characteristics of home health care service.

The data was collected from the Home Health Care Center at Yonsei University Medical Center during the first demonstration period, from September 1994 to August 1996.

The results have been summarized as follows:

1. The diagnoses of referred patients consisted mainly of long-term diseases like cancer, cerebrovascular disease, chronic pulmonary disease, eta., and included mixed diseases. They also required specific treatments and medical appliances. The home health care service was composed of nursing service, medical service, educational service and social service. Almost 44% of the total working hours of home health care nurses was taken up with hom-visiting and transportation.

2. The payment system for home health care reflected the composition of working hours, but not the characteristics of services in full. Education is important to home health care service for maintaining the independent function of a patient and

required much time. So it is necessary that the fee for educational service seperate from the service fee.

3. The cost of home health care is not properly reflected the prime costs of labor and transportation.

4. Despite the insufficient calculation of cost, the Home Health Care Center had a net financial gain during the demonstration period. It is assumed that the increase in the frequency of home-visiting was the main cause.

In conclusion, it is necessary that the fee for educational service be added to the cost structure of home health care.

The results of this study suggest that another study is required to calculate the fee for educational service and to apply the payment system of home health care not only to public home health care but also to snf-based home health care.
