Traditionally, amputation for treatment of subungual melanoma was recommended. When the thumb is involved, amputation of thumb shows shortcoming in cosmetic aspect, and cause damage more than 50% of function of hand. To minimize function and cosmetic loss, wraparound flap from big toe was performed in the two thumbs. This salvage surgery yielded excellent cosmetic and functional resusts. There has been one recurrence of lymph node and no deaths with a minimum of 8 years follow up.
