교대근무 경찰 공무원의 자살 생각 관련 요인

Abstract
This study is a descriptive and correlational study that aims to evaluate the increasing suicide ideation rate of police officers, identify factors related to suicide ideation of police officers on shifts specially including mental health factors. The conceptual framework of the study was based on the stress-vulnerability model of Zubin and Spring (1977), investigating demosocial characteristics, mental health-related characteristics and suicide-related factors of police officers on shifts. In particular, health-related factors measured sleep quality, post-traumatic stress disorder symptoms, and levels of depression. Data collection was conducted online from March 1 to 14, 2021, and 152 studies were used for analysis, excluding one that responded insufficiently among the 153 recruated ones, and the collected data were analyzed as descriptive statistics, independent t-test and multiple linear regression at significance level .05 using the SPSS 25.0 program. The main findings of the study are as follows. 1. The results of the suicide ideation and health related factor levels of 152 subjects in this study are as follows. The suicide thought score was 1.79±4.16 on average, of which 26 (17.1%) responded to "Suicidal ideation group" and the score was 9.19±1.10 on average. In addition, mental health-related factors included sleep quality, post-traumatic stress disorder symptoms, and depression scores of 7.19±3.67 overall, with 96 (63.2%), and 9.33±2.79. Post-traumatic stress disorder symptoms averaged 1.40±1.62 points, with 37 (24.3%) of the subjects clinically significant stress disorder symptoms. For depression, the overall mean was 9.16±11.53 and 39 (25.7%) of the total subjects were classified as depressed, with an average of 28.14±13.39. 2. Among the 'suicidal ideation group' subjects in this study, the characteristics that showed statistically significant differences in suicidal ideation are workplace, work-related stress, work-family compatibility, and emotional support systems. The level of suicidal ideation differed significantly depending on the work location of the suicidal ideation group (𝜒2=11.605, p=.009), which was followed by statistically significant results on suicidal ideation levels in work stress (𝜒2=43.796, p<.001), there was also a significant difference in work-family compatibility (𝜒2=23.415, p<.001), when asked about the presence of a person with emotional support, check the statistical significance (𝜒2=23.505, p<.001) did. 3. The results of identifying factors related to suicidal ideation are as follows. If the target person's workplace is a police station (β=0.218, p=.001), with high work stress levels (β=0.231, p=.002), the higher the depression level (β=0.648, p=<.001), It has been shown to have a significant association with higher level of suicidal ideation. Among them, depression levels were the most relevant factor in suicidal ideation. Based on the results of this study, it was confirmed that the suicidal ideation of police officers working in shifts were quite high. In particular, the influence of workplaces, work stress, work-family compatibility, and emotional support systems was confirmed to have significant effects on the suicide ideation of police officers on shift workplaces, work stress, and depression. Thus, the suicide-related nursing interventions by shift police officers suggests that an integrated approach to monitoring suicide ideation should be developed considering individual, occupational, and mental health characteristics. The results of this study could help nurses develop strategies to provide health care that fits the characteristics of their occupations in the community.

