Abstract

본 연구는 치매환자를 돌보는 간호사의 대처유형과 소진, 우울을 파악하고 이 변수들간의 관계를 분석하기 위한 서술적 조사연구이다. 본 연구는 종합병원, 정신병원, 노인(전문)요양병원, 그리고 노인요양시설에서 치매환자를 주로 돌보는 간호사 121명을 대상으로 하였다. 자료 수집은 구조화된 설문지를 이용하였고 자료 수집기간은 2008년 4월 1일부터 5월 26일까지였다. 연구도구로는 치매환자를 돌보는 간호사의 스트레스 상황에서 대처유형은 Lazarus와 Folkman(1984)이 개발한 Ways of coping을 한정식과 오가실(1990)이 번역하여 수정?보완한 도구로 측정하였고, 소진은 Maslach와 Jackson(1981)이 개발한 MBI(Maslach Burnout Inventory)를 장은숙(1995)이 번역한 도구를 사용하여 측정하였다. 또한 우울은 Beck(1967)의 Depression Inventory(BDI)를 이영호와 송종용(1991)이 한국의 성인 집단을 대상으로 표준화시킨 도구를 사용하였다. 자료분석은 SAS 9.1을 이용하여 전산통계 처리하였다.

연구결과는 다음과 같다.

1. 치매환자를 돌보는 간호사의 대처유형 평균 점수는 86.53점(점수범위 33~132), 문항별 평균 점수는 평균 2.62점(점수범위 1~4)이었고, 각 영역별 대처유형 중 가장 많이 사용하는 항목은 문제중심 대처로 2.91점이었다. 소진의 평균 점수는 66.15점(점수범위 22~154), 문항별 평균 점수는 평균 3.01점(점수범위 1~7)이었고, 각 영역별로는 개인적 성취감 감소가 3.47점으로 가장 많이 경험하였다. 우울정도는 평균 점수가 7.07점(점수범위 0~63)으로 정상범주에 속하였으며, 총 121명의 대상자 중 정상상태 87명(71.9%), 경한 우울상태 24명(19.8%), 중등도 우울상태 7명(5.8%), 그리고 심한 우울상태는 3명(2.5%)으로 나타났다.

2. 치매환자를 돌보는 간호사의 일반적 특성과 주요 변수들 간의 차이를 검증한 결과 대처유형은 치매교육 유무(t=-2.3, p<0.5)에서 유의한 차이를 보였고, 소진은 근무지(F=7.84, p<.00)에서 유의한 차이가 있었다. 우울과는 결혼 상태(t=2.51, p=.01), 임상경력(F=3.15, p=.03), 치매환자 간호경력(F=3.35, p=.02), 그리고 근무형태(t=2.54, p=.01)에서 유의한 차이를 보였다.

3. 주요 변수들 간의 관계를 검증한 결과 치매환자를 돌보는 간호사의 총 임상경력은 치매교육(r=.21, p=.02)과 양의 상관관계, 우울(r=-.21, p=.02)과는 음의 상관관계가 있었다. 대처유형 중 문제중심 대처는 소진(r=-.25, p=.01)과 음의 상관관계가 나타났으며 소진은 우울(r=.28, p=.00)과 양의 상관관계가 있었다.

본 연구의 결과 치매환자를 돌보는 간호사들은 문제중심 대처유형을 가장 많이 사용하는 것으로 나타났고, 소진의 정도는 다른 연구결과와 비교해 낮게 나타났다. 또한 우울 정도는 정상범주에 속해 긍정적인 정신건강 상태였다. 치매환자의 공격행동은 간호사의 소진 및 우울과는 관련이 없는 것으로 나타났다. 이와 같은 결과는 치매교육을 받은 간호사가 다양한 대처유형을 유의하게 사용하기위해서 병원 및 시설 내 원내교육과 간호사 보수교육을 통한 치매간호교육 강화가 요구됨을 시사해 주고 있다. 본 연구 결과를 토대로 치매환자를 돌보는 간호사들이 심화된 지식 및 기술을 습득할 수 있는 노인간호 교육프로그램의 활성화가 더욱 요구된다고 보여진다.





[영문]

This study is a descriptive research study which was conducted to identify coping types, burnout, and depression of nurses caring for patients with dementia and analyze relations between the variables. This study researched 121 nurses who take care of patients with dementia at general hospitals, mental hospitals, senior recuperation hospitals, and senior recuperation facilities. A structuralized survey was used to gather resources from April 1 to May 26, 2008. For this research, the coping types of nurses were measured using the Ways of Coping questionnaire by Lazarus and Folkman (1984) which was translated, modified, and supplemented by Jung Shik Han and Ga Sil Oh (1990) and burnout was measured by the Maslach Burnout Inventory (MBI) of Maslach and Jackson (1981) which was translated by Eun Sook Jang (1995). Also, depression was measured using the Beck Depression Inventory (BDI) by Beck (1967) which was standardized by Young Ho Lee and Jong Yong Song (1991) to fit adults in Korea. Data analysis was processed using SAS 9.1.

The following are the findings of this study:

1. The average score of the nurses’ coping types was 86.53 (range: 33-132) and the average score of each question was 2.62 (range: 1-4). The most frequently used coping types was problem-centered coping with the score of 2.91. The average score for burnout was 66.15 (range: 22-154) and the average score of each question was 3.01 (range: 1-7). In detail, the majority of nurses experienced a decrease in the sense of personal accomplishment with a score of 3.47. The average score of depression was 7.07 (range: 0-63) and was normal. Among the 121 subjects, 87 (71.9%) were normal, 24 were in mild depression (19.8%), 7 in moderate depression (5.8%), and 3 in severe depression (2.5%).

2. As a result of analyzing the differences between the nurses’ general characteristics and the three variables, it was found that coping types significantly varied according to education in dementia (t=-2.3, p<0.5) and burnout significantly varied according to workplace (F=7.84, p<.00). Depression was significantly related to marital status (t=2.51, p=.01), clinical experience (F=3.15, p=.03), experience in nursing patients with dementia (F=3.35, p=.02), and the type of employment (t=2.54, p=.01).

3. The correlations of the three variables showed a positive correlation between the total clinical experiences of the nurses and education in dementia (r=.21, p=.02) and a negative correlation between the total clinical experiences of the nurses and depression (r=-.21, p=.02). Also, problem-centered coping had a negative correlation with burnout (r=-.25, p=.01) and a positive correlation with depression (r=.28, p=.00).

This study identified that nurses who take care of patients with dementia mostly use problem-centered coping, and the degree of burnout was low compared to the results of other studies. Also, the degree of depression was normal, showing positive mental health. Patients’ aggressive behaviors were not related to the nurses’ burnout and depression. These results indicate that hospitals and medical facilities must expand their dementia nursing programs for nurses who have taken educational courses in dementia to utilize various coping types accordingly. This study also indicates that it is important to revitalize senior nursing programs for these nurses to acquire more professional knowledge and skills.
