B형 간염 후의 다양한 임상경과는 감염 후 나타나는 간세포의 손상이 바이러스에 의하여 직접 유발되는 것이 아니고 감염된 간세포에 의한 숙주의 면역반응에 의한 것으로 알려져 왔고 숙주의 면역 반응 조절에 관여하는 사람주요조직 적합항원(HLA)이 중요한 역할을 할 것으로 추측되었으며 HBV의 지속적 감염상태에서 벗어나는 데 있어서 유전적 요소가 중요하다고 보고된 바 있다. 그러나 우리 나라에서는 아직까지 HBV 감염 후 숙주의 면역반응에 의한 HBV의 제거와 HLA type의 관련성에 대한 뚜렷한 보고가 없다. 따라서 연구자는 세브란스병원에서 이식을 위해서 HLA 표현형(phenotype)을 검사했던 환자의 자료를 후향적 방법으로 조사하여 한국인에서 HBV의 감염지속 및 항체형성여부와 HLA-DR type의 연관성이 있는지를 알아보고자 하였다.

본 연구는 1990년 1월부터 1997년 8월까지 신장이식을 위해 연세의대 세브란스 병원에 내원한 한국인 환자 및 건강한 신장공여자들로 총 1372명을 대상으로 하였으며 이 중 간염 바이러스 표지자가 모두 음성(이하 비노출군)이 946명이었고 HBV 감염 후 HBsAg이 양성(이하 만성보균자군)인 83명, HBsAg이 음성이고 anti-HBs가 양성(이하 항체형성군)인 343명이었고 항체형성군 중 anti-HBc가 양성(이하 자연회복군)은 243명이었다. 또 건강한 신장공여자들만에서도 위와 같이 각 군을 구분하고 각 군 및 말기신부전증환자들과 서로 비교하였다.

1. 총 연구대상자는 1372명으로 남자 924명, 여자 448명으로 남녀비는 약 2:1 이었고, 평균 연령은 각각 37.4세 및 38.3세 이었다. 총연구대상자 중 건강한 신장공여자들은 총 424명으로 남자 259명, 여자 165명 이었고, 평균 연령은 각각 39.5세 및 39.1세 이었다.

2. 각 군의 HLA-DR type의 빈도를 살펴보면 총 연구대상자에서 DR4는 가장 높은 빈도를 보였고 DR5가 그 다음이었다. 각 군에서는 공통적으로 DR4가 가장 높은 빈도를 보였고 비노출군에서는 DR5가, 만성보균자군에서는 DR9가, 자연회복군과 항체형성군에서는 DR6이 두 번째로 높은 빈도를 보였다. 건강한 신장공여자들에서는 비노출군에서 DR6이 두 번째로 높은 빈도를 보이는 것을 제외하고는 위와 같은 빈도분포를 보였다.

3. HBV 감염 후 만성보균자군과 자연회복군간의 비교시 자연회복군에서는 DR6이, 만성보균자군에서는 DR9가 상대군보다 유의하게 높은 빈도를 보였다(각각 p<0.001, RR=3.72, p<0.001, RR=0.33). 만성보균자군과 항체형성군간의 비교시에서도 위와 마찬가지 결과를 보였다(각각 p<0.001 RR=3.58, p<0.001 RR=0.32). 이는 건강한 신장공여자들에서도 같은 결과를 보여주었다.

4. HBV 감염 후 자연회복 및 항체형성과 관련될 것으로 생각되는 DR6은 DRl3과 DRl4로 분리되는데 이 중 DRl3에서 좀더 유의한 통계적인 차이를 보여주었다(p<0.01).

이상의 결과에서 한국인에서 HBV 감염 후 만성보균자군에서는 HLA-DR9의 빈도가 높았고 HLA-DR6의 빈도가 상대적으로 유의하게 낮았으며, 자연회복군과 항체형성군에서는 HLA-DR6(특히 DRl3)의 빈도가 상대적으로 유의하게 높았던 것으로 미루어 보아 HBV 감염 후 HBV 제거와 항체형성에 숙주의 유전적 요소로서 MLA-DR type이 일부 연관될 것으로 생각된다.

[영문]

The course after hepatitis B virus infection appears not to be related to variations in virulence of the HBV itself and may be influenced by the host immune response, which in turn may be regulated by the major histocompatibility complex. HLA has been emphasized as a genetic factor to eliminate HBV. But, so far, there was no conclusive relationship between HLA and the course of HBV infection in Korean. We, therefore, studied Korean who visited Yonsei University Medical Center for renal transplantation to determine whether the clearance of HBV was related to a particular HLA allele.

In our study, there were total 1372 subjects including 946 persons who have not been exposed to HBV(Non-exposures), 83 persons who have persistent HBsAg after HBV infection(Carriers), and 343 persons who developed antibodies to HBV or recombinant HBsAg(Immune responders). Of 343 persons in the group of Immune responders, 243 persons were who recovered from HBV infection(Clearances). We classified the healthy donors(total 424 subjects) into several subgroups in the same way as above and compared MLA types among each other, also compared the healthy donors with end stage renal disease patients.

1. Among 1372 subjects, 924 were male and 448 were female(Mean age, M:37.4yrs, F:38.3yrs), Healthy donors in total subjects were 424(259 in male, 165 in female, and 37.4 yrs, 38.3yrs of mean age, respectively).

2. Commonly, HLA-DR4 was most frequent among the total subjects, also in the group of Non-exposures, Carriers, Clearances, and Immune responders. The second frequent HLA-DR type was DR5 among the total subjects and in the group of Non-exposures, DR9 in the group of Carriers, and DR6 in the group of Clearances and Immune responders. Results in healthy donors were same except the frequency of DR6 in the group of Non-exposures.

3. HLA-DR6 was significantly frequent in the group of Clearances and Immune responders compared with the group of Carriers(p<0.001, RR=3.72 and p<0.001, RR=3.58, respectively). HLA-DR9 was significantly frequent in the group of Carriers compared with the group of Clearances and Immune responders(p<0.001, RR=3.33 and p<0.001, RR=3.32). Results in healthy donors were same as above.

4. HLA-DRl3 was significant between the HLA-DR6 subgroups(DRl3, DRl4) which may influence the clearance of HBV.

In conclusion, HLA-DR6(esp, HLA-DRl3) may be associated with elimination of HBV as one of the host factors influencing the immune response to HBV and HLA-DR9 with persistent HBV infection.