본 연구는 경찰 공무원의 증가하는 자살을 예방하고자 교대근무 경찰 공무원의 자살 생각 관련 요인을 파악하고, 건강 요인과의 관련성을 통합적으로 파악하고자 시도된 서술적 조사연구이다. 본 연구의 개념적 기틀은 Zubin과 Spring (1977)의 스트레스-취약성 모델(Stress-vulnerability model)을 기반으로 하였고, 교대 근무 경찰 공무원의 인구 사회학적인 특성, 건강 관련 특성을 조사하였다. 특히, 건강관련 요인으로 수면의 질, 외상 후 스트레스 장애 증상, 우울에 대한 수준을 포함하였다. 자료 수집은 2021년 3월 1일부터 `3월 14일까지 교대근무에 종사하는 경찰 공무원을 대상으로 온라인 설문으로 진행하였으며 회수된 153명의 자료 중 불충분하게 응답을 한 1명의 자료를 제외한 152명의 연구 자료가 최종 분석에 이용되었으며, 수집된 자료는 SPSS 25.0 프로그램을 사용하여 유의수준 .05에서 기술통계, 독립표본 t검정, 분산분석 및 교차분석, 다중선형회귀분석으로 분석하였다. 연구의 주요한 결과는 다음과 같다. 1. 본 연구 대상자 152명의 자살 생각과 정신건강 요인 수준을 확인한 결과는 다음과 같다. 자살 생각 점수는 평균 1.79±4.16점이었으며, 이 중 '자살 생각 있음'에 응답한 대상자는 총 26 명(17.1%)이며 평균 점수는 9.19±1.10점으로 확인되었다. 이와 함께 건강 관련 요인 중 수면의 질, 외상 후 스트레스 장애 증상, 우울의 점수를 확인하였으며, 수면의 질은 전체 평균 7.19±3.67점이며, 수면의 질이 나쁜 군이 96명(63.2%)으로 확인되었고 평균 9.33 ±2.79점이었다. 외상 후 스트레스 장애 증상은 전체 평균 1.40±1.62점이며 대상자 중 스트레스 장애 증상이 임상적으로 유의한 군이 37명(24.3%)이었다. 우울의 경우 전체 평균은 9.16±11.53점이며 전체 대상자 중 우울군으로 분류된 대상자는 39명(25.7%)명으로 평균 28.14 ±13.39점으로 보고되었다. 2. 본 연구의 ‘자살 생각 있음’ 대상자 중 일반적 특성에서 자살 생각에 통계적으로 유의한 차이를 보인 특성은 근무지, 업무 스트레스, 일과 가정의 양립, 정서적 지지체계 유무이다. ‘자살 생각 있음’의 근무지에 따라 자살 생각 수준에 유의한 차이를 보였다 (𝜒2=11.605, p=.009). 이어서 업무 스트레스에서 자살생각 수준에 통계적 유의한 결과를 확인하였으며(𝜒2=43.796, p<.001), 일과 가정의 양립에서도 유의한 차이를 보였고(𝜒2=23.415, p<.001), 정서적 지지가 가능한 사람의 유무를 묻는 질문에서 통계적 유의를 확인(𝜒2=23.505, p<.001)하였다. 3. 자살 생각 관련 요인을 확인한 결과는 다음과 같다. 대상자의 근무지가 경찰서일 경우(β=0.218, p=.001), 업무 스트레스 수준이 높은 경우(β=0.231, p=.002), 우울 수준이 높을수록(β=0.648, p=<.001) 자살 생각에 유의한 영향을 미치는 것으로 나타났다. 이 중 우울 수준이 자살 생각 수준에 관련성이 가장 높은 요인이었다. 본 연구 결과를 바탕으로 교대근무 경찰공무원의 자살 생각은 상당히 높은 수준임을 확인하였다. 특히 교대근무 경찰 공무원의 자살 생각 유무에 근무지, 업무 스트레스, 일과 가정의 양립, 정서적 지지체계의 차이를 확인하였고, 자살 생각 관련요인으로 근무지, 업무 스트레스, 우울 수준이 유의미하게 작용함을 확인하였다. 따라서, 교대근무 경찰 공무원의 자살 생각 예방 관련 간호 중재 개발을 필요로 하는데 자살 생각에 대한 모니터링 시 개인적, 직업적, 정신건강적 특성에 따른 취약성을 고려한 통합적 접근이 시도되어야 하며 자살 생각을 감소시키기 위한 조직 차원의 우울과 업무 스트레스와 관련된 적절한 관리와 자살 생각이 많았던 근무지인 경찰서에 대한 관심과 대책 마련의 필요성을 시사한다. 본 연구 결과는 지역사회에서 직업의 특성에 걸 맞는 건강 관리를 제공하는 간호사의 전략 수립에 도움이 될 수 있을 것이다.
