처방수행을 위한 임상간호사의 의사결정실태 분석

의사의 처방은 환자의 진단 및 치료뿐 아니라 진단과 치료에 관련된 여러 분야의전문인들과 의사소통하기 위한 중요한 자료가 되며 간호사는 의사의 처방을 관리하며 수행한다.

간호사의 활동중 처방수행과 관련된 간호활동은 가장 큰 비중은 차지하고 있으며 환자들은 간호사의 처방수행과 관련된 간호활동을 가장 중요하게 인지하고 있다. 또한 간호사는 의사의 처방을 환자에게 직접 수행하므로 전문적인 지식과 자율적인 판단하에서 의사처방을 수행해야 한다.

본 연구는 의사처방의 실태와 의사처방 업무수행을 위한 간호사의 의사결정실태를 파악하고저 조사와 면담을 병행한 기술적 연구이다.

자료 수집 기간은 1989년 11월 7 일부터 12월 17일까지 40일간이었으며 분석대상은 부산시내 B종합병원 내과중환자실, 내과일반병동, 정형외과병동에 입원한 100명 환자의 1,002회 처방과 이들 처방수행에 따른 간호사의 의사결정 활동이었다.

조사방법은 본 연구자와 2명의 연구보조원이 매일 3시간씩 간호사실에 있는 환자기록에서 의사의 처방과 처방에 대한 간호사의 활동을 조사하고 환자기록에서 확인할수 없는 사항은 활동한 간호사와 직접 면담하여 간호사가 제시한 처방변경요청을 수정, 삭제, 보완으로 구분하여 마련된 조사도구에 기록 하였다.

조사도구는 본 연구자가 직접 개발한 것으로 환자상태, 의사처방, 간호사의처방변경요청 및 면담 내용을 기록할 수 있게 만들었다.

연구결과를 요약하면 다음과 같다.

1) 입원시 처방의 요소에서 진단, 상태, 앨러지에 대한 내용은 없었고 활력증상, 활동, 식이 및 간호에 대한 내용이 내과중환자실과 일반병동에서는 구체적이었던 반면에 정형외과에서는 정규적인 처방이거나 처방이 나지 않았다.

2) 처방요소를 입원기간 별로 분석한 결과 활력증상, 활동, 식이, 간호와 같은지지적 요소의 처방은 입원 첫날 가장 많았고 입원 2일부터는 감소하였으며 투약은 입원기간에 관계없이 계속 높았다.

3) 간호사의 처방수행 활동에서 나타날수 있는 처방변경요청의 여부, 수정, 삭제 및 보완의 처방변경요청 대안 중 한가지 이상의 대안선택, 처방변경요청에 따른 의사의 반응, 간호사의 활동 및 활동후에 기대되어지는 결과들을 보여주는 처방수행을 위한 간호사의 이론적인 의사결정 체계를 도출하였고, 본자료에서 실제 간호사들이 처방수행을 위해 내린 의사결정 실태를 파악하였다.

4) 처방수행을 위한 간호사의 의사결정에서 처방변경요청의 여부와 요청의 종류 및 빈도를 조사한 결과 처방대로 수행한 경우가 60.3%로 처방변경요청을 제시한 경우보다 많았고 간호사가 처방변경요청을 지시한 39.7%의 처방중 보완요청이 23.3%로 가장 많았으며 전체 처방변경요청 빈도는 675회로 요청빈도의 범위는 1-6회였다.

5) 의사결정요인은 의료인요인, 물리적요인 및 환자요인으로 분류할 수 있었고 환자요인에 의해서 처방변경요청을 제시한 경우가 가장 많았다. 의사결정요인별로 처방변경요청의 내용을 분석했을 때 환자요인에 의해서는 보완요청이, 의료인요청에 의해서는 삭제요청이, 물리적요인에 의해서는 수정요청이 많았던 것으로 나타났다.

6) 환자요인은 환자의 상태가 악화되거나 호전되는 경우, 예방적 조치가 필요하거나 부작용이 나타난 상태 및 특이성으로 구분될 수 있으며 환자에게 부작용이 나타났거나 합병증을 예방하기 위해 처방변경요청을 제시한 경우가 많았다.

7) 환자가 내과중환자실에 입원하고 합병증이 있으며 의식상태가 불명료하고 입원기간이 9일 이내인 경우에서 처방변경요청을 제시하는 경우가 많았다.

8) 처방요소에 따른 처방변경요청의 빈도를 조사한 결과 투약에서 가장 많았다.

9) 입원기간에 따라 처방변경요청의 빈도를 조사한 결과 입원기간에 관계없이 보완요청이 삭제나 수정요청보다 많았다.

이상의 연구결과를 통하여 다음과 같은 결론을 얻었다.

1) 입원처방에서 간호계획시 정보를 제공할 수 있는 진단, 환자의 상태 및 앨러지에 대한 내용은 없었다. 간호에 대한 처방내용을 병동특성에 따라 비교해보면 내과중환자실과 일반병동에서는 너무 구체적이어서 간호활동에 대한간호사의 자율성과 책임감이 적어질 가능성이 있었던 것에 비하여 정형외과병동에서는 간호에 대한 언급이 거의 없어 간호사가 스스로 판단하여 활동하고 있었으나 법적인 보호방안은 없었다.

