수술전 환자에 있어 수술실간호원 방문이 불안에 미치는 영향에 관한 연구

본 연구는 수술환자에 있어 수술실 간호원의 수술전 방문이 수술환자의 불안과 환자의 전반적인 안녕에 어떠한 효과를 미치는지를 규명하기 위한 목적에서 시도되었다.

환자가 입원하게 되면 흔히 내과적, 외과적 치료를 받게되는데 특히 수술환자의 경우는 수술과 마취에 대한 두려움과 불안에 쌓이게 되어 심리적인 압박이 더욱 증가하게 되고, 환자가 가지는 각종의 불안감은 질병의 원인, 경과, 나아가서 예후에까지 많은 영향을 미치게 된다. 그러므로 수술환자의 불안감을 중재, 완화시킴으로써 수술후 회복과 예후에 도움을 주는 계속적이고 질적인 간호를 수행하기 위하여 수술실 간호원의 수술전 방문은 필수불가결한 것이라 생각하였다. 이에 본 연구는 수술전 환자에 있어 수술실 간호원의 방문이 불안에 미치는 간호중재 효과를 분석, 규명하여 특수분야에서의 전인간호와 자율성을 증진시키는데 기여하고자 하였다.

The purpose of this study was to identify the effect of preoperative visits by operating room nurses on the patients level of anxiety and on general patient welfare in the postoperative period.

The patients, once hospitalized, usually have medical on surgical treatment. But in the case of preoperative patients, they might have some fears on the operation and anesthesia, which would increase their psychological stress and these sort of anxiety might affect the etiology, progress and even to the prognosis of disease.

Therefore, I think that the preoperative visit of operating room nurse is necessary to establish consistent and quality nursing care which will help their postoperative welfare by intervention and release their anxiety.

· This study is to increase the comprehensive nursing care and autonomy in special field by evaluate and identify the effect of nursing intervention of the visit by operating room nurses in preoperative patients.

· The subjects for this study were 40 patients hospitalized in Y University Hospital in Seoul, 40 patients who were suitable for the purpose of this study were selected and the quasiexperimental study was made by dividing them in two groups, one group is the patients who lad preoperative visit and the other did net have.

· The research tool used to measure the anxiety was Kim's tool which was translated STAI designed by Spielberger, and pulse measunrments were done by SERVOMED in EKG equipment manufactured by HELLIGE company and B. P. were measured in Rt & Lt brachial artery by B.P manometer attached in anesthesia machine.

· Data collection was done from 0ct. 12, 1987 to Nov. 14, 1987 and analysis of data was done by use of percentage, t-test. AN0VA. Pearson Correlation Coefficients.

The results of this study was as follows;

1. Hypothesis I: The hypothesis that "In Preoperative patients, there will be a difference in the level of state anxiety in the operating room for subjects who had and those who did not have visit by operating room nurses," was supported.(t=3.04, p<.01)

2. Hypothesis Ⅱ: The hypothes is that "In preoperative patients, there will be a difference in the level of state anxiety 24 hours postoperatively for subjects who had and those who did not have visits by operating room nurses." was rejected

(t=-0.4, p>.05)

3. Hypothesis Ⅲ: The hypothesis that "In preoperative patients, there will be a difference in the change of vital sign (pulse. B.P.) between preoperative evening and preanesthesia time for subjects who had and those who did not have preoperative visits by operating room nurses." was rejected. (BP: t=-1.21, p>.05 pulse t=1.16, p>.05)

4. The analysis of varience showed that the higher trait anxiety, the higher change of pulse. (r=.6019, p<.01)

5. In control group there was positive relationship between trait anxiety. state anxiety in operating room and state anxiety 24 hours postoperatively.

That is, the higher the trait anxiety, the higher the state anxiety and the higher the state anxiety in the operatively.(r=.4813. p<.05, r=.5978. p<.O1).

In conclusion, the nursing intervention of preoperative visit by operating room nurses which reduced the anxiety and released psychological stress, and made the preoperative patients comfortable, is supported.
