만성 관절통으로 내소한 일부 농촌 주민의 치료행위 조사

한국의 농촌주민 상병조사에서 신경감각계 질환은 비교적 높은 순위의 유병률을 나타내고 있으며, 근년에 이르러 이 질환의 유병률이 점차 상승 추세에 있다는 사실은 이 질환으로 인한 농산물생산의 감소가 국가의 경제적인 손실에 영향을 미치는 농업종사자의 심각한 건강 문제라 할 수 있다. 따라서 농촌지역에서 이러한 관절통 환자들의 질병실태와 치료행위를 조사함으로서 앞으로의 농촌보건진료원의 통증환자관리에 대한 기초자료를 제공하고자 한 것이 이 연구의 목적이다.

조사대상의 설정은 전라북도의 농촌지역의 한 보건진료소에 1개월이상 지속적인 관절의 통증을 호소하여 1984년 6월 1일부터1984년 10월 31일까지 내소한 농업종사자로 정하였

다. 자료의 수집은 설문지에 의한 면접조사자 98명과 면접조사자중 진단을 위한 혈액채취자 69명에 대한 혈액검사를 하였다.

조사결과의 주요내용은 다음과 같다.

1. 혈액검사 (ASL, CRP, RA )의 양성반응자는 69명 중 17명(24.6%) 이었다.

2. 통증환자들의 치료 행태는 1차 치료방법의 선택은 약국, 2차의 선택은 한약, 3차의 선택은 침술, 4차의 선택은 뜸, 5차의 선택은 민간요법이나 홀몬주사를 이용한 순위였고 병원치료는 1차 선택에서 소수의 사람만이 이용했다.

3. 통증환자의 치료 결과에 대한 만족도는 혼합치료(양약, 한약 침술등)자에게서 19.4%만 적혈구 침강속도(ESR)의 정상치 범주에 속했다. 환자의 치료후 호전감과 ESR의 정상여부와는 관련이 없는 것으로 나타났다.

4. 혈액검사 (ASL, CRP, RA) 양성 반응군의 적혈구침강속도 (ESR)의 평균치는 27.8이었고 음성반응군의 평균치는 20.2로 둘 다 적혈구침강속도(ESR)의 비정상치 범주를 벗어나지 않았다.

A Survey of Rural Residents' Treatment Behavior on having Joint Pain

Recent surveys in rural Korea indicate that neurosensory diseases occupy a high

rate of! frequencies, and this rate is currently on the increase.

This disease cause a serious problem of farmers' losing field activities.

Therefore, this survey is aimed to observe treatment behavior of rural farmers

with having joint pain.

Selection of patients was from the rural residents who visited the local health

and medical care unit, located in Cholla Bukdo, after having joint pain for more

than one month lasted.

Questionnairs and blood test such as ESR, ASL, CRP, RA were the main tools for

collecting statistical information on the patients. A total number of 93 patients

were responded to questionnair survey, and a total number of 69 patients were

responded to blood test.

Major findings were summarized as follows :

1. Blood tests such as ASL, CRP and RA gave positive rate wart 24.6 perdent.

2. For treatment the patients selected drug stores in the first preference, herb

drug in the second, acupuncture in the third, moxacautery in the fourth, finally

folk remedies or hormone injection. First preference of clinic was negligible and

having non-treatment was 22 percent.

3. The mean value in the positive group of ESR blood text t belonged to an

abnormal value category.

From the above findings, it is necessary to set up a new education and training

system of the Community Heat th Practitioners for efficient management of patients

with joint pains wi thin primary health care system in the community.
