통증관리 교육이 간호사의 지식과 태도, 환자의 만족도에 미치는 효과 : 간 절제술 환자를 대상으로

Abstract

통증은 수술 후 환자의 예후와 만족도에 영향을 미치는 중요 변수이므로 효과적인 관리가 요구된다. 효과적인 통증관리에 있어 중요한 것은 환자에게 중재를 수행하는 간호사의 통증관리에 대한 지식과 태도의 적절성이므로, 간호사의 지식과 태도 향상을 위한 통증관리 교육이 필요하다. 따라서 본 연구는 간호사에게 시행한 통증관리 교육이 간호사의 지식과 태도, 환자의 통증관리 만족도에 미치는 효과를 확인하기 위해 시도되었다. 본 연구는 통증관리 교육 전, 후 간호사의 통증관리에 대한 지식과 태도 변화를 확인하기 위해서 비 동등성 대조군 전후 설계방법을 사용하였으며, 간호사를 교육시킨 효과로 환자의 통증관리 만족도가 변화되는지를 확인하기 위해서 모의대조군 전후 설계방법을 사용하였다. 서울시 일 상급 종합병원의 외과병동 간호사 35명과 간 절제술 후 환자 60명이 연구에 참여하였으며, 통증관리 교육에 참여한 간호사는 18명이었다. 간호사 교육용 자료는 연구자가 문헌고찰을 통해 개발한 후 전문가에게 내용 타당도 검증을 받았고, 간호사의 통증관리에 대한 지식과 태도를 확인하기 위해서 Ferrell and McCaffery(2014)가 개발한 “Knowledge and Attitudes Survey Regarding Pain” 도구를 연구자가 수정, 보완하여 사용하였으며, 환자의 통증관리 만족도는 문헌고찰을 통해 연구자가 개발한 도구를 사용하였다. 수집된 자료의 분석은 x^2-test, Fisher’s exact test, t-test, Mann Whitney test, Wilcoxon Signed Rank test를 이용하였다. 문헌고찰을 통해 개발한 통증관리 교육자료의 전문가 내용 타당도는 모든 항목에서 .80이상으로 확인되었으며, 실험군 간호사의 통증관리에 대한 지식과 태도는 교육 전 21.61±2.50점에서 교육 후 26.72±3.18점으로 통계적으로 유의한 상승을 보였다(p=<.001). 실험군 간호사에게 간호를 받은 환자의 통증관리 만족도 역시 교육 전 3.00±0.93에서 교육 후 4.13±0.83으로 통계적으로 유의하게 상승되었다(p=.002). 따라서 본 연구를 통해 통증관리 교육이 간호사의 통증관리에 대한 지식과 태도를 향상시키는데 효과적이었으며, 간호사의 통증관리에 대한 지식과 태도가 향상되면 환자의 통증관리 만족도 역시 상승할 수 있다는 것이 확인되었다.

Background: Pain is an important variable affecting the prognosis and satisfaction of the patient after surgery, so effective management is needed. What is important in effective pain management is the adequacy of knowledge and attitudes about pain management in nurses practicing interventions for patients. Therefore, it is necessary to provide pain management education to improve nurse’s knowledge and attitude. Objective: The purpose of this study was to explore the effects of the pain management education on nurses’ knowledge, attitude, and patients’ satisfaction. Methods: This study was non-equivalent control group non-synchronized designed to examine effects of pain management education for nurses, and it’s effect was evaluated by using a pre and post-pain management education on nurses’ knowledge and attitude regarding pain management, and satisfaction of patients was evaluated by using simulated control group pretest-posttest design. Total 35 nurses and 60 patients post liver resection were participated in the analysis at general hospital’s surgical ward located in Seoul, and total 18 nurses of experimental group was participated in education program. Pain management education contents for nurses were developed through literature review by researcher, and then the educational contents were evaluated validity by experts group. The knowledge and attitude of nurses were collected by using “Knowledge and Attitudes Survey Regarding Pain” by developed Ferrell and McCaffery(2014), and it was modified and revised by researcher. The pain management satisfaction of patients was collected by using tool developed through literature review by researcher. Collected data analyzed by x^2-test, Fisher’s exact test, t-test, Mann Whitney test, and Wilcoxon Signed Rank test. Results: As for experts’ validity on pain management education contents, CVI is >.80 whole contents. The nurses’ knowledge and attitude of experimental group was significantly increased from pretest 21.61±2.50 to posttest 26.72±3.18 after the pain management education(p=<.001). Also, pain management satisfaction of patients in received nursing to experimental group nurses was significantly increased from pretest 3.00±0.93 to posttest 4.13±0.83(p=.002). Conclusion: This study showed that pain management education is very effective method in improving nurses’ knowledge and attitude regarding pain management, and as long as nurses’ improved knowledge and attitude about pain management, pain management satisfaction of patients had increased.
