국내 및 주요국 희소의료기기 관리 제도 분석을 통한 규제 개선

Abstract

"희소의료기기는 대체로 연구 및 개발 비용이 높기 때문에 일반 의료기기에 비해 가격이 높고, 생산량이 적어 수요와 공급이 불균형하게 형성된다. 국내 희소의료기기 제조․수입업체의 개발과 공급은 일부 품목에 대한 글로벌 대기업의 시장 독점과 R&D 비용에 비해 현저히 낮은 수익성을 이유로 매우 더딘 상황이다. 정부가 주도적으로 관리하고 있으나 여전히 개선이 요구되는 상황이다. 본 연구는 한국의 희소의료기기 관리 체계를 미국, 일본, 그리고 유럽과 비교하여 국내 제도에서 개선해야 할 부분을 파악하였다. 이를 바탕으로 희소의료기기 지정기준 확대와 품목․수입 허가 의무화 및 희소의료기기 규제 레지스트리 도입을 제시하였다. 또한 희소의료기기 제조업체에 대한 지원방안으로 국내 규제기관의 컨설팅, 허가심사 시 우선순위 부여를 통한 심사기간 단축과 희소의료기기 제조업체 인증제도를 도입 등의 방안을 제안하였다. 다만, 법인세 감면 및 정부기관 발주 사업 지원 시 가산점 차등 지급과 같은 혜택은 희소의료기기 제조사 인증 기업에 한하여 제공된다. 그러나 제시된 각 항목의 세부사항 보완을 위한 후속 연구가 필요하다. 본 연구가 향후 희소의료기기 산업의 발전에 기여할 수 있기를 기대한다.

Because humanitarian use devices generally have high R&D costs, they are more expensive than general medical devices and have low production, resulting in an imbalance of supply and demand. The development and supply of humanitarian use devices of domestic medical device manufacturers and importers is very slow due to the market monopoly of large global companies for some items and their remarkably low profitability compared to R&D costs. Although the government is leading the management, improvement is still required. This study compared humanitarian use device management system of Korea with the United States, Japan, and Europe to identify areas that need to be improved in the domestic system. Based on this, it suggested expanding the criteria for designating humanitarian use devices, making it mandatory to permit items and imports, and introducing a regulatory registry of humanitarian use devices. In addition, as a support plan for humanitarian use device manufacturers, it proposed measures such as the consulting of domestic regulatory agencies, shortening the approval review period by priority granting, and introducing a certification system for humanitarian use device manufacturers. However, the benefits such as providing corporate tax refund and differential payment of the additional points when applying government ordering projects are the only for certified companies as humanitarian use device manufacturer. Additional research is required to make up for the details of each suggestions. It is expected to contribute the development of rare medical device industry."
