연세대학교 가족계획 크리닉의 자궁내 장치 초삽입 부인에 관한 연구

연세대학교 가족계획 크리닉에서 1972년 1월부터 1979년 12월까지 자궁내장치 시술을 받은 3,333명의 초삽입 유배우 가임여성을 대상으로 자궁내장치 초삽입시의 대상부인의 일반적인 사항과 출산력수준을 조사했으며, 자궁내장치 채택후 매달 기간이 경과함에 따른 중단원인별 사용중단율을 구하기 위해 생명표방법을 이용하여 중단확률을 구했다.

이 연구의 구체적인 목적은

1) 자궁내장치 초삽입 당시의 일반적인 특성과 출산력수준을 조사한다.

2) 1963년 1월부터 1971년 12월까지의 연세대학교 가족계획크리닉에서 조사연구한 자궁내장치 초삽입부인에 대한 결과와 본조사의 결과를 비교 분석한다.

3) 자궁내장치 채택후의 부작용과 중단원인을 매월 기간의 경과에 따라 분석한다.

4) 자궁내장치 채택후 24개월간 매월 기간의 경과에 따른 중단원인별 누적사용중단율을 생명표 방법에 의해 구하는 것 등을 연구목적으로 하는데 중요한 결론은 다음과 같다.

1. 대상부인의 0.2%는 19세 이전에, 6.0%는 20∼24세에, 34.9%는 25∼29세에 초삽입을 시행했고, 30∼34세에 초삽입한 부인은 35.2%였다.

2. 대상자의 평균임신횟수는 4.3회였으며 평균출산횟수는 2.6회였다.

3. 전체대상자의 65.8%는 인공유산의 경험을 가진 자였고, 34.1%는 무경험한 자로서, 무경험자의 연도별 인공유산횟수는 1972년의 27.4%에서 1979년 41.6%로 월등히 증가하였다.

4. 대상자의 평균생존자녀수는 2.5명이었다.

5. 마지막 임신종결후 초삽입까지의 기간은 6개월 이전의 초삽입이 51.6%였다.

6. 대상부인의 23.2%는 동통을, 15.5%는 출혈을 호소하였다.

7. 생명표방법에 의한 삽입시부터 24개월 동안의 누적사용률은 58.1%였고 누적사고임신율은 2.09 %, 누적자연배출율은 9.4 %, 누적제거율은 31.2%였다.

The author observed distribution of general and fertility levels, rates of termination for all reason rate of expulsion and removal and rate of pregnancy towards 3333 women who had been first inserted with lUDS from Jan. 1972 through Dec. 1979 in Yonsei University family planning clinic.

The purpose of this study can be specified as below:

1. To know the general aspect and fertility Level of women when they accepted the IUD firstly.

2. To compare the result of Previous study and that of this study.

3. To know monthly side effect and reason for termination after Insertion.

4. To investigate monthly rates and cumulation rates of termination, continuation, expulsion, pregnancy and removal through life table method.

As the results of this study fellowing conclusion was obtained.

1. Regarding the age distribution of women 25-29 years of age group ranked highest with 34.9% out of all women and that of 20-24 was 6.0% and 19 years below was 0.27.

2. The average frequency of gravidity was 4.3 times and the average frequency of parity was 2.6 times.

3. 65.8 percent of all women experienced the artificial abortion and 34.1 percent had never experienced the artificial abortion, annual number of non experienced artificial abortion was increased from 27.4% at 1972 to 41.6% at 1979.

4. The number of present living children was 2.5 in average.

5. Interval between last pregnancy and first insertion was mainly 6 month after last prognancy terminated (51.6%).

6. Out of total women 23.2% had pains and 15.5% had vaginal bleeding

7. The cumulative rates of termination through the life table told us that the rate of pregnancy was 2.09%, the rate of natural expulsion was 9.4%, the rate of removal was 31.2%, and the rate of continuing users was 58.1% through out 24 months.
