Abstract
PURPOSE: The aim of this study was to explore the risk factors associated with traffic accidents involving fatigue and sleep-related characteristics among occupational drivers in Korea. This cross-sectional descriptive study was conducted to provide fundamental information for establishing public health policy for promoting sleep-related health and reducing traffic accidents among occupational drivers. METHODS: A total of 180 occupational drivers were recruited from 10 commercial vehicle companies located in Gyeonggi Province, Chungcheong Province, Incheon Metropolitan City, and Daegu Metropolitan City via convenience sampling from March 22 to May 4, 2018. A structured questionnaire was administered to collect data, including traffic accident risk (15 items), driver’s perceived fatigue (10 items), quality of sleep (18 items), daytime sleepiness (eight items), health status (12 items), general characteristics (14 items), and work-related characteristics (eight items). Data were analyzed using descriptive analysis, the independent t-test, the chi-squared test, one-way analysis of variance or relevant nonparametric analysis, Pearson’s correlation analysis, multiple linear regression, and binary logistic regression with the IBM SPSS 24.0 program. RESULTS: In the final sample of 161 drivers, their vehicle types were categorized into trucks (n=79), construction vehicles (n=40), taxis (n=21), and buses (n=21). The mean age of participants was 53.03±9.42 years old, and the majority of them were males (98.1%). 1. The mean score of the traffic accident risk index of participants was 1.97±0.76 (range:1~5). From multiple linear regression analysis, the model explained 35.1% of the traffic accident risk index. For bus drivers compared with taxi drivers (β=0.27, p<.01), high perceived fatigue (β=0.29, p<.01), excessive daytime sleepiness (β=0.22, p<.01), and poor mental health status (β=-0.18, p=.02) were associated with higher traffic accident risk index scores. 2. The prevalence of constant driving despite fatigue, bad weather, or heavy traffic for work among the participants was 50.9%. From binary logistic regression analysis, the model explained 24.2% of constant risky driving. Working for more than 12 hours per day compared with working 12 hours per day or fewer (OR=3.79, 95% CI=1.75-8.22) and excessive daytime sleepiness (OR=10.11, 95% CI=1.10-92.68) were associated with constant risky driving. 3. The prevalence of not wearing seatbelts among the participants was 30.4%. From binary logistic regression analysis, the model explained 37.9% of not wearing a seatbelt. High perceived fatigue (OR=1.09, 95% CI=1.02-1.15) and poor mental health status (OR=0.92, 95% CI=0.86-0.98) were associated with not wearing a seatbelt. Excessive daytime sleepiness (OR=0.06, 95% CI=0.01-0.46) was associated with wearing a seatbelt. 4. The prevalence of “often” or “always” driving over the speed limit was 17.4%. From binary logistic regression analysis, the model explained 38.1% of speeding. Current smoking habit (OR=4.25, 95% CI=1.46-12.38), alcohol usage (OR=7.38, 95% CI=1.86-29.30) were associated with speeding. 5. The prevalence of recordable crashes in one’s career as an occupational driver was 44.7%, and 24.8% of participants had experienced car crashes within the past year. The prevalence of having a near miss during the past week was 24.2%, and 41.6% of participants had received moving violations in the past 12 months. From binary logistic regression analysis, the model explained 20.4% of traffic accidents experienced within the past year. Low quality of sleep (OR=1.29, 95%CI=1.08-1.54) was associated with traffic accident experience. CONCLUSIONS: The study findings revealed that traffic accident risk is associated with high perceived fatigue, low quality of sleep, excessive daytime sleepiness, poor mental health status, vehicle types, and long working hours per day among occupational drivers. This suggests that we need to develop health management nursing interventions to enhance quality of sleep, improve physical and mental health, and manage fatigue for occupational drivers. Vehicle-related organizations or communities need to establish public policies to reduce the traffic accident risk associated with occupational drivers.

