Abstract
본 연구는 국내 임산부의 체중증가 양상을 파악하기 위해 후향적으로 전자의무기록 자료를 수집하여 분석한 서술적 조사연구로 우리나라 임산부의 체중증가 지침을 만들기 위한 기초자료를 제공하고자 시도되었다. 대상자는 2013년 9월부터 2018년 8월까지 서울 소재 Y대학 S상급종합병원의 분만실에서 질식분만 및 제왕절개수술로 분만한 임산부의 의무기록을 대상으로 하였다. 해당 기간 동안의 총 분만 건수는 6503건이며, 이를 대상자 선정 및 제외 기준으로 전산처리하여 도출한 최종 분석 대상자는 2214명이다. 자료수집 결과는 기술통계를 이용하여 빈도, 평균, 및 표준편차로 기록하였으며 one-way ANOVA, Chi squre test, Fisher’s exact test와 사후분석은 Scheffe를 이용하여 주요 특성을 비교하였다. 본 연구의 주요 결과는 다음과 같다. 1. 전체 임산부 중 단태아 임신, 30대, 임신 전 정상 Body Index Mass(BMI)이면서 고위험 임신에 해당하지 않는 임산부로 구성된 기준군(n=868)의 총 체중증가량은 13.52kg로 Institute of Medicine(IOM)에서 제시한 임신 전 BMI 정상 체중군 체중증가 권고안인 11.3~15.9kg에 부합한다. 2. 고위험 임신군은 기준군에 비해 평균 연령대가 높았는데 특히 임신성 당뇨군과 임신 중 고혈압성 질환군의 경우 기준군에 비해 연령이 통계적으로 유의미하게 높았다(F=11.413, p<.001). 또한 임신성 당뇨군과 임신 중 고혈압성질환군의 경우 임신 전 BMI가 과체중으로 나타나 기준군과 조기진통군에 비해 통계적으로 유의미하게 높았다(F=143.77, p<.001). 3. 연령대가 높을수록 총 체중증가량이 많은 것으로 나타났고 30대군에 비해 20대와 40대군에서는 주수별 체중증가량의 변동이 큰 것으로 나타났다. 4. 쌍태아군은 단태아군에 비해 총 체중증가량이 25%가량 많았으며, 쌍태아군과 단태아군의 체중증가의 차이는 18주부터 발생했고 그 차이는 주수가 지남에 따라 점점 더 벌어지는 양상으로 나타났다. 5. 임신 전 BMI에 따른 체중증가량 비교에서는 정상체중군에서 총 체중증가량이 가장 많았고 그 다음으로 저체중군, 과체중군, 비만군 순으로 총 체중증가량이 많았으며, 임신 전 BMI가 높을수록 주수별 체중증가 변동 폭이 큰 것으로 나타났다. 6. 임신성 당뇨군과 기준군은 총 체중증가량은 유사했으나 임신 1삼분기(12주)까지의 체중증가량은 임신성 당뇨군에서 더 높게 나타났다. 7. 임신 중 고혈압성질환군은 기준군보다 총 체중증가량이 낮았지만 임신 중 고혈압성질환의 다빈도 발병 시기인 20주 이후부터는 체중이 비교적 꾸준히 증가하는 양상으로 나타났다. 8. 조기진통군은 조산의 위험성이 높아 대개 36주 미만에 분만하였는데 해당 주수까지의 체중증가량을 비교하면 기준군과 유사한 것으로 나타났다. 본 연구는 현재까지 IOM에서 제시한 체중증가 권고안 외에는 우리나라 실정에 맞는 임산부의 체중증가 기준이 없는 실태에서 국내 임산부의 체중증가 양상을 파악한 연구로서 그 의의가 있다. 고위험 임신의 경우 연령대와 임신 전 BMI와의 관련성이 있음이 입증되어 고령 여성이나 과체중 및 비만 여성이 임신하는 경우 임신 초기에서부터 보다 면밀한 산전관리를 제공해야할 필요성이 있다. 또한 고위험 임신으로의 이행을 줄이기 위해서는 각 질환과의 관련성이 높은 시기에 보다 세심한 관찰 및 주의를 통해 이를 예방할 수 있도록 해야 할 것이다.

PURPOSE: This study was aimed to describe maternal weight gain pattern of high-risk pregnant women in South Korea. METHODS: The design was a descriptive study using reviewing electronic medical records of Y-university healthcare system. A total of 6503 medical records of women, who had delivered babies from September 1st, 2013 to August 31st, 2018 were screened. Inclusion criteria were women who had 1) the records of weight, height, and medical diagnosis related to pregnancy and 2) 5 times or greater records of gestational weights during pregnancy. Final sample size was 2214, and women who were 30’s, singe pregnancy and not diagnosed by pregnancy-related health problems such as gestational diabetes mellitus (GDM), hypertensive disorders of pregnancy (HDP) and preterm labor (PTL) were considered as a reference group. The collected data were analyzed on SPSS 20.0, using descriptive statistics, a one-way ANOVA, the Chi square test, the Fisher’s exact test, and the Scheffe’s test. RESULTS: The mean of total maternal weight gain during pregnancy of the reference group (n=868) was 13.52kg. The average ages of both GDM and HDP groups were higher than reference group (F=11.413, p<.001). Pre-pregnancy body mass index (BMI) was higher in GDM and HDP groups compared to the reference group (F=143.77, p<.001). The mean of total maternal weight gain of twin pregnancy group was about 25% higher than that of single pregnancy group, and the gap of weight gain pattern was started from 18th weeks of pregnancy. The weight gain during the first trimester in GDM group was greater than that in the reference group, although there was no difference in the total weight gains between GDM and reference groups. CONCLUSIONS: The results indicated that high-risk pregnancy was related to women’s ages and pre-pregnancy BMI. The excessive weight gain during the first-trimester may be associated with GDM. Therefore, nursing interventions such as providing an educational program for proper weight manage before and during pregnancy should be offered to women.
