성대폴립 환자의 수술 적 치료 전후의 Lx speech studio를 중심으로 한 음성 분석

지금까지 성대의 기능과 음질을 측정할 수 있는 여러 가지 검사들이 발달되어져 왔지만 아직까지 여러 검사들 나름대로 장단점을 가지고 있어 더 나은 검사방법에 대한 필요는 계속되는 실정이다. Lx Speech Studio(Laryngograph Ltd, UK)는 이러한 문제를 극복하고자, 모음 연장발성 뿐만 아니라 2분 이상의 음성을 분석할 수 있도록 고안된 프로그램이다. 그럼으로써 좀 더 일상 대화 시에 일어나는 문제를 분석할 수 있게 되었다. 본 연구에서는 성대폴립 환자에서 수술 전후 음성변화 및 정상 대조군과 환자군과의 차이를 측정하기 위해서 현재 임상에서 널리 사용되고 있는 음향분석(acoustic analysis)과 위에서 설명한 Lx speech studio program을 이용한 전기성문파형검사를 실시하였다. 또한 이들 검사를 수술 전 후 그리고 정상성인에서 실시하여 수술 전후 성대음의 변화 및 정상군과 환자군과의 차이를 종합적으로 분석하고자 하였고 더불어 새로운 검사법인 Lx Speech Studio의 결과를 다른 검사들, 특히 현재까지 임상적으로 많이 사용되고 있는 음향분석의 측정치들과 비교하여 Lx Speech Studio의 임상적 유용성에 대해 알아보고자 하였다. / 성대폴립으로 진단된 환자 30명과 정상 대조군 30명을 대상으로 청지각적인 검사(perceptual test), 음향검사, 후두 스트로보스코피 검사, Lx speech studio program을 이용한 전기성문파형검사를 시행하였고 정상, 수술 전, 수술 후를 각각 비교하였다./ 환자군을 청지각적인 검사결과에 따라 등급(grade) 1과 등급(grade) 2로 나누어 비교한 결과 정상군과 환자군과의 비교에서는 성문폐쇄율(Qx), 주파수 불규칙성(CFx), jitter, shimmer 가 유의한 차이를 나타냈고 수술 전과 후의 비교에서는 성문폐쇄율, 주파수 불규칙성, jitter, shimmer, HNR(harmonic to noise ratio)가 유의한 차이를 나타냈다. 특히 성문폐쇄율의 경우 청지각적 검사에서 정도가 경미한 군에서는 증가한 반면, 정도가 심한 경우에서는 오히려 감소하는 양상을 나타냈다./ 결론적으로 성대폴립환자의 음성분석에서 음향분석이나 청지각적인 검사, 후두 스트로보스코피, Lx Speech Studio를 이용한 음성분석 모두 의미가 있는 검사 방법으로 생각되어지며 음성분석을 행할 때 한 가지 검사보다는 이들 검사를 모두 종합하여 판단하는 것이 좀 더 상세한 정보를 획득할 수 있을 것으로 생각된다. 특히 Lx Speech Studio의 경우 다른 음성검사들의 단점을 극복할 수 있는 우수한 검사방법으로 생각되며 아직까지는 많이 사용되고 있지 않지만 다른 검사들과 병용하였을 경우 충분히 하나의 의미 있는 검사로서 자리매김 할 수 있을 것으로 생각된다.

[영문]The purpose of this study is to analyze the EEG measures from Lx Speech Studio program(Laryngograph Ltd, UK) in patient with vocal polyp. Thirty normal adults and thirty vocal polyp patient were examined with perceptual test, acoustic analysis and Lx Speech Studio program(Laryngograph Ltd, UK). First, we compared normal group with patients group. The patients group was divided with two group, according to the Grade(GRBAS). In the comparison of these three group, CFx(%), Qx(%), jitter, shimmer were significantly different. Specially, Qx was increased in grade 1 but significantly decreased in grade 2. In comparison of preo-peration and post-operation, CFx(%), Qx(%), jitter, shimmer and HNR were significantly different. /This study proved that Lx Speech Studio program(Laryngograph Ltd, UK) was reliable method in analysis of vocal polyp patient. And perceptual test, Acoustic analysis and Videostroboscopy were found to be reliable and meaningful methods. This study suggests that multi-dimensional analysis with diverse methods is required. Lastly, EGG with Lx Speech Studio program(Laryngograph Ltd, UK) that were tried newly in this kind of study is believed to be a great vocal function test that can overcome shortcomings of other methods and may be used as a basic test method of future voice test.
