범용상아질접착제가 CAD/CAM 수복물의 교합 변화에 미치는 영향에 관한 무작위 배정 임상 연구

Abstract

본 무작위 배정 임상 연구의 목적은 CAD/CAM 수복물을 RelyX™ Ultimate Clicker™(“RelyX Ultimate”, 3M ESPE, Neuss, Germany)로 합착 시 범용상아질접착제에 따라 세라믹 수복물의 접착 전 후의 교합의 변화에 영향을 미치는지 평가하기 위함이다. 이를 위하여 실험군으로 Tetric N-Bond Universal(“TNU”, Ivoclar Vivadent, Schann Liechtenstein)을 사용하였으며, 대조군으로 Single Bond Universal(“SBU”, 3M ESPE, Neuss, Germany)을 사용하였다. TNU는 RelyX Ultimate으로 합착을 하기 전에 광중합을 시행한 반면, SBU는 RelyX Ultimate 합착과 동시에 광중합을 시행하였다. 2016년 4월부터 2016년 10월까지 연세대학교 치과대학병원 보존과에서 CAD/CAM 수복물 치료를 받은 환자를 대상으로 Blue ceramic 상태를 기준으로 하여 1차 결정화 과정 및 광택 과정 시, 2차 최종 합착시를 교합지 및 심스탁과 환자의 불편감으로 교합이 높아졌는지 평가하였고, 최종 합착 시 상태를 기준으로 하여 재 내원 시 교합이 높아졌는지를 3차 평가하였다. 범용상아질접착제간의 차이를 분석하기 위해 fisher’s exact test, Chi-square test를 SPSS 프로그램 ver.23.0으로 통계처리 하였다. 수복된 총 39개 case 중 29개의 case가 연구에 포함되었으며, 1차 평가와 2차 평가에서 교합이 높게 평가 된 경우는 TNU에서 1 case로 전체 수복물의 3.4%뿐이었다. 3차 평가에서 교합이 높게 평가 된 경우는 SBU에서 4 case였다. 하지만 이는 모두 통계적으로 유의미한 차이가 없었으며, 교합이 높아지는데 영향을 미치는 변수 또한 통계적으로 유의미한 영향을 미치지 않았다. 따라서, 위의 결과를 종합해 볼 때 범용 상아질 접착제가 CAD/CAM 수복물을 RelyX Ultimate로 합착 시 합착 전 후의 교합 변화에 영향을 미치지 않았다.

The present randomized controlled trial was aimed at evaluating changes in the occlusion of ceramic restorations produced with CAD/CAM before and after attachment by using RelyX Ultimate according to universal dentin adhesives. In regard to the type of adhesive used, Tetric N-Bond Universal (TNU; Ivoclar Vivadent, Schann, Liechtenstein) was used for the experimental group and Single Bond Universal (SBU; 3M ESPE, Neuss, Germany) was used for the control group. TNU was subjected to light curing before mixing it with RelyX Ultimate. SBU was subjected to light curing while being mixed with RelyX Ultimate. This study included patients who underwent CAD/CAM restoration treatment in the department of conservative dentistry in Yonsei University Dental Hospital between April 2016 and October 2016. We measured occlusion after crystallization & glazing, and final adhesion by using articulating paper and Shimstock, and by evaluating the discomfort felt by the patients. Then, we compared it with occlusion in the blue ceramic state. We also measured occlusion during the patients’ second visit to the clinic and compared it to the occlusion measured for the last time after the final adhesion. To analyze the differences between the universal adhesives, Fisher exact and chi-square tests were performed by using the SPSS ver. 23.0 software. Of 39 restoration cases, 29 were included in this study. An increase in occlusion was observed in only one case of TNU after crystallization and glazing, accounting for 3.4% of all cases. In four SBU cases, an increase in occlusion was found after the final adhesion. However, the increase in occlusion was not statistically significant. The influence of the variables that contributed to the increase in occlusion was not statistically significant. Based on the findings, the universal dentin adhesives does not affect the occlusion before and after attachment produced with CAD/CAM by using RelyX Ultimate.
