하악 구치부에서 ITI SLA(Sand-blasted, Large-grit, Acid-etched) 임플란트를 이용한 단일 치아 수복의 후향적 임상 연구

Abstract

[한글]본 연구는 하악 대구치 부위에서 ITI SLA임플란트 시스템을 이용하여 단일 치아를 수복한 158개의 임플란트에 대해, 하악 제 1대구치 부위 단일 임플란트와 제 2대구치 부위 간의 누적 생존률을 임상적으로 평가하여 다음과 같은 결론을 얻었다.1. 총 147명에서 제 1 대구치 부위에 98개, 제 2 대구치 부위에 60개, 총 158개의 임플란트가 식립되었다. 직경에 있어서 4.8mm 이상의 장폭경을 가진 임플란트가 전체의 91.1%를 차지했다. 길이에 있어서 10mm 이상의 임플란트는 전체의 96.2%를 차지했다.2. 4 개의 임플란트에서 골이식 및 골유도재생술식이 사용되었다. 나사 풀림 현상이 SynOcta screw type abutment에서 9 개 발생하였다. 크라운 탈락은 10 개 발생하였다.3. 총 158개 중 1개 만 실패하여, 99.4%의 누적 생존률을 보였다. 제 1 대구치와 제 2 대구치의 누적 생존률은 각각 99%, 100%를 나타내었다. 이상의 결과를 볼 때, 하악 대구치에서 단일 치아 상실시, 통상의 계속 가공 의치를 대체할 만한 임상적으로 예측 가능한 우수한 치료 방법으로 고려할 수 있을 것이다.

[영문]The purpose of this study was to evaluate cumulative survival rate (CSR, %) of mandibular posterior single tooth implants replaced with ITI SLA (Sand-blasted, Large-grit, Acid-etched) implant system and compare the CSR between first and second molar. The findings from the results were as follows;1. Total of 158 implants were inserted into 147 patients. 68 patients were males, 79 patients were females and their mean age was 47.8 years. 98 implants were placed in first molar area and 60 implants were placed in second molar area. In terms of diameter, implants with wide diameter over 4.8mm dominated (91.1%). Implants with length over 10mm were used (96.2%).2. In the two cases, there was a slight transient numbness which recovered within 1-2 months. Nine SynOcta screw type abutments demonstrated screw loosening. There were ten cases of crown fallen-out from decementation.3. Only one failed out of 158 implants. The CSR was 99.4%. The CSRs for first molar and second molar were 99% and 100%, respectively.From the results, it was concluded that single tooth replacement implant in the mandibular posterior area, might be considered as the effective treatment modality comparable to the conventional crown and bridge.
