Helicobacter pylori가 감염된 환자에서 위 반지세포암종에 동반된 MALT 림프종

Abstract

Helicobacter pylori (H. pylori) plays a substantial role in the pathogenesis of primary gastric carcinoma and low grade gastric mucosa-associated lymphoid tissue (MALT) B-cell lymphoma. The synchronous occurrence of both lesions have been reported very rarely. Here we present a case of gastric signet ring cell carcinoma and MALT lymphoma developed simultaneously with H. pylori infection. A 46-year-old man was referred for the evaluation of abnormal endsoscopic findings. Upper endoscopy revealed early gastric cancer in the lower body of the stomach and subsequent biopsy of the lesion proved to be a signet-ring cell carcinoma. After confirmation of signet-ring cell carcinoma, subtotal gastrectomy was performed. Histopathological study of the resected specimens showed collision tumor, which was composed of gastric signet ring cell carcinoma and low grade B-cell MALT lymphoma. H. pylori was detected in the resected specimen and the standard eradication therapy was instituted. We suspect H. pylori infection can be a etiologic factor in the development of both MALT lymphoma and carcinoma of the stomach in this patient. We report this unusual case along with a review of the related literature
