뇌질환 환자의 절대안정에 대한 중요성 인식정도와 실행정도에 관한 조사연구

뇌질환 및 뇌손상에 관한 치료방법의 발달로 많은 환자의 사망을 줄이게 되었으나 아직도 다른 질병에 비해 사망율이 높으며 완전히 회복되어도 여러가지 후유증이 남아 가벼운 증상으로부터 심하면 사회생활을 하지 못할 정도로 다양한 증상을 보이고 있다. 이러한

여러가지 건강문제들은 초기에 뇌압상승의 조절 및 예방이 잘 시행될때 큰 위험을 막을 수 있다. 이에 대한 중요한 예방책의 하나는 절대안정 유지 즉 육체적인 활동의 제한과 정신적, 정서적 휴식상태를 갖게 하는 것이다.

그러므로 본 연구는 환자가 실제로 어느정도 절대안정을 중요하게 인식하고 있는지, 또 어느정도 실행하고 있는지를 조사해 보며 잘 실행되고 있지 않은 경우, 그 원인을 파악 분석하여, 치료목적 및 예후에 좋은 성과를 이를 수 있는 간호를 계획하는데 유용한 과학적 기초자료를 제공하기 위한 목적으로 시도되었다.

연구대상은 Y대 부속병원 신경과 및 신경외과 병동에 입원한 뇌질환환자로써 선정기준에 부합되는 총50명이었으며 자료수집은 1987년 10월 15일부터 11월 7일까지 21일간 질문지법과 면접법을 사용하여 수집하였다.

연구도구는 문헌과 신경외과에서 처방되어 사용하고 있는 절대안정 준수사항을 근거로 하여 본 연구자가 개발 제작하여 사용하였다.

자료를 분석하기 위해 일반적 특성은 인수와 백분율로 산출하였고, 환자의 절대안정에 대한 중요성 인식정도와 실행정도에 대해서는 최소값, 최대값, 평균값, 표준편차, 평균평점을 산출하였고 환자의 절대안정에 대한 중요성 인식정도와 실행정도와의 관계를 파악하기 위해서는 Pearson 적률상관계수를 이용, 분석하였다.

한편, 환자의 일반적 특성과 절대안정에 대한 중요성 인식정도와 실행정도의 관계를 알아보기 위해서는 t-test, ANOVA로 분석하였다.

본 연구의 결과는 다음과 같다.

1. 절대안정의 중요성 인식정도는 평균이 56.660, 최소점수가 34.00, 최대점수가 70.00이었고 표준편차는 10.307이었다

그리고 중요성 인식정도가 가장 높은 항목은 "가족이나 주위사람들이 스트레스나 자극적인 말이나 행동을 하지 않게 해야한다"이었고 중요성 인식정도가 가장 낮은 항목은 "체위 변경은 환자 자신이 해서는 안된다."로 나타났다.

2. 절대안정의 실행정도는 평균이 45.180, 최소점수가 26.000, 최대점수가 68.000이었고 표준편차는 11.367이었다. 그리고 실행 정도가 가장 높은 항목은 "가족이나 주위사람들이 스트레스나 자극적인 말이나 행동은 하지 않게 해야 한다"이었고 실행정도가 가장

낮은 항목은 "체위변경은 환자 자신이 해서는 안된다"로 나타났다.

3. 절대안정의 중요성 인식정도와 실행정도와의 관계는 통계적으로 유의한 순상관관계가 있는 것으로 나타났다( r = .2991, p=.017).

4. 절대안정의 중요성 인식정도와 일반적 특성과의 관계에서는 종교만이 통계적으로 유의한 관계가 있는 것으로 나타났다( F= 2.797, P = .051).

5. 절대안정의 실행정도와 일반적 특성과의 관계에서는 질환명만이 통계적으로 유의한 관계가 있는 것으로 나타났다 ( t = 4.009, P=.025)

이상의 결과를 고찰해 볼때 환자의 절대안정의 중요성 인식정도가 높을수록 실행정도가 높음을 알 수 있었다.

그러므로 절대안정이 처방된 환자 간호시 절대안정에 대한 중요성 인식정도를 높여주고 절대안정을 잘 실행할 수 있도록 돕는 간호교육의 필요성을 제시하고 있다.

This descriptive study was undertaken in order to determine if there is a relationship between the level of perception and the level of performance in regard to absolute bed rest in patients with cerebral disease.

The purposive sampling technique was used in selection of subjects. The 50 patients in this study were admitted to the department of Neurology and Neurosurgery in Y-University Hospital from Oct. 15th to Nov. 7th, 1987.

The instrument used for this study was developed by the researchers based on references and guidelines used for absolute bed rest in the Neurosurgery department of Y-University Hospital and it consisted of 14 items, each with a five point Likert type scale.

The data were analyzed by means of t-test, Pearson Correlation and ANOVA according to purpose of the study and S.P.S.S. was utilized at the Computer Center on Yonsei University.

The results of this study are summarized as follows :

1. The level of perception of absolute bed real had a mean of 56.600 and a minimum of 34.00, and a maximum of 70.00 and a standard deviation of 10.307.

2. The level of performance of absolute bed rest had a mean of 45.180, and a minimum of 26.000, and a maximum of 68.000 and a standard deviation of 11.367.

3. There was a significantpositive correlation between the level of perception and performance of absolute bed rest in cerebral disease patients (r=.2991, p=.017).

4. The only significant difference between the level of perception of absolute bed rest and the demographic characteristics wag religion (F=2.797, p=051).

5. The only significant difference between the level of performance of absolute bed rest and the demographic characteristics was the diagnosis (t=4.009, p=.025).

In conclusion, this study revealed that when there is a higher level of perception, there is a higher level of performance of absolute bed rest in cerebral disease patients. Therefore, nurses must be aware that is important to estabilish effective nursing care plans and educational programs to increase the patients' for the need of absolute bed rest.
