혈액종양이 있는 청소년의 경험

본 연구는 우리나라 사회문화적 맥락에서 혈액종양을 경험하는 청소년들의 경험과정을 탐구하여 이에 대한 실체이론을 개발하고자 실시되었으며, 본 연구의 연구 문제는 "혈액종양 질환 청소년의 경험은 어떠한가?"이다.본 연구에서는 Strauss와 Corbin(1998)이 제시한 근거이론방법을 사용하였으며 연구 참여자는 일 대학 부속병원에서 혈액종양 질환(Hematologic malignancies)을 진단 받고 항암치료 중이거나 항암치료가 종료된 청소년 10명이었다. 자료수집 기간은 2007년 1월부터 2007년 6월까지였다. 자료수집은 연구참여자와의 심층면접에 의해 이루어졌고 자료가 포화될 때까지 이론적 표출을 시도하였으며, 자료분석은 자료수집과 동시에 실시하였다.연구결과 개방코딩 결과 93개의 개념과 36개의 하위범주, 그리고 14개의 범주가 도출되었고, 축코딩의 패러다임에 의한 범주분석 결과 혈액종양 청소년 환자의 경험의 중심현상은 ‘이탈되어진 삶’으로 확인되면서 이에 영향을 주는 인과적 요인으로는 ‘신체적 고통’, ‘생활의 제약’, ‘자존감 저하’, ‘감정의 불안정’, ‘고립된 생활’이 확인되었고 맥락적 요인으로는 ‘환자역할만 강조하는 부모의 역할기대’, ‘사회적 편견’이, 중재적 요인으로는 ‘타인의 지지’가 확인되었고 작용’상호작용 전략으로는 ‘상황 재해석’, ‘가치관의 변화’, ‘이타심이 생김’, ‘자아성찰’이 확인되었고 그 결과 ‘자아통합’이 확인되는 패러다임 모형이 도출되었다.선택코딩 결과 확인된 핵심범주를 통하여 8개의 가설적 관계 진술문을 도출하였고, 가설에 근거하여 유형을 분석한 결과 혈액종양 청소년의 적응유형은 ‘정체형’과 ‘통합 완성형’으로 도출하였고, 핵심범주를 중심으로 조직화 하여 이론적 개요를 기술하였다.시간에 따른 혈액종양 청소년의 경험 과정을 분석한 결과 ‘자아의 혼란기’, ‘재해석기’, ‘자아 재구성기’의 3단계로 파악되었고, 이 과정은 타인의 지지와 같은 중재적 요인의 영향을 받았다.본 연구의 결과는 혈액종양이 있는 청소년의 사회심리적 적응을 돕기 위해 ‘이탈되어진 삶에서 자아통합 해나가기’라는 과정 속에서 개별적인 간호중재 전략을 개발하는 기틀을 제공해 주며, 여기서 제시된 가설적 관계 진술문들은 가설 검증을 통해 보다 풍부한 지식체의 확장에 기여할 것이다.

[영문]The main purpose of this study was to observe the experience process of the adolescents with Hematologic malignancies, and to develop a substantial theory on it. The question for the study is "What is the experience of adolescents with Hematologic malignancies like?"The research method used in this study was the Grounded Theory Method developed by Strauss & Corbin (1998), and the participants for this study were 10 adolescents who had been on and off chemotherapy in one university hospital. The data collection period was from January, 2007 to June, 2007. The data were collected through in-depth interview with the participants, and theoretical samplings were tried until the data reached saturation. Data collection and data analysis were performed simultaneously.For the result of the study, 93 concepts, 36 subcategories and 14 categories were derived from the open coding. From the category analysis based on the axial coding, it was verified that the central phenomenon of adolescents with Hematologic malignancies is "a deviated life" and the causal conditions that influence this phenomenon are "physical sufferings" "restriction of life" "decline of self-esteem" "emotional instability" "isolated life", and the contextual conditions are "expectations of parents who only emphasize the role of patients", the intervening conditions are "support from others", the action/interaction strategies are "reinterpretation of the situation" "change of value" "generation of other-centered mind". From this observation, a paradigm model in which "self integration" is verified has been derived from this observation.8 theoretical relative statements were derived from the core category with the result of the selective coding, and two adjustment types-"stagnation" and "integration"-were derived with the analysis of the types of adolescents with Hematologic malignancies based on the theory. A theoretical outline is described with the core category as the center.The experience process is divided into 3 steps: "the period of self-confusion" "the period of reinterpretation" "regeneration of self", and this process is influenced by intervening conditions such as others’ support.The result of this study provides a frame for individualized nursing intervention strategies in helping the psychosocial adaptation of adolescents with Hematologic malignancies through the process "from the deviated life to self-integration". The theoretical relative statements will contribute to expansion of knowledge through the verification of hypothesis.