본 연구의 목적은 국내 직업운전자의 피로 및 수면관련 특성을 파악하여, 교통사고 위험성에 영향을 미치는 요인을 규명하기 위함이다. 본 연구는 직업운전자의 수면관련 건강을 증진하고, 교통사고를 감소시킬 수 있는 보건정책 마련의 근거자료를 제공하고자 하는 횡단적 서술적 조사연구이다. 본 연구의 자료수집은 2018년 3월 22일부터 5월 4일까지 경기도, 충청도, 인천광역시, 대구광역시 소재의 총 10개 운수업체에 종사하는 180명의 직업운전자를 편의표집하여 시행하였다. 최종 161명을 대상으로 한 분석에는 직업운전자의 교통사고 위험성 15문항, 직업운전자 자각피로 10문항, 수면의 질 18문항, 주간졸림증 8문항, 건강상태 12문항, 일반적 특성 14문항 및 근무관련 특성 8문항의 총 85문항으로 구성된 자가보고 설문 자료가 이용되었다. 수집된 자료는 IBM SPSS 24.0 프로그램을 이용하여 기술적 통계, 독립표본 t검정, 교차분석, 분산분석 또는 해당 비모수검정, 상관관계분석, 다중선형회귀분석과 이분형 로지스틱회귀분석을 사용하여 분석되었다. 본 연구의 결과는 다음과 같다. 총 161명의 직업운전자는 운전 차량 종류에 따라 트럭 79명, 건설 관련 차량 40명, 택시 21명, 버스 21명으로 구분되었다. 대상자의 연령은 평균 53.03±9.42세로 대부분 남자(98.1%)였다. 1. 대상자의 교통사고 위험지수 점수는 평균 1.97±0.76(범위:1~5)점으로 보고되었다. 대상자의 교통사고 위험지수에 대한 다중선형회귀분석 결과, 모형의 설명력은 35.1%이며, 운전차량 종류가 택시운전자보다는 버스운전자일수록 (β=0.27, p<.01), 운전자 자각피로도가 높을수록(β=0.29, p<.01), 과다 주간졸림증이 있거나(β=0.22, p<.01), 정신적 건강상태가 나쁠수록(β=-0.18, p=.02), 교통사고 위험지수 점수가 높아지는 것으로 확인되었다. 2. 대상자의 50.9%가 피로, 악천후 또는 심한 교통체증에도 불구하고 운전을 지속하였다. 지속적 위험운전 여부에 영향요인을 분석한 결과, 회귀 모형의 설명력은 24.2%이며, 하루 평균 12시간 이하 근무하는 그룹보다 12시간 초과 근무하는 그룹이(OR=3.79, 95% CI=1.75-8.22), 과다 주간졸림증이 있는 경우(OR=10.11, 95% CI=1.10-92.68), 지속적 위험운전을 할 확률이 더 높았다. 3. 대상자의 30.4%는 운전 중 안전벨트를 착용하지 않는다고 보고하였다. 안전벨트 착용 여부에 영향 요인을 분석한 결과, 회귀 모형의 설명력은 37.9%이며, 직업운전자의 자각피로도가 높아질수록(OR=1.09, 95% CI=1.02-1.15), 정신적 건강상태가 나쁠수록(OR=0.92, 95% CI=0.86-0.98), 안전벨트를 착용하지 않는 확률이 높은 것으로 나타났다. 과다 주간졸림증이 있는 경우(OR=0.06, 95% CI=0.01-0.46), 안전벨트를 착용하는 확률이 더 높았다. 4. 대상자의 17.4%가 ‘자주’ 또는 ‘항상’ 시속 16 km(10 miles)이상의 과속을 하는 것으로 나타났다. 과속 여부에 영향 요인을 분석한 결과, 회귀 모형의 설명력은 38.1%이며, 비흡연자보다 흡연자가(OR=4.25, 95% CI=1.46-12.38), 비음주자보다 음주자가(OR=7.38, 95% CI=1.86-29.30), 과속할 확률이 더 높았다. 5. 대상자의 44.7%가 직업운전자 근무경력 중 교통사고 기록이 있었고, 24.8%는 지난 1년동안 교통사고를 경험한 것으로 나타났다. 대상자의 24.2%는 지난 1주일동안 아차사고(a near miss)를 경험한 적이 있으며, 41.6%가 지난 1년동안 교통법규 위반 고지를 받은 적이 있다고 보고하였다. 지난 1년간 교통사고 경험 여부에 영향 요인을 분석한 결과, 회귀 모형의 설명력은 20.4%이며, 수면의 질이 낮을수록(OR=1.29, 95%CI=1.08-1.54), 교통사고 경험할 확률이 더 높았다. 본 연구에서 직업운전자의 높은 자각피로도, 낮은 수면의 질, 과다 주간졸림증, 나쁜 정신적 건강상태와 차량 종류, 장시간의 일일 근무시간이 교통사고 위험성에 영향을 미치는 것을 확인하였다. 따라서, 직업운전자 직업 특성에 알맞은 피로 관리, 수면의 질 또는 신체적·정신적 건강상태를 향상시킬 수 있는 건강관리 간호중재안 개발을 필요로 한다. 또한, 직업 환경을 고려한 교통사고 위험성을 감소시키는 방안에 대해 해당 기관과 지역사회의 정책 마련이 필요함을 시사한다.
