외상을 경험한 군 병사들의 스트레스 반응 개념개발

Abstract

[한글]본 연구는 ‘외상을 경험한 군 병사들의 스트레스 반응은 무엇인가?’라는 개념적인 질문을 제기하면서 외상 후 스트레스 반응 개념의 속성과 명확한 정의를 규명한 서술적 연구이다. 연구방법은 Schwartz-Barcott와 Kim(1993)이 개발한 개념 개발을 위한 혼종모형을 적용하였다.자료수집 기간은 2005년 5월 2일부터 8월 27일 까지였으며 연구 대상자는 외상 후 스트레스 장애 진단을 받고 입원한 대상자와 외상 후 스트레스 장애로 진단 받은 후 군 병원 정신과 외래를 통해 치료를 받고 있는 4명의 병사들이었다. 자료수집 방법은 면담과 참여관찰이었고 면담내용은 녹음을 원칙으로 하였으며 개방형 질문을 사용하여 연구 대상자가 자신의 경험과 느낌을 이야기 하도록 하였다. 면담이 끝난 후 녹음된 자료와 연구자의 참여관찰 내용은 즉시 기록으로 옮겨 자료를 분석하였다.자료 분석은 혼종모형에서 제시하고 있는 세 단계에 따라 진행되었으며 이론적 단계에서는 간호학뿐만 아니라 타 학문의 문헌을 고찰하면서 광범위하게 외상 후 스트레스 반응 개념의 속성과 정의를 분석하였다. 현장 작업 단계에서는 각 사례별로 참여 관찰한 자료와 심층면담 자료를 삽입하였고 이 자료에 나타난 외상 후 스트레스 반응의 속성을 분석하였으며 현장에서 관찰된 외상 후 스트레스 반응의 정의를 제시하였다. 최종 분석 단계에서는 문헌과 현장에서 파악된 외상 후 스트레스 반응의 속성을 비교, 외상 후 스트레스 반응의 정의를 제시하였다.최종적으로 정의된 외상 후 스트레스 반응의 속성과 정의는 다음과 같다.1. 외상 후 스트레스 반응 속성이론적 단계와 현장 연구에서 규명된 각 속성을 비교, 분석하여 최종적으로 다음과 같은 외상 후 스트레스 반응 속성을 제시한다.·심리적인 충격을 주는 외상성 사건을 경험한다.·자신을 피해자로 인식하며 우울, 죄책감, 수치심을 경험한다.·외상성 사건의 재경험, 지속적인 회피, 과도한 민감성을 포함한 불안 증상을 경험한다.·참을 수 없는 분노를 경험한다.·살아가는데 필요한 전반적인 기능에 장애가 일어난다.·신체적인 증상으로 고통 받는다.·부적응적인 행동이 나타난다.2. 외상 후 스트레스 반응 정의본 연구에서 최종적으로 분석된 외상을 경험한 군 병사들의 스트레스 반응 정의는 다음과 같다.‘외상 후 스트레스 반응은 통상적으로 경험할 수 없는 극심한 외상성 사건과 관련된 심리적인 충격을 받았을 때 경험되며 자신을 피해자로 인식하는 것과 우울, 죄책감, 수치심, 외상성 사건의 재경험·지속적인 회피·과도한 민감성을 포함한 불안 증상, 참을 수 없는 분노, 살아가는데 필요한 전반적인 기능의 장애, 신체적인 증상으로 인한 고통, 부적응적인 행동으로 구성된 복합적인 현상이다.’

[영문]There is a need to define the concept of post traumatic stress response more appropriately in the context of Korean military culture. This research is an attempt to analyze and develop the concept of military soldiers' post traumatic stress response by applying the Hybrid Model suggested by Schwartz-Barcott and Kim(1993).The data were collected from May 2 to August 27, 2005. The participants of the study were four soldiers diagnosed as post traumatic stress disorder who were recruited from a military hospital, including hospitalized and ambulatory care patients. Qualitative research methods of in-depth interviews and participant observation were used for data collection. The contents of the interviews were recorded on tape. Data analysis progressed according to the three phases suggested by the Hybrid Model. For the theoretical phase, nursing and other literature were reviewed to analyze attributes and develop a working definition of the concept of post traumatic stress response. For the fieldwork phase, in-depth interview data and participant observation data were analyzed to identify the attributes of post traumatic stress response revealed in these data. Data analysis progressed at the same time as the fieldwork. Finally, a new definition of post traumatic stress response was developed.1. Attributes of post traumatic stress responseAccording to the attributes identified from the theoretical and fieldwork phase, the final attributes of post traumatic stress response in military soldiers are suggested;·experiencing psychological shock such as fear, helplessness, or horror caused by a traumatic event which threatens ones personal integrity·considering oneself as a victim and experiencing depression, guilt, and shame·experiencing anxiety symptoms including reexperiencing, avoidance /numbness, and hyper-arousal·experiencing intolerable and explosive anger·inability to carry on basic functioning for maintaining life·suffering from physical problems·developing disadaptive behaviorsIn particular, the result that the post traumatic stress response was not only an anxiety disorder but also the holistic human experience was notable in this research.2. Definition of Post Traumatic Stress ResponseThe following final definition of post traumatic stress response is suggested."Post Traumatic Stress Response" is a complex phenomenon including consideration as a victim, experience of depression, guilt, shame, anxiety symptoms, and anger, inability to function, suffering from the physical problems, and developing disadaptive behaviors that can follow the experiencing of an extreme traumatic event.
