Comparative study on the surge capacity for COVID-19 response in Republic of Korea and Vietnam

Abstract

This study aimed to compare the surge capacity for COVID-19 response in Korea and Vietnam to suggest the improvement plan on COVID-19 surge capacity response in Vietnam. The research purpose of this study is as followed. Firstly, it determined current status of infectious disease control system of Vietnam and Korea based on surge capacity. Secondly, it identified the advantage and disadvantage of current system, policies, law and experience related to infectious disease control by 4S strategy of surge capacity in Vietnam and Korea. Thirdly, it suggested improvement plan of surge capacity development in health system in Vietnam based on the outcomes of comparative studies As a result, in terms of space strategy, Vietnam has established triage model for COVID-19 to classify the patients and hospital. However, it applied binary approach that only consider on diagnose and treatment which did not consider on each stages in triage model and general disease treatment. This may lead to put burden on certain designated hospital for COVID-19 treatment. Moreover, utilization for other sector were weaker than Korea that it has utilized private hotels only for quarantine purpose. In terms of staff strategy, Vietnam has distributed their healthcare workforce by triage model. However, lacking consideration of safety and welfare for frontline workforce were identified. Regarding to supplies strategy, it has already possessed local production line to produce medical resources. However, lacking of standard of scarce resource management and priority were also identified which requires improvement. Moreover, system strategy in Vietnam seems efficient to countermeasure the COVID-19 response by strong central governmental control by Ministry of Health, Vietnam. However, it needs to suggest the guideline for internal management arrangement in order to make proper decision making by healthcare workforce in emergency situation. The study expects to be used as affordable research materials to activate surge capacity in COVID-19 response

이 논문은 한국과 베트남의 COVID-19 관련 대규모 재난 대응을 위한 제도 관련 문헌 고찰을 수행하고 한국과 베트남의 법 제도를 비교하여 베트남 제도의 개선방안을 도출하기 위한 연구를 수행하였다. 본 연구의 목적 관련하여 세부적 내용은 다음과 같다. 첫째, COVID-19 대응 관련 한국과 베트남의 현황을 파악한다. 둘째, surge capacity 관련 4S 전략을 중심으로 각 국가별 대응 체계의 장단점을 비교 분석한다. 셋째, 비교분석 결과를 토대로 베트남의 COVID-19 대응을 위한 제도개선 방안을 제시한다. 연구 수행 결과, 의료시설(space) 관련하여 베트남은 중증도 분류체계를 마련하였으나 시설 구분에 있어 진단과 치료로 이분적인 구분방법으로 한정하여 대규모 환자 발생 시 이를 수행하기 어려운 형태를 가지고 있으며 일반 환자에 대한 구분이 고려되지 않음을 확인하였다. 그 외 다른 시설을 활용하여 부족한 의료시설을 대체한 한국과 달리 베트남의 경우 격리 시설 활용 목적을 위한 타 시설 활용사례 외에 다른 사례가 없어 이에 대한 적극적인 대처가 필요한 것으로 확인되었다. 의료인력 관련하여 베트남은 중증도 분류체계에 따라 환자를 구분하고 이에 따른 의료인력을 시설 내에 배치하는 것으로 구성하였으나 부족한 의료인력을 마련하는 제도적 근거와 최전선 배치 의료인력에 대한 안전 및 복지 확보에 있어 고려되지 않음을 확인하였다. 의료자원 관련하여 베트남은 생산시설을 확보하였으나 부족한 의료자원을 구분하는 기준 및 활용에 대한 우선순위가 제도적 차원에서 개선될 필요성이 발견되었다. 의료체계 관련하여 베트남은 보건부 중심의 강력한 중앙정부의 명령체계를 구축하여 효과적으로 전국적으로 COVID-19를 효과적으로 대응한 것을 확인하였으나 내부 관리 차원에서 의료진들이 응급상황에 신속하게 의사 결정할 수 있는 체계가 마련되지 않아 이를 적극적으로 개선할 필요가 있다. 본 연구결과를 통해 효과적인 COVID-19 대규모 재난 대응을 위하여 참고할 수 있는 좋은 연구자료가 되길 희망한다.
