조혈모세포이식 환아 가족의 교육자료 개발

본 연구는 조혈모세포이식 환아 가족과 조혈모세포이식병동 간호사를 대상으로 이식 단계별 교육요구를 파악하였으며, 조혈모세포이식을 받은 환아와 가족에게 적절한 정보를 제공하기 위한 간호중재의 하나로 방법론적 연구를 사용하여 교육자료를 개발하였다.

본 연구의 대상자는 서울시내 1개 대학병원의 조혈모세포이식을 받은 환아 가족 20명과 조혈모세포이식병동 간호사 20명을 대상으로 하였으며 직접면담과 설문지를 통해 자료를 수집하였다.

본 연구의 도구는 이영신(2001)이 개발한 골수이식환자 가족의 요구사정을 위한 도구 61문항을 기초로 하여 교육요구와 관련된 문항을 예비조사를 통하여 총 28문항으로 개발하였다.

연구 결과는 다음과 같다.

1. 조혈모세포이식 환아 가족이 요구하는 교육내용은 이식 단계별로 차이가 있었다.

2. 조혈모세포이식 환아 가족이 요구하는 이식 단계별 교육내용과 조혈모세포이식 병동 간호사가 이식 단계별로 선택하는 교육내용은 차이가 있었다. 즉 환아 가족은 환아의 신체적 간호 및 정보와 관련된 내용이었으며 조혈모세포이식 병동 간호사는 환아의 심리적, 정서적, 영적인 내용도 교육해야 한다고 생각하였다.

3. 조혈모세포이식 병동의 간호사가 교육내용에 적절하다고 선정한 교육방법은 소책자법 이었다. 이밖에도 교육목적에 따라 그룹교육, 비디오, 전문가 의뢰, 시범법 등을 선택하였다.

이상의 결과를 종합해 보면 조혈모세포이식 환아 부모의 교육요구는 이식 단계별로 차이가 있으며, 환아의 부모와 간호사가 요구하는 교육내용에도 차이가 있음을 알 수 있었다. 또한 이식 병동 간호사가 교육내용에 적절하다고 선정한 교육방법은 소책자법이었으며 본 연구자는 연구 결과를 근거로 하여 조혈모세포이식 환아 부모를 위한 이식단계별 소책자를 개발하였다.

[영문]

The main purpose of this methodological study to develop teaching material for families of children with Hemopoietic Stem Cell Transplantation(HSCT).

The study was conducted in one university hospital located in Seoul. In order to identify the areas in need of teaching, 20 families going through and 20 nurses working at the HSCT unit were included.

Data were collected a 28-item questionnaires developed by the researcher. The data were analyzed by using SPSS PC package.

The major results are as follow.

1. Families of children with HSCT had different teaching need related to the different phase of HSCT.

2. Families focused on patient’s medical status, therapies, complications and skills necessary to care for the patients. But, Nurses included the patient’s psychological, emotional and spiritual needs.

3. Nurses preferred a booklet as most effective methods. And other methods group meeting, video, referral to experts, and demonstrations.

In conclusion, Families of children with HSCT identified patient’s medical status, therapies, complications and skills related to HSCT as areas in need of teaching.

On the basis of this study, the educational booklet was developed for the pediatric HSCT families as an effective nursing intervention.

It is suggested that this booklet to be used in other HSCT units. Also, it should be revised to meet specific procedures and regulations of each unit.
