진료협력센터 간호사 역할분석과 의료서비스 소요기간 및 환자만족도

Abstract

본 연구는 서울시 소재 일개 상급종합병원의 단위기관인 심장혈관병원 진료협력센터 간호사의 업무를 확인하고, 진료협력센터 의뢰군과 일반 의뢰군 간 외래진료 및 입원치료에 소요되는 기간과 환자만족도의 차이를 확인하여 진료협력센터 간호사 역할의 효과에 대한 기초적인 자료를 제공하기 위한 서술적 조사연구이다. 진료협력센터 간호사 업무의 내용과 빈도는 심장혈관병원 진료협력센터 간호사가 2016년 10월 24일부터 10월 28일까지 체크리스트를 이용하여 자료 수집하였다. 연구대상자는 2016년 4월 4일부터 9월 30일까지 심장내과 외래에 내원한 초진환자 중 선정기준에 부합되는 진료협력센터 의뢰군 64명, 일반 의뢰군 64명으로 총 128명을 선정하였다. 진료협력센터 의뢰군과 일반 의뢰군의 환자만족도와 일반적인 특성은 구조화된 설문지로 자료를 수집하였고, 의뢰관련 특성과 진단 및 치료에 소요되는 기간은 의료정보시스템과 전자의무기록 조회를 이용하여 확인하였다. 자료 분석은 SPSS/WIN 20.0 프로그램을 이용하여 기술 통계, independent t-test를 실시하였다.

본 연구의 주요결과는 다음과 같다. 1. 심장혈관병원 진료협력센터 간호사의 업무는 독립적인 역할 38.3%, 의존적인 역할 37.0%, 상호의존적인 역할이 24.7%로 나타났다. 2. 진료예약 의뢰일부터 외래 초진일까지 소요기간은 진료협력센터 의뢰군이 2.41(±2. 46)일로 일반 의뢰군의 6.88(±6.10)일 보다 짧았으며, 통계적으로 매우 유의한 차이 가 있었으며(t=-5.079, p<.001), 외래 초진일부터 2번째 외래 내원일까지 소요기간은 진료협력센터 의뢰군이 2.82(±2.32)일로 일반 의뢰군 13.57(±5.17)일보다 통계적으로 유의하게 짧았다(t=-14.197, p<.001). 3. 진료협력센터 의뢰군과 일반 의뢰군 간 외래 초진일부터 수술일까지 소요기간은 진료협력센터 의뢰군이 2.29(±1.38)일로 일반 의뢰군의 11.88(±9.95)일보다 짧았으며, 통계적으로 유의한 차이를 보였다(Z=-2.693, p=.007). 4. 환자만족도는 80점 만점에 진료협력센터 의뢰군이 72.28(±7.22)점으로 일반 의뢰군 68.39(±8.22)점보다 통계적으로 유의하게 높았다(t=2.764, p=.007). 예약 및 접수 절차 (t=2.078, p=.040), 간호사 진료서비스(t=2.070, p=.041), 병원환경 및 기타 (t=2.657, p=.009) 영역의 환자만족도에서 진료협력센터 의뢰군이 일반 의뢰군 보다 통계적 으로 유의하게 높게 나타났다. 5. 진료협력센터 의뢰군의 진료협력센터 업무에 대한 환자만족도는 4점 만점 중 외래 진료 추천의향 3.91점, 검사진행 및 결과 확인일까지 소요기간 3.89점, 진료 분야 결 정 3.88점, 연속적인 의료서비스 제공 3.86점, 간호사의 충분한 설명과 안내 3.82점, 진료예약 대기일 3.82점, 입원일까지 대기일 3.81점, 예약 절차 3.78점의 순서로 모 든 문항에서 환자만족도가 3.7점 이상으로 높게 나타났다. 6. 진료협력센터 의뢰군이 가장 필요하다고 생각하는 진료협력센터 업무는 외래진료 및 검사진행의 신속성이 40.6%, 간호사의 충분한 설명과 상담 32.8%, 외래진료부터 입원, 수술 진행의 신속성 14.1%, 진료 분야 상담 및 결정이 12.5% 순서로 나타났 다.

본 연구는 의료서비스 전달 과정에서 진료협력센터 간호사의 역할을 독립적, 의존적, 상호의존적인 역할의 업무로 확인하고, 진료협력센터 의뢰군과 일반 의뢰군 간 외래진료와 입원치료 소요기간과 환자만족도의 차이를 확인하여 진료협력센터 간호사 역할의 효과를 확인하는 시도로서 의의가 있다. 이러한 연구결과를 바탕으로 향후 연구대상자와 변수를 확대시켜 진료협력센터 간호사의 역할과 환자결과에 대한 상관관계와 영향요인을 파악하기 위한 연구 및 진료협력센터 간호사의 역할이 간호의 전문분야의 하나로 자리매김하기 위한 진료협력센터 간호사의 표준화된 실무와 간호활동에 대한 수가를 마련할 수 있는 연구를 제언한다.

The purpose of this study was to identify nursing performance at the referral center of a cardiovascular hospital, a unit of a tertiary general hospital in Seoul, South Korea, and to analyze the differences in the length of time spent for outpatient treatment and hospitalization and patient satisfaction between the group of patients who came in through the referral center (the referral group) and the group of patients who came in through different routes (the general group). The contents and frequency of tasks of nurses at the referral center were obtained from October 24, 2016 to October 28, 2016 by the researcher of this study, who is a nurse at the referral center, using a checklist. A total of 128 patients were selected among the new patients who visited the cardiology outpatient clinic from April 4, 2016 to September 30, 2016, as study participants. The satisfaction and general characteristics of the patients were analyzed by using a structured questionnaire. The referral-related characteristics and duration for the diagnosis and treatment were investigated by using the medical information system and reviewing the electronic medical records. The collected data were analyzed by using the SPSS/WIN 20.0 program for descriptive statistics and independent t-tests.

The results of this study are summarized as follows: 1. The tasks of nurses at the referral center were identified as 38.3% of independent role, 37.0% of dependent role, and 24.7% of interdependent roles. 2. The duration between the date of an appointment was made and the date of first visit to the outpatient clinic was calculated as 2.41(±2.46) days in the referral group, statistically shorter than 6.88(±6.10) days in the general group (t=-5.079, p<.001).
