골육종 환자의 불안과 가족 부담감, 사회적 지지에 관한 연구

Abstract
[한글]본 연구는 골 육종 환자의 불안과 가족의 불안감, 사회적 지지를 측정하고 그 상관관계를 파악하여 환자와 가족에게 도움을 줄 수 있는 효과적인 중재 프로그램을 개발하는데 기초 자료를 제공하기 위해 시도된 서술적 상관관계 연구이다.

연구대상은 서울 소재 Y의료원과 W병원에서 2000년 9월 26일부터 11월 7일까지 골 육종이라고 진단을 받고 현재 치료 중이거나 치료가 끝난 환자와 환자의 가족을 대상으로 하였다.

연구도구는 구조화된 설문지로 환자의 불안을 측정하기 위해 Spielberger의 State-Trait Anxiety Inventory를 한국인에 맞게 번역한 김정택의 불안 검사를 사용하였고, 가족의 부담감은 서미혜와 오가실(1992)이 개발한 부담감 도구로 측정하였다. 사회적 지지정도를 측정하기 위한 도구는 박지원(1985) 에 의해 개발된 사회적지지 측정도구를 이용하였다.

수집된 자료는 SAS를 이용한 전산 처리하였으며 연구 대상자의 인구 사회학적 특성은 실수와 백분율로 연구 대상자의 인구 사회학적 특성과 환자가 지각하는 상태불안정도와 가족의 부담감, 사회적 지지는 산술평균, 표준편차 t-test, ANOVA로 분석하였고 환자가 지각하는 상태불안정도와 가족의 부담감, 사회적 지지와의 관계는 Pearson Correlation Coefficient를 이용하였다.



본 연구의 결과는 다음 과 같다.

1. 골 육종환자가 지각한 불안(20문항, 4점척도)의 최대값은 80점이고 최 소값은 22점으로 불안의 평균점수는 50.5점 각 항목의 평점은 2.52로 나 타나 환자의 불안은 높은 것으로 나타났다.

2. 보호자가 지각한 가족 부담감(25문항, 5점척도)의 최대값은 116점, 최소 값은 33점으로 그 차이가 상당히 크며 평균점수는 82.2이고 각 항목의 평점은 3.28로 나타나, 부담감이 상당히 높은 편이었으며, 가장높은 부 담감을 나타낸 항목은 “나는 환자에게 필요한 사람이다”로 평점4.69 “나는 환자에게 도움을 주고 있다”평점 4.30의 높은 점수를 보여 환자 에게 많은 도움을 주고 있다고 생각하는 것으로 나타났고,“환자에게 드는 비용 때문에 경제적 여유가 없다”가 평점 4.0 “환자에게 드는 비용이 너무 비싸다”가 평점4.43으로 경제적인 측면에서 높은 부담감 을 가지고 있는 것으로 나타났다.

3. 보호자가 지각한 사회적지지(25문항, 5점척도)의 최대값은 109점, 최소 값은 25점이며 평균값은 76.4점으로 나타났고 각 항목의 평점은 3.05로 나타났으며, 사회적 지지의 영역별 점수를 보면 정서적지지 평점 3.15, 정보적 지지 평점 3.16, 평가적 지지 평점 3.11. 물질적 지지 평점 2.78 로 나타나 보호자들은 물질적 지지를 가장 못 받고 있다고 생각하는 것 으로 나타났다.

4. 환자 가족이 지각하는 사회적 지지와 가족 부담감과의 관계는 r=-.152로 역 상관 관계가 있는 것으로 나타났으나 통계적으로 유의하 지 않았다.

5. 골 육종 환자가 지각하는 불안과 사회적 지지와 관계는 r=-.144로 역 상관 관계가 있는 것으로 나타났으나 통계적으로 유의하지 않았다.



이러한 결과로 볼 때 골 육종 환자의 불안과 가족의 부담감은 비교적 높으나 이들이 느끼는 사회적 지지는 비교적 낮다고 볼 수 있고, 경제적인 부담감과 환자의 미래에 관한 불안이 크다. 그러므로 간호사들은 골 육종 환자의 불안과 가족의 부담감을 감소시킬 수 있는 효과적인 중재 전략을 개발하는데 노력을 기울여야 할 것이라 생각되고, 특히 환자의 불안을 감소시키기 위한 정보제공과 경제적인 부담감을 감소시켜 줄 수 있는 사회자원 활용 방안 등을 마련하는데 적극적인 노력을 해야 할 것이라 여겨진다.



[영문]This study is the descriptive correlation research to measure Osteosarcoma patient's anxiety, family burden and social support; and to offer the basic data developing effective arbitration program which helps the patient's family by understanding the correlation of them. The object of this study is to survey the patients and their families who were diagnosed as Osteosarcoma from Sep. 26 to Nov. 7 in 2000 and are receiving medical treatments at present or finished the medical treatments.

The organized questionnaire was used;

In order to measure the patient's anxiety, we used Kim Jung Taik's anxiety test which had been translated Spielberger's State-Trait Anxiety Inventory and Seo Mi Hea & Oh Ga Sil's means of burden (1992) had been utilized to measure the family burden. On the other side, Park Ji Won's measuring means for social support was used. The collected data were calculated by using SAS by computer. The patient's social characteristics from the point of population has been analyzed by a real number and one hundredth part percentage. The feeling of condition instability degree, social support and family burden has been measured in terms of arithmetic mean. Standard deviation t-test, ANOVA and Pearson Correlation Coefficient has been used to correlate the family burden, social support and condition instability degree which patient feels.



The results of the study are as follows:



1. The anxiety maximum which Osteosarcoma patient feels is 80 points and the minimum is 22 points. On the other hand, anxiety mean is 50.5 points, and each article's mean is 2.25 points. These results shows that patient's anxiety is high.



2. The maximum of family burden (25 questions, 5 points measure) which guardians feel is 116 points, and the minimum is 33 points. The difference between the two is considerably big. The mean is 82.2 points and each article's mean is 3.28. This shows that family burden is pretty high. The article of the highest burden is " I′m a necessary person to the patient" shows high points 4.30. This high point say that patient"s family gives much help to him".

"Patient's family is economically in strained circumstances because of the cost for patient" shows 4.0 points and "the cost is very expensive" is 4.43 points. These articles reveals patient's family have a rather high burden in the view of economic side.



3. The maximum of social support (25 questions, 5 point measure) which patient's family feels is 109 points while the minimum is 25 points. The mean is 76.4 points and each article′s mean is 3.05 points. Considering the social support according to each field, emotional support is 3.15 points, information support is 3.16 points, evaluation support is 3.11 points and material support is 2.78 points. The patient's family thinks the material support is the worst field of all.



4. The relation between family burden and social support which patient's family feels record r = -.152. It also means inverse correlation, but has no meaning statistically.



5. The relation between social support and Osteosarcoma patient's anxiety shows r = -.144. It means that the relation is inverse correlation, but has no important meaning statistically.



These results show that Osteosarcoma patient's anxiety and family burden are comparatively high, but social support is pretty low and that family's economic burden and patient's anxiety for the future are relatively high. Therefore all the nurses must take an effort to develop effective arbitration strategy diminishing Osteosarcoma patient's anxiety and family burden. Especially they must concentrate their energies for finding the methods which furnish the useful information for reducing patient's anxiety and try to supply the practical plan of social materials use in order to diminish the economic burden of patient's family.
