중환자실 간호사의 의사소통능력, 팀워크역량, 인간중심간호 수행과 환자안전간호활동과의 관계

Abstract

This study aimed to identify the relationships between communication skills, teamwork competency, person-centered care and patient safety nursing activities among ICU nurses working at a tertiary hospital. By examining these relationships, the study seeks to provide foundational data for developing strategies to enhance patient safety nursing activities among ICU nurses. The main results of this study are as follows. 1. The average score for communication skills of the participants was 97.65 (±12.49), with a mean rating of 3.25(±0.42). The average score for teamwork competency was 36.91(±4.64), with a mean rating of 3.69(±0.46). The average score for person-centered care was 53.29(±6.86), with a mean rating of 3.55(±0.46). The average score for patient safety nursing activities was 286.23(±28.47), with a mean rating of 4.47(±0.45). 2. There was no statistically significant difference in patient safety nursing activity scores based on gender, age, education level, work department, total clinical experience, ICU experience, or history of patient safety incidents. 3. Patient safety nursing activities showed a statistically significant positive correlation with communication skills (r=.35, p < .001), teamwork competency (r=.43, p < .001), and person-centered care (r=.39, p < .001). 4. The factors related to patient safety nursing activities were teamwork competency (β=.27, p = .026) and person-centered care (β=.21, p = .039), and the explanatory power of this model was 20%. It was found that the higher the ICU nurses' teamwork competency and person-centered care, the higher the level of patient safety nursing activities. To strengthen teamwork competency, it is necessary to provide education and training programs that promote collaboration and communication among nurses, facilitate smooth intra-team communication, and reinforce mutual support within the team. Additionally, to enhance person-centered care, personalized nursing care that comprehensively considers patients' physiological, psychological, and social needs should be provided, and an environment should be created where patients and their families can actively participate in the treatment process. This approach can improve the quality of patient safety nursing activities in the ICU environment, strengthen trust between patients and nurses, and further ensure patient safety.

본 연구는 일 상급종합병원에 근무하는 중환자실 간호사를 대상으로 의사소통능력, 팀워크역량, 인간중심간호 수행과 환자안전간호활동과의 관계를 파악하여, 환자안전간호활동의 관련성을 규명함으로써, 중환자실 간호사의 환자안전간호활동을 향상시키기 위한 전략 수립을 위한 기초자료를 제공하기 위해 시도되었다. 본 연구는 서술적 상관관계 연구로 강원도 소재 일 상급종합병원의 중환자실 간호사 127명을 대상으로 자가보고 방식의 설문조사로 시행되었다. 연구도구는 한국 보건산업 진흥원(2010)에서 개발한 병원인증평가항목과 국제의료기관평가위원회(Joint Commission International [JCI])의 의료기관 인증 표준(Standards for Hospitals)의 국제적 환자안전 목표 6개 영역 중 중환자실 간호업무와 관련된 내용을 중심으로 조윤주(2012)가 수정한 환자안전간호활동 측정도구를 사용하였으며, 의사소통능력은 Shortell 등(1991)이 개발하고 조용애 등(2013)이 번역한 측정도구와 팀워크역량은 정민주 등(2012)이 개발한 17문항을 이동준(2021)이 10문항으로 재구성한 측정도구와 인간중심간호 수행은 강지연 등(2018)이 개발한 측정도구를 사용하였다. 수집된 자료는 IBM SPSS Statistics 29 프로그램을 이용하여 기술통계, Independent t-test, One way ANOVA, Pearson’s correlation coefficient, Multiple linear regression을 이용하여 분석하였다. 본 연구의 주요 결과는 다음과 같다. 1. 대상자의 의사소통능력의 평균은 97.65점(±12.49), 평균평점 3.25(±0.42)이고, 팀워크역량의 평균은 36.91점(±4.64), 평균평점 3.69(±0.46)이었고, 인간중심간호 수행의 평균은 53.29점(±6.86), 평균평점 3.55±0.46이었다. 환자안전간호활동의 평균은 286.23점(±28.47), 평균평점 4.47±0.45이었다. 2. 일반적 특성에 따른 성별, 연령, 학력, 근무부서, 총 임상경력, 중환자실 근무경력, 환자안전사고 유, 무는 환자안전간호활동 점수와 통계적으로 유의한 차이가 없었다. 3. 환자안전간호활동은 의사소통능력(r=.35, <.001), 팀워크역량(r=.43, <.001), 인간중심간호 수행(r=.39, <.001)과 통계적으로 유의한 양의 상관관계가 있었다. 4. 환자안전간호활동의 관련 있는 요인은 팀워크역량(β=.27, p=.026), 인간중심간호 수행(β=.21, p=.039)이었으며, 이 모형의 설명력은 20%이었다. 이상의 연구 결과를 통해 중환자실 간호사의 팀워크역량이 높을수록, 인간중심간호 수행이 높을수록 환자안전간호활동 수준이 높은 것을 확인하였다. 본 연구결과를 바탕으로 중환자실 간호사의 환자안전간호활동 수준을 높이기 위해서는 팀워크와 인간중심간호 수행을 강화하는 것이 핵심적이다. 팀워크역량을 강화하기 위해서는 간호사들 간의 협력과 소통을 촉진하는 교육과 훈련 프로그램을 제공하여, 팀 내 의사소통을 원활하게 하고, 상호 지원을 강화하는 것이 필요하다. 또한, 인간중심간호 수행을 강화하려면, 환자의 생리적, 심리적, 사회적 요구를 종합적으로 고려한 개인 맞춤형 간호를 제공하고, 환자와 가족이 치료에 적극적으로 참여할 수 있는 환경을 조성해야 한다. 이러한 접근은 중환자실 환경에서 환자안전간호활동의 질을 향상시키고, 환자와 간호사 간의 신뢰를 강화하여 환자안전을 더욱 보장하는 데 기여할 것이다.
