정신분열병 환자의 사회적 인지 측정도구 개발

본 연구는 정신분열병 환자의 사회적 인지 측정 도구를 개발하기 위한 방법론적 연구이다.도구 개발은 다음과 같은 과정을 통해 진행되었다. 우선 이론적 배경에 근거하여 사회적 인지의 구성 차원은 정서적 특성 차원과 사회적 단서 차원으로 분류되었다. 정서적 차원은 각성수준과 쾌수준의 2개 요인으로 분류되었다. 그리고 사회적 단서의 차원은 구체적 단서와 추상적 단서의 2개 요인으로 분류되었다.다음은 국내 TV 드라마와 영화에서 실제 대인관계 상황을 대표하고 위에 분류한 4가지 구성개념이 잘 나타나는 21 장면을 선정하였다. 선정된 장면으로부터 252개의 진술문을 작성하였고 이를 전문가 6명에게 타당도를 검정하여 18장면, 214문항으로 수정 보완하였다. 이는 다시 일반인 76명을 대상으로 문항 난이도를 표준화 하고 사회적 단서의 정서적 특성에 따른 분류 과정을 거쳐 12장면, 144문항으로 선별하였다. 선별된 장면과 문항을 기초로 하여 정신분열병 환자 30명을 대상으로 사전조사를 실시한 결과 Cronbach''s α 값이 .90이었다. 정신분열병 환자 196명을 대상으로 신뢰도와 타당도 검정을 하였는데 우선 2차에 걸친 문항분석을 통해 문항간 상관계수가 .3 이하인 문항들을 위주로 순서대로 삭제하는 단계를 거쳤다. 이상의 문항 정련과정을 거쳐 최종 8장면 80문항의 도구가 완성되었다. 첫 조사 2주후에 조사-재조사 신뢰도를 검정하였으며 전체 Cronbach''s α 값은 .89이었다. 각 구성요인별로는 구체적 단서 .78, 추상적 단서 .83으로 나타나 높은 내적 일관성을 보여주었다. 도구의 구성 타당도 검정을 위해 상관계수법을 적용하여 분석한 결과 척도의 구성요인과 전체 도구의 총점 간에는 유의한 통계적 상관관계가 보여져 도구의 구성타당도를 지지 받았다. 준거 타당도 검정을 위해서 간호 관찰척도 검사를 이용하여 사회적 단서 인지와의 관계를 분석한 결과 사회적 인지의 총점과 통계적으로 유의한 상관관계를(p< .01> 보여 준거 타당도에도 지지를 받았다.정신분열병 환자의 구체적 단서 요인과 추상적 단서 요인간의 점수를 비교한 결과 통계적으로 유의한 차이(t= 6.880, p< .000)를 보였으며 일반인과 정신분열병 환자의 사회적 단서 인지 점수를 비교한 결과는 통계적으로 유의한 차이(t= -13.601, p<.001)를 보여줌으로써 정신분열병 환자의 차별적 손상과 일반인과의 능력 차이를 구별할 수 있는 변별력에 대해 지지를 받았다고 할 수 있다.이상의 도구 개발 과정을 거쳐 행동, 대화 단서를 포함하는 구체적 단서 요인과 정서, 의도단서를 포함하는 추상적 단서 요인으로 구성된 사회적 인지 측정도구를 개발하였다. 도구는 8장면과 80문항으로 이루어진 이분 척도이다.본 연구에서 개발된 사회적 인지 측정 도구는 신뢰도와 타당도가 만족할 만한 수준으로 검정되었으며 기존의 도구와는 달리 국내 상황을 근거로 하였기 때문에 좀 더 포괄적이고 다차원적으로 사회적 인지를 측정할 수 있을 것으로 기대되어 진다. 앞으로 본 연구에서 개발된 도구는 정신분열병 환자의 사회적 인지 연구와 간호 중재 개발에 기여할 것이며 이를 통한 많은 경험적 입증을 통해 이론 개발에도 기여하게 될 것으로 기대된다.

[영문]The purpose of this study was to develop an instrument that could measure the social cognition of schizophrenia. The construction of this instrument was drawn from the extensive literary review of theoretical evidence of social cognition. The domains of social cognition were categorizedas follows; emotional characteristics and social cues. Emotional characteristics were broken into two factors; the arousal factor and the pleasantness factor. The social cues were divided into two factors; concrete cue and abstract cue.We chose 21 scenes describing the real interpersonal relationships from domestic TV dramas and movies, which represented the four drawn domains well. 252 items were developed from the scenes and 6 experts assessed our preliminary instrument for clinical validity. The preliminary instrument was then revised to eighteen scenes and 214 items; after which, the instrument passed through a standardization process by being applied to 76 normal controls. 12 scenes and 144 items were selected after this stage. After item analysis for the results from 196 schizophrenics in twice, Finally, our instrument was to be composed of 8 scenes and 80 items. The verification of test-retest reliability resulted in r=0.523 (p<0.01) and the internal consistency Cronbach''s α was 0.89. These figures supported the consistent reliability of our instrument.The significant correlation between the scores of each domain and the total score proved the construct validity of our instrument. The developed instrument showed positive correlations with Nurse''s Observation Scale for in-patient evaluation(r=0.346, p<0.01). The results also support the validity of our instrument. The concrete cue scores and the abstract cue scores in schizophrenia showed a statistically significant difference(t=6.880, p<0.000). There was also a significant difference (t=-12.344, p<0.001) between the controls. Because the scores of social cognition in the schizophrenia proved the discriminating power of our instrument; and as a result, our instrument can distinguish the ability difference between the normal and the schizophrenic''s partiality,In conclusion, our instrument demonstrated consistent reliability and validity; hence, we believe our instrument will contribute to the social cognitive research of schizophrenia and the development of nursing intervention with schizophrenic patients in the future.
