현역 장교, 하사관의 의료이용행태를 분석하여 현행 현역 장교, 하사관의 의무지원제도의 합리적이고 효율적인 개선방안을 모색하기 위해 1990년 9월 27일부터 10월 5일까지 oo지역과 ooo지역에 근무중인 장교, 하사관 369명을 대상으로 설문조사를 실시하여 다음과 같은 결과를 얻었다.

1. 의료이용률은 2주간 동안 외래의 경우 백명당 14.9명이 군의무시설을 13.6명이 민간의료기관을 이용하였으며, 1년동안 입원인 경우 백명당 7.9명이 군의무시설을 4.1명이 민간의료기관을 이용하였다.

2. 2주간 동안 외래의 52%는 군의무시설을 48%는 민간의료기관을 이용하였으며, 1년동안 입원의 66%는 군의무시설을 34%는 민간의료기관을 이용하였다.

3. 가장 많이 이용하는 군의무시설을 외래의 경우에는 사단의무대, 입원의 경우에는 통합병원이었고, 가장 많이 이용한 민간의료기관은 외래의 경우에는 약국, 입원의 경우에는 병원이었으며 군의무시설을 이용한 가장 많은 이유는 거리가 가깝기 때문이었으며, 민간의료기관을 이용한 가장 많은 이유는 의료의 질이 좋기 때문이었다.

4. 의료기관 선택과 관련있는 변수는 외래의 경우에는 최기민간의료기관, 전, 공사구분, 이용시간, 이용시기였으며, 의료기관 선호경향의 경우에는 전, 공상구분, 이용시간, 중증도, 수술여부, 이용시기였다.

5. 군의무시설 만족도보다 민간의료기관 만족도가 높게 측정되었고, 민간의료기관에 대한 만족도는 장교, 하사관들의 의료기관 선택과 통계학적으로 유의한 관계가 있었다.

6. 다변수 로지스틱 회귀분석 결과 외래의 경우 장교, 하사관들의 의료기관 선택과 관련이 있는 변수는 최기 민간의료기관, 민간의료기관에 대한 만족도, 전,공상구분, 군의무시설에 대한 만족도가 선정되었고 각각의 비차비는 0.31, 1.33, 0.46, 0.90이었다. 의료기관 선호경향의 경우에는 전,공상구분, 이용시기, 군의무시설에 대한 만족도, 이용시간, 민간의료기관에 대한 만족도, 최기 군의무시설까지의 소요시간이 선정되었고 각각의 비차비는 0.37, 2.38, 0.83, 1.87, 1.18, 1.05였다.

7. 제도개선에 관한 의견은 98.4%가 개선필요성을, 99.2%가 의료보험제도의 적용을 희망하고 있었다.

장교, 하사관들의 의료이용은 제도상 군의무시설에서 무료로 의료서비스를 제공하고 있으나 다수의 장교, 하사관들이 민간의료기관을 이용하고 있었고, 대부분의 장교, 하사관들이 의료보험제도 도입을 통한 제도개선을 희망하고 있었다. 이러한 상황하에서 향후 군 의무지원제도의 개선은 지속적인 전투력유지를 위한 공백없는 의무지원을 보장함과 동시에 의료의 질을 상승시켜 항상 이용가능한 양질의 의료를 제공하여 군의 전력을 극대화하는 방향으로 추진되어야 할 것이다.



[영문]

This study was designed to analyze health care utilization behavior of officers and N.C.O's on active duty to improve the efficiency and effectiveness of the military health care utilization system. A questionnaire survey was conducted from

August 27 to October 5, 1990 and data were collected from 369 military officers and N.C.O's in one military A.P.T. and two army divisions.

The results were as follows:

1. Average ambulatory care utilization rates for military and civilian medical facilities were 14.9 and 13.6 per 100 persons during the two weeks period. Average inpatient care utilization rates for military and civilian medical facilities were 7.9 and 4.1 per 100 persons per year.

2. The utilization of military medical facilities and civilian medical facilities for outpatients during the two weeks period were 52% and 48%, respectively. The utilization of military medical facilities and civilian medical facilities for inpatients were 66% and 34%, respectively.

3. The most frequently utilized military medical facilities were military clearing station for outpatient and the general military hospital for inpatient. The most utilized civilian medical facilities were drugstore for outpatient and hospital for inpatient. The primary reason for the utilization of military medical facilities was geographical access for outpatient and medical cost for inpatient. The primary reason for the utilization of civilian medical facilities was quality of care for both outpatient and inpatient.

4. The variables which were significantly associated with the choice of medical facilities for outpatient were the nearest civilian medical facilities, injuring on duty, timing of visit and leave. In addition, the variables which were significantly associated with the choice of medical facilities for the preference of medical facilities were injuring on duty, timing of visit, severity, surgery and leave.

5. Civilian medical facilities were more preferred to military medical facilities. There was statistically significant relationship between the satisfaction with civilian medical facilities and the choice of medical facilities.

6. The stepwise multiple logistic regression showed that the factors influencing the selection of medical facilities were the nearest civilian medical facilities, satisfaction with civilian medical facilities, injuring on duty, satisfaction with military medical facilities for outpatients, with odds ratio of 0.31, 1.33, 0.46, 0.90, respactively. In addition, stepwise multiple logistic regression showed that the factors influencing the selection of medical facilities were injuring on duty, leave, satisfaction with military medical facilities, timing of visit, satisfaction with civilian medical facilities, time to reach the nearest military medical facilities for the preference of medical facilities, with odds ratio of 0.37, 2.38, 0.83, 1.87, 1.18, 1.05, respectively.

7. 98.4% of the respondants felt the need for improvement of the military medical utilization system, and 99.2% of the respondants wanted to be covered by the health insurance.

Based on these results, it is concluded that the future of military medical utilization system should support a continuity of care as well as a quality of care to achive the highest level of health status of military forces.
