Abstract
독립적인 생활이 가능한 경도 치매환자도 지속적인 인지 기능의 저하로 인해 숫자를 처리하고 계산하는 과정에서 어려움이 발생한다. 이에 본 연구에서는 알츠하이머성 치매 환자 및 일반 노인 각 16명씩을 대상으로 숫자 표기 방법에 따른 숫자 처리 및 계산 과제를 실시하였다. 환자군은 정상군에 비해 숫자 처리와 계산 수행 시 오류율과 오류 유형에서 어떠한 차이가 있는지 살펴보았다.
연구 결과에 따르면 첫째, 환자군은 정상군보다 숫자 산출 및 숫자 변환(숫자 받아쓰기, 숫자 읽기, 한글 변환)에서 오류율이 높았다. 숫자 처리 오류 유형은 환자군이 정상군보다 다양하였고, 두 집단 모두에서 구문적 오류가 가장 높았다. 둘째, 환자군은 정상군보다 덧셈과 곱셈에서 오류율이 높았다. 계산의 오류 유형 또한 환자군이 정상군보다 다양하였으며, 두 군에서 모두 산술 규칙 오류와 사실적 오류가 가장 많았다. 여러 오류 유형 중, 받아내림 오류가 환자군에서 더 적게 나타난 원인은 정상군에서 암산으로 과제를 수행한 대상자가 많았기 때문이다. 셋째, 숫자 처리와 계산 사이에는 이중 해리가 있었다.
본 연구를 통해 숫자 표기법에 따라 분류된 아라비아 숫자와 한글 숫자로 치매군에서 나타나는 숫자 처리 및 계산의 오류를 제시하고, 숫자 처리를 신경인지장애의 선별 문항으로 제안한 점에서 임상적 의의를 찾을 수 있다. 후속 연구에서는 한국어의 음운 특성을 고려하여 숫자 자극을 선정하고, 의미 기억의 영향이 최소한으로 적용되는 질문을 개발하여 치매 환자의 숫자 산출 특성 연구가 이루어져야겠다.
나아가 치매의 종류, 중증도 등의 기준을 설정하여 치매군 내에서의 숫자 처리 및 계산 특성을 비교 분석하는 연구가 이루어지길 기대해 본다.


In spite of mild dementia patients' capacity for independent living, they still go through difficulties in dealing with number processing and calculation caused mainly by continuous deterioration in cognition. This study conducted tasks of number processing and calculation by number notation. The subjects of the study were comprised of two groups, an Alzheimer’s disease(AD) group(n=16) and a healthy elderly group(n=16). The study compared the error rates and error types in number processing and calculation between the two groups.
The results are as follows: Firstly, the AD group showed significantly higher errors in number production, number dictation, number reading and transcoding to verbal numeral compared with the controls. The AD group presented more diverse error types in number processing. Both groups had the highest error rates in syntactic errors. Secondly, the AD group demonstrated remarkably higher errors than the control group in subtraction and multiplication. In addition, the AD group showed higher diversity than the control group in error types of calculation. Arithmetic rule errors and factual errors were the two most frequent errors
in both groups. Notably, the AD displayed lower rates in borrowing errors than the controls. It may be due to increase in errors in the control group as most of the subjects performed mental calculation. Thirdly, there was a double dissociation between number processing and calculation.
This study is significant in that it had dealt with a detailed analysis in number processing, specifically number transcoding to verbal numeral. Further studies may be carried out in the effect of semantic memory and number stimulus after considering phonemic features of the Korean language. In addition, comparative analysis of number processing and calculation characteristics among AD patients with different dementia types and severity would strengthen the evidence for clinical applications.
