미국 LDT 조사 분석을 통한 국내 LDT 도입 방안

Abstract

의료분야가 치료에서 진단과 예방 중심의 패러다임으로 전환되는 추세에 따라 체외 진단용 의료기기에 대한 수요는 꾸준히 증가하고 있다. 이에 따라 체외진단검사를 수행하는 임상검사실의 질 관리의 중요성도 점점 커지고 있다. 국내에서는 여러 임상검사실 인증 제도를 운영함으로써 검사의 정확도와 정밀도를 관리하고 있다. 이 중 NGS 임상검사실 인증을 받은 임상검사실은 식품의약품안전처장의 품목허가를 받지 않은 NGS 검사용 장비를 사용할 수 있다. 이는 미국의 실험실 자체 개발 테스트인 LDT와 유사한 형태이다. 현재는 NGS에만 이러한 LDT 형태를 적용했지만, 국가 재난 상황이나 희귀질환 분야에서도 LDT 적용이 필요하다. 국내에 LDT가 안정적으로 도입되기 위해서는 검체 분석 수탁기관이 충분한 비용을 임상검사실 질 관리에 투자할 수 있도록 검사료 지급 구조 개선이 필요하다. 또한 최근 미국에서 LDT 규제 강화의 움직임이 있었는데, 이를 통해 미국의 CLIA처럼 국내에서도 임상검사실 질 관리를 의무화할 필요가 있고, 정부가 LDT를 통제할 수 있도록 LDT 현황을 파악할 수 있는 시스템을 구축할 필요가 있음을 도출하였다. The demand for in vitro diagnostic medical devices is steadily increasing as the medical field shifts from a therapeutic to a diagnostic and prevention-oriented paradigm. Accordingly, the importance of quality management of clinical laboratories that perform in vitro diagnostic tests is also growing. In Korea, several clinical laboratory certification systems are operated to manage the accuracy and precision of tests. Among them, clinical laboratories that are certified as NGS clinical laboratories can use NGS testing instruments that has not been approved by the head of the Ministry of Food and Drug Safety. This is similar to LDTs, which are laboratory-developed tests in the United States. Currently, this form of LDT is only applied to NGS, but it is also necessary to apply LDT in the field of orphan diseases and national disaster situations. In order for LDT to be introduced stably in Korea, it is necessary to improve the reimbursement system so that independent medical laboratories can invest enough money in clinical laboratory quality management. In addition, there has been a recent movement to strengthen LDT regulations in the United States, which has led to the conclusion that it is necessary to mandate clinical laboratory quality management in Korea like CLIA in the United States, and that it is necessary to establish a system to identify the current state of LDT so that the government can control LDT.
