알코올 중독자의 회복체험

인간과 환경의 본질 및 인간과 환경간의 상호작용에 관심을 갖는 간호영역에서는 지식체를 개발하기 위해, 간호의 초점을 인간의 삶의 경험과, 우주와 공동 창조하는 건강(cocreating health)에 두고 있다. 즉 인간경험의 현상을 규명하고, 그것이 삶 그 자체와 어떤 관계가 있는가를 파악하는 것이다. 따라서 간호지식체를 개발하기 위해 탐구할 주요초점은 대상자의 경험이며, 이 경험을 인간되어감(human becoming)의 과정 곧 건강과정으로 보고 있다.

본 연구의 목적은 알코올 중독자의 회복체험의 구조를 확인하고, 그 체험의 구조와 인간되어감(human becoming) 이론과의 관련성을 파악하여, 알코올 중독자의 회복체험에 대한 지식체를 개발하는 것이다.

자료수집 기간은 1994년 8월부터 1995년 5월까지이었으며, AA에서 3년 이상 단주생활을 유지하고 있는 9명의 참여자를 대상으로 하였다.

연구를 위한 이론적인 기틀은 Parse의 인간되어감 이론이며, 연구방법에 있어서도 Parse의 연구방법론을 적용하였다. 즉 연구자와 참여자의 나-너 관계과정(dialogical engagement)을 통하여 관찰 기록과 녹음의 형태로 얻어진 자료를 참여자의 언어에서 추출-연구자

의 언어로 종합(extraction-synthesis)하는 단계와, 발견적 해석의 단계를 거쳐 분석하였다. 자료의 신뢰성(trustworthiness)과 정확성(rigor)을 높이기 위해, Sandelowski(1986, 1993)와 Burns(1989)의 질적 연구 기준에 따라, 신뢰성, 적합성, 감사가능성 및 일치성을 평가하였으며,Hedger & Cowles(1993)와 Koch(1994)가 개발한 결정과정 절차에 따라, 현장기록을 자세히 하였다. 그 결과, 다음과 같은 회복체험의 구조가 확인되었다.

"알코올 중독자의 회복체험은, 알코올 중독자들이 극심한 괴로움, 환각경험, 자살기도 등의 절박한 상황에 처했을 때, 알코올에 중독 되었음을 자인하고, 새롭게 의미를 부여할 뿐 아니라(가치화), 단주 후 잠재력이 발휘될 수 있으나, 동시에 재발의 가능성도 있으며(가능성-한계성), AA와의 부단한 접촉과 지지적인 이해를 통하여 술과 멀어지고(연결성-분리성), 단주 및 단주생활 유지를 위해 노력함은 물론(강화성), 위대한 힘의 체험을 통한 영적 변화로 새로운 생활을 영위해 가는 과정이다(변형성)."

이 구조를 통합시켜 표현하면,

"알코올 중독자의 회복체험은 자신이 알코올 중독자라는 사실을 인정하고, 가치 있는 것을 선택하여, 단주의 기회와 한계의 리듬을 구체화하며, 인간관계에서는 함께 있음과 멀어짐의 삶을 통하여 추진-저항하면서, 현실을 인정함과 동시에 세계관이 확대되는 경험이다. "

이렇게 통합된 구조를 인간되어감 이론의 개념과 연결한 개념적 해석은 다음과 같다.

"알코올 중독자의 회복체험은 가치가 가능-한계, 연결-분리의 과정을 통하여 단주생활이 강화되고, 세계관이 변형되는 경험이다. "그러므로, 회복체험의 구조는 이론적 수준임을 확인할 수 있었다.

이상의 연구결과를 기존문헌과 이론적 기틀에 따라 논의하면, 알코을 중독자의 회복은, 그들이 인간과 우주와의 상호작용을 통하여 끊임없이 변화하고, 경험 속에서 의미를 발견하여, 가치 있는 삶의 방법으로 초월하기 위해, 의도적으로 노력하는 건강경험, 치유의

과정, 및 되어감(becoming)의 과정이라고 할 수 있다.

The purpose of this study is to understand the structure of alcoholics' lived experience of recovery and to apprehend the connectedness of the structure of the experience with each concept of "human becoming" theory, and to develop the body of

knowledge of alcoholics' lived experience.

The data collection of this study was conducted from August, 1994 to May, 1995.

Research participants were nine male Persons keeping sobriety for more than three years. The researcher had interviews with them over their lived experience of recovery, recorded them on tapes, and made an analysis according to Parse's "human becoming research methodology." The Data were collected using the dialogical engagement process of "I and You,"the participant researcher and the participant subject. The Data were analyzed using the extraction-synthesis and heuristic interpretation. The criteria of Guba and Lincoln(1985), and Sandelowski(1986):

credibility, auditability, fitness and objectivity were used to test the validity and reliability of the data.

The following is a description of core concept of participants' lived experience of recovery showed in the process of analysis

1. Admitting one's alcoholism in an impending situation and taking on a new meaning.

2. There being the possibility of a relapse in concurrence as well as the display of one's potentiality.

3. Alienated from alcohol by means of continuous connection with A.A(Alcoholics Anonymous) and supportive understanding.

4. Making an effort for sobriety and the maintenance of sobriety life.

5. Transformed spiritually through High Power and leading a new life.

The above core concept can be synthesized and expressed as a structure.

The structure of the alcoholics' lived experience of recovery can be expressed:

(a) They admit their alcoholism and acquire a new meaning, when they are in such an impending situation as acute suffering, hallucination experience, suicide attempt, and so forth; (b) there can be not only the display of their potentiality

but the possibility of a relapse; (c) they are alienated from alcohol by means of continuous relation with A.A and supportive understanding; (d) they struggle for sobriety and the maintenance of its life; and (e) they are transformed spiritually through High Power and lead anew life.

Using Parse's human becoming research methodology, the structure of the alcoholics' lived experience of recovery identified in this study is conceptually interpreted as:

The alcoholics' lived experience of recovery is powering transforming through enabling-limiting and connecting-separating of valuing.
