급성 골수구성 백혈병 줄기세포에서 mTOR 발현의 임상적 의의

급성 골수구성 백혈병(AML) 세포에서는 종양 세포의 증식과 고사를 조절하는 여러 가지 신호전달 경로들이 활성화 되어 있다. 그 중에서도 중심 역할을 하는 mammalian target of rapamycin (mTOR) 및 Ras homolog enriched in brain (Rheb) 경로는 백혈병의 치료와 연관하여 많은 관심을 모으고 있으나, 실제 임상 경과와의 연관성에 대해서는 명확히 규명되어 있지 않다. 본 연구에서는 백혈병 아세포 및 줄기세포를 대상으로 mTOR 및 Rheb의 활성도를 유세포 분석법을 이용하여 측정하고 그 임상적 의의를 밝히고자 하였다.급성 골수구성 백혈병 세포주 5가지와 AML 환자 15명의 백혈병 아세포에서 mTOR와 Rheb의 활성도를 각 물질이 인산화 되어 있는 세포의 빈도로 측정하였다. 세포주에서는 mTOR 활성세포의 빈도가 평균 92.8%였으며 Rheb에서는 79.1%였다. 환자의 백혈병 아세포 (CD33+)에서는 mTOR 5.9%와 Rheb 모두 정상 세포에서의 활성도에 비하여 유의하게 높았다 (p<0.001). 이 환자군을 완전 관해에 도달한 군(CR군)과 불응성 반응을 보인 군(non-CR군)으로 분류하여 mTOR와 Rheb의 활성도를 비교해 본 결과, 두 군의 CD33+mTOR+ 및 CD33+Rheb+ 세포 빈도 모두 통계적 차이가 없었다 (p=0.263, p=0.213).백혈병 줄기세포를 대상으로 한 실험에서는 AML 환자 17명의 진단 및 관해 시 채취한 골수에서 CD34+CD38- 세포 내 mTOR의 인산화 빈도를 측정하였다. 측정된 CD34+CD38- 세포의 빈도는 19.58% (0.3%-81.6%)이었으며, 그 중 mTOR+ 세포가 차지하는 분율 (mTOR+/CD34+CD38- fraction)은 10.31% (0.6%-54.1%)였다. 전체 세포에 대한 CD34+CD38-mTOR+ 세포의 빈도는 2.52% (0%-30.7%)였다. 동시에 CD34+CD38+ 세포에서도 mTOR의 활성도를 측정하였고, mTOR+/CD34+CD38- 분율은 65.9% (0% -99.1%), CD34+CD38+ mTOR+ 빈도는 1.4% (0%-17.3%)였다. 백혈병 줄기세포에서 mTOR 발현 양상의 임상적 의의를 살펴보기 위하여 CD34+CD38-mTOR+ 빈도의 중앙값 (0.48%)을 기준으로 낮은 군과 높은 군으로 분류하여 비교하였다. 1차 관해 유도 치료에 불응성 반응을 보인 환자수는 줄기세포의 mTOR 활성 빈도가 낮은 군에서 1명 (12.5%), 높은 군에서 3명 (32.3%)이었으며 (p=0.576), 첫 번째 완전 관해 기간은 각각 117일과 94일로 낮은 군에서 오래 유지되는 경향을 보였다 (p=0.079). 생존 분석에서는 무병생존율이 각각 72.9%, 41.6%, 전체 생존율이 48%, 0%로 낮은 군이 높은 군에 비해 우월하였다 (p=0.31, p=0.041). 이 환자군의 임상경과 중, 관해 유도 치료 전후의 CD34+CD38-mTOR+ 빈도는 감소하는 경향을 보였으나 통계적 유의성은 없었다 (p=0.326).본 실험에서는 진단 시의 AML 줄기세포 내 mTOR 활성도는 관해 유지 및 생존과 관련이 있었으며 치료 과정에 있어 예후를 예측하는 중요한 인자로 고려해 볼 수 있었다. 이를 토대로, 백혈병 줄기세포에서 비롯되는 종양의 증식에 mTOR가 중요한 역할을 함을 예상할 수 있으며, mTOR 활성을 조절함으로써 백혈병의 진행을 억제할 수 있을 것으로 생각된다.



[영문]Recent advances in the understanding of leukemogenesis in acute myelogenous leukemia (AML) showed that several intracellular signal transduction pathways play an important role in controlling the survival, progression and apoptosis of the cell. The mammalian target of rapamycin (mTOR) is known as the key molecule of the PI3K/Akt pathway and its inhibitor, rapamycin, has been recognized to have a potent anti-tumor effect in AML. However, mTOR is constitutively activated in most AML cases and the correlation of its expression status with clinical data has not been clearly established. I tried to assess mTOR activation not only in AML blasts but also in AML stem cells to analyze their clinical implication.The activities of mTOR and Rheb were measured in the target cell by flow cytometric analysis. The mean frequencies of activated mTOR / Rheb were 92.8% / 79.1% in AML cell lines and 85.46% / 95.6% in primary AML blasts. These were higher than the values of normal CD33+ cells (5.9% / 35.1%, p<0.001).I divided the patients into a good prognosis (CR group) and a poor prognosis group (non-CR group) to assess differences in the activated mTOR/Rheb frequencies of each group. The frequencies of CD33+mTOR+ / CD33+Rheb+ cells were not significantly different between two groups (p=0.263, p=0.213).To assess the mTOR activity in AML stem cells, paired bone marrow samples were obtained both at the time of diagnosis and complete remission from 17 patients. The frequency of total CD34+CD38-mTOR+ cell was 2.52% (0%-30.7%) and mTOR+ fraction of CD34+CD38- cell was 10.31% (0.6%-54.1%). In case of CD34+CD38+ cell, mTOR+ fraction was 65.9% (0%-99.1%). The frequency of CD34+CD38+mTOR+ cell was 1.4% (0%-17.3%) and it was lower than that of CD34+CD38-mTOR+ cell (p=0.595). In the subanalysis of the patients, lower CD34+CD38-mTOR+ cell group showed longer duration of complete remission (117 days vs. 94 days, p=0.079), better disease free survival (72.9% vs. 41.6%, p=0.317) and overall survival rate (48.61% vs. 0%, p=0.041). The CD34+CD38-mTOR+ cell frequencies were decreased after remission induction chemotherapy in 12 cases (70.5%). The average CD34+ CD38-mTOR+ cell frequency was decreased from 2.52% to 0.7% after the treatment (p=0.333).In conclusion, the results of this study suggest that mTOR activity in AML stem cells at diagnosis correlates with survival of the patient, thus it might be used as a possible prognostic factor. An exogenous modulation of mTOR activity might be an important modality controlling the tumor cell proliferation in AML.
