1980년대 중반 이후 후천성 면역 결핍 증후군 및 항암 화학 요법, 면역 억제요법의 확산으로 인해 다시 결핵의 유병율이 증가하고 있는데, 이러한 환자에 있어 신속한 치료가 필수적이기 때문에 조기 진단의 필요성도 증대되고 있다. 지금까지 임상 검체로부터 균을 검출하는 것이 결핵의 진단에 있어 통상 검사로 이용되어 왔으나 객담 도말 검사의 경우 민감도가 낮고 배양 검사의 경우 검사에 소요되는 기간이 길다는 문제가 있다. 최근 새로운 배양법이나 중합효소 연쇄 반응(polymerase chain reaction, PCR)을 이용한 방법이 개발되었으나 방법와 비용의문제로 널리 이용되기는 아직 어려운 실정이다. 이전부터 비교적 저렴하고 손쉽게 시행할 수 있는 혈청학적 방법들이 시도되어 왔는데 다른 균들과의 교차반응 때문에 널리 시행되지 못하다가 1972년 enzyme-linked immunosorbent assay(ELISA) 방법이 개발되고 균중에 특이한 항원과 항체를 이용할 수 있게 되면서 객담내 결핵균 항원 및 혈청내 항결핵 항체 검출을 통한 결핵 진단법이 기존의 검사 방법에 상응하는 민감도와 특이도를 가지게 되었다. 한편, 결핵의 진단율을 높이고 다른 폐질환을 감별하기 위한 기관지 내시경의 이용도 늘어나고 있어 이번 연구를 통해 단순 기관지 세척액내 결핵관 항원의 농도와 혈청내 결핵균 특이항원에 대한 항체의 역가를 ELISA로 측정하여 각 검사법의 민감도 및 특이도를 구하여 검사의 임상적 유용성을 알아보았다.

본 연구는 1995년 1월부터 1995년 10월까지 외래에서 또는 입원 후 기관지 내시경과 함께 단순 기관지 세척을 실시하고 거의 같은 시기에 객담 및 혈청을 채취하였던 136명의 환자들을 대상으로 기관지 세척액에서 double sandwich ELISA를 이용하여 결핵균 항원의 농도를 측정하고, 혈청내의 38 kDa 항원과 결핵균 culture filtrate에 대한 항체를 ELISA로 측정하여 다음과 같은 결과를 얻었다

1.객담 배양 검사의 민감도는 68.8%, 특이도는 100%였으며, 기관지 세척액 배양검사의 민감도는 79.5%, 특이도는 100%였다.

2.단순 기관지 세척액에 대한 결핵균 항원 검사의 민감도는 29.3%, 특이도는 88.6%였다.

3.38 kDa 항원에 대한 항체 검사 결과 민감도는 38.1%, 특이도는 92.0%였으며, 결핵균 culture filtrate 항원에 대한 항체 검사 결과 민감도는 42.9%, 특이도는 89.7% 였다. 두 가지 검사를 조합할 경우 민감도는 19.0%, 특이도는 98.9%였다.

4.객담 또는 세척액 도말 음성이었던 군에서 38 kDa 항원에 대한 항체 검사의 민감도는 36,8%, 특이도는 91.9%였고, 결핵균 culture filtrate 항원에 대한 항체 검사의 민감도는 42.1%, 특이도는 89.5%였다.

이상의 결과로 볼 때 기관지 세척액내 결핵균 항원 검사는 기존의 결핵 진단법을 대치하기는 어려울 것으로 판단되나, 혈청내 결핵균 특이 항원에 대한 항체검사는 특이도가 높아 객담 도말 음성인 결핵 고위험군에서 조기에 결핵 치료를 시행할 수 있는 근거로 이용될 수 있을 것이다. 앞으로 충분한 수의 정상 대조군과 더 많은 환자를 대상으로 하고, 항체 검사가 결핵 치료의 경과를 반영할 수 있는 지표로서 이용될 수 있는지에 대한 연구가 병행되면 임상적으로 더 유용할 것이다.

Since mid-1980's, tuberculosis has been increasing in incidence, especially among immunocompromised patients such as cancer patients undergoing chemotherapy or those patients with AIDS. Early diagnosis and treatment of tuberculosis in these patients is critical. Tuberculosis can be diagnosed by direct microscopy and/or by culture of the Mycobacterium tuberculosis from body fluids or biopsy specimens. Direct microscopy after Ziehl-Neelsen staining is a rapid and useful diagnostic method to perform economically, but sensitivity is relatively low and culture of mycobacteria is time consuming. Detection of mycobacterial DNA in clinical samples by the polymerase chain reaction is highly sensitive but laborious and expensive.

Therefore, rapid, sensitive and readily applicable new tests need to be developed for the early diagnosis of M.tuberculosis infection. A serodiagnostic test for tuberculosis is inexpensive and rapid. By combining the ELISA results obtained with a set of purified antigens and monoclonal antibodies, sensitivity and specificity of diagnostic tests directed against sputum tuberculosis antigen and serum antituberculos is antibody may be increased beyond the other conventional diagnostic test. To improve diagnostic yields the use of diagnostic fiberoptic bronchoscopy is increasing. In this study, bronchial washing fluids and serum specimens of 136 consecutive patients who were admitted to medical department of Yonsei University Medical Center from January 1995 to October 1995 were collected, Mycobacterial antigens in bronchial washing fluid were detected by a double sandwich ELISA method. Affinity-purified goat antibodies to M.tuberculosis were used as capture antibody and rabbit antibodies to Mycobacterirum species as detection antibody. Serum antibodies to 38 kDa antigen and unheated culture

filtrate of M.tuberculosis were detected by an ELISA method. Smear and culture tests for acid fast bacilli were performed simultaneously. The results obtained were as follows.

1. Sensitivity of the sputum culture was 68.8%, and specificity was 100%.Sensitivity of the bronchial washing fluid culture was 79.5%, and specificity was 100%.

2. Sensitivity of the antigen capture assay for bronchial washing fluid was38,1%, and specificity was 92.0%.

3. ELISA with 38 kDa antigen showed a sensitivity of 38.1%, and the specificity was 92.0%. Sensitivity of the antibody detection test using culture filtrate of M.tuberculosis was 42.9%, and specificity was 89.7%.

4. In smear-negative patients, sensitivity of the ELISA using 38 kDa antigen and culture filtrate was 36.8% and 42,1%, respectively. The specificity was 91.9% and 89.5%, respectively.

According to these results, antigen capture assay for bronchial washing fluids cannot substitute traditional diagnostic tests, but the serodiagnostic test for 38 kDa antigen and culture filtrate shows high specificity, Therefore, serodiagnostic test for tuberculosis would justify antituberculous therapy to be started in high-risk patients before the results of culture studies are known