2) 간호사는 의사처방이 적합한지를 판단하고 수행한다. 즉 의사결정을 통하여 처방을 수행하고 있다. 이러한 의사결정은 의사결정체계를 형성하고 있고, 이 체계에는 의사의 반응도 포함되어 있다. 따라서 처방수행은 의사와 간호사가공동체로서 함께 사고하고 협력하여 이루어지는 활동이다.

3) 간호사의 처방수행을 위한 의사결정요인으로는 의료인, 물리적 및 환자요인이있었다. 즉 간호사의 처방수행은 이들 요인들을 모두 고려하여 내리는 포괄적인의사결정에 의해서 이루어지는 활동이다. 특히 환자요인에 의해서 간호사가 처방변정요청을 제시한 빈도가 가장 많아 간호사는 무엇보다도 환자를 위한전문의료인으로서 활동하고 있음이 나타났다.

이상에서 입원 처방에 간호계획시 정보를 제공하는 환자의 진단, 상태 및 얼러지에대한 사항이 기록되고 간호에 대한것은 간호사가 처방하고 수행할 수 있는 합법적인방안이 모색되어야 할 것이라고 본다.

또한 처방수행을 위한 간호교육시 간호사외 의사결정과 책임에 대한 교육이 강조되고 간호사의 처방수행활동이 환자를 위한 전문인으로서의 활동임을 규정해나가야 할 것이라고 본다.

The doctor's order is an important material not to diagnose and treat the patient's disease but to communicate with other speciallists for the diagnosis and treatment, and nurses handle and perform the order.

Among nurses's tasks, the performance of doctor's orders is most important and patients generally recognize it as the primary nursing effort.

As nurese contact directly with patients and perform doctor's orders to patients, they should have specific knowledges and make corned judgement before they carry out doctor's orders.

This study is a descriptive one to understand the actual condition of doctor's orders and that of nurse's decision in performing doctor's orders by investigation and interview.

The objects of this study were 1,002 doctor's orders for 100 patients on Internal Medicine Intensive Care Unit(I.C.U.), Internal Medicine General Wards(G.W.) and Orthopedic Surgery Wards(O.S.) in B University Hospital and nurse's decision for the performance of these orders. Data were collected from 7th, November to 17th, December, 1989.

The study was done with a specially designed sheet by author in which the patient condition, doctor's orders, nurse's proposition of changing orders, and interview with nurses were recorded.

Author and two coworkers reviewed patient's records at nurse station for three hours daily to investigate doctor's orders and nurse's performance and interviewed with nurses to ascertain situations not recorded on patient charts. We classified the contents of nurse's propositions of changing orders into modification, deletion, and new requirement and recorded them on the designed sheet.

The results are summarized as follows :

1) In admitting order components, there were no discriptions about diagnosis, patient condition, and allergy in all wards. In I.C.U. and G.W. orders for vital signs, activity, diet, and nursing were prescribed in details, however in 0.5. those were prescribed only formally or not done at all.

2) According t? admission period, orders of supportive components such as vital signs, activity, diet, and nursing were prescribed most frequently at the first hospitalized day and after second day the frequency of these prescription were decreased gradually.

The frequency of orders for medication showed no change according to the period.

3) Theoretical nurse's decision tree model composed of the following consequences observed when nurses performed doctor's orders was designed. The consequences were presentation of proposition of changing elders, choice of one or more alternatives in proposition contents (modification, deletion, and new requirement), doctor's response, nurse's performance, and expected outcomes. In this study, the nurse's decision for actual performance of orders was analized according to this decision tree model.

4) In nurse's decision, the frequency of performance without any proposition of changing orders were higher(60.3% ) than that of performance with proposition. Total proposition was 675 times with the extreme frequency of 6 times for one order and the preposition of new requirement(23.3% ) was larger than the other contents.

5) The factors influencing nurse's decision were classified into medical personnel factor, physical factor, and patient factor and proposition of changing orders was presented mainly due to patient factor. As to each influencing factor, the most frequent proposition content were as follows : new requirement in patient factor, deletion in medical personnel factor, and modification in physical factor.

6) Patient factors were divided according to physical condition into worsening, improving, the need for prophylaxis of complications, the occurrence of sideeffects, and specificities. Preposition was preposed usually due to the occurrence of side effects and the need for prophylactic management.

7) In I.C.U. patients with complications and poor mentality proposition was proposed usually within 9th hospitalized day.

8) According to the components of doctor's orders, the highest frequency of preposition was seen at medication orders.

9) As the frequency of proposition was analized according to admission period, the frequency of new reguirement was higher than that of deletion or modifi-cation irrespective of admission period.

As above results, the conclusions are as follows :

1) In admitting orders, there were no discriptions about diagnosis, patient condition, and allergy which could give useful informations far making nursing care plan. As the contents of orders for nursing compaired according to each ward, those were so detaily discripted in I.C.U. and G.W. that there seemed to be the possibility of negligence in nurse's self-regulation and responsibility of nursing practice. However in O.S., there was nearly no discription about nursing, and nurses judged and practiced by themselves without any plan of legal protection.

2) Nurses judge whether doctor's orders are adequate or not and perform the orders. So they are making decisions for the performance of the orders, and these decision processes form an organized decision tree. Doctor's responses are contained in this decision tree, therefore nurse's performance of the orders is a cooperative work of doctor and nurse.

3) The factors influencing nurse's decision to perform the orders are medical personnel factor, physical factor, and patient factor. Nurses make decision comprehensively under the consideration of all these factors. Nurse's proposition of changing orders is presented mainly due to patient factor that reveals the nurse is a special medical personnel working primarily for the patient.
