최근 심혈관계질환의 발생과 관련 있다고 알려진 혈압(blood pressure)이나 혈중 지질농도(serum lipid profiles)등 양적 형질의 유전적 관련성(genetic association)을 밝히고자 하는 다양한 연구들이 진행되고 있다. 그런데 이런 연들에서 연구 대상들이 혈압강하제나 지질강하제와 같은 치료 약물을 복용하는 경우, 그 영향을 배제하여 관련성을 분석하기 위해 복용대상자들을 제외하는 경우가 대부분이었는데, 이는 자료의 손실이라는 심각한 단점을 가지고 있다. 따라서 본 논문에서는 치료 약물 등의 효과를 보정하여 양적 형질의 유전적 관련성을 분석하기 위한 통계학적 방법들을 비교 . 평가해보고자 한다.양적 형질에 영향을 미치는 유전적 요인과 환경적 요인을 정하여 회귀 모형을 세우고 치료 효과를 보정하는 방법으로는 제외, 무보정, 지시변수 사용, 비모수적으로 잔차를 수정하는 방법, 상수 추가, 중도절단 회귀모형 등을 제시하고 연세대학교 심혈관계질환 유전체연구센터의 자료를 이용하여 실제로 적용시켜보았다. 자료에서 관상동맥질환군과 고혈압군을 택하여 남, 여 각각을 대상으로 총 콜레스테롤과 저밀도지단백에 대하여 각각 분석하였고 방법의 비교는 회귀계수의 p-value를 기준으로 하였다. 관상동맥질환군의 경우 비모수적으로 보정하는 방법이 p-value가 제일 작은 경우가 많았고, 그 다음으로는 치료대상을 제외하는 경우, 상수를 추가하는 경우, 중도절단 회귀모형을 이용하는 순으로 나타났다. 고혈압군은 상수를 추가하여 보정하는 방법이 p-value가 작은 편에 속했고, 비모수적으로 보정하는 방법과 무보정, 제외하는 방법 순으로 나타났다.



[영문]

The population-based genetic association studies of continuous traits can be seriously distorted when the traits are subject to the effects of certain treatment. Without appropriate adjustment of treatment effect, the results of analyses may be fundamentally flawed.In this thesis, we compared the performance of regression-based statistical approaches that were currently used or advocated to adjust a treatment effect. The six methods used to compare their relative performance were: excluding treated individuals from data, non-adjustment for treatment effect, modelling treatment as a covariate(indicator variable), non-parametric adjustment of treatment, adding a constant value to measurements for treated individuals, and censored normal regression. Also, we applied these methods to real genetic and clinical data to demonstrate a pattern of their behaviour. We used a sample of 1,005 coronary artery disease patients (male 766, female 239)and 1,783 hypertension patients (male 836, female 947) who have the information of lipid profiles(total cholesterol, low density lipoprotein) and single nucleotide polymorphisms(SNPs) from Yonsei cardiovascular genome center. The results of our study provided that two of the adjustment methods were more powerful than other methods for analysis of genetic association with serum lipid profiles. These were: the addition of a sensible constant to the observed lipid profiles in treated subjects, non-parametric adjustment method based on averaging ordered residuals. Censored normal regression might also be advocated in some situations. However, we found that three approaches used relatively commonly had serious problems and should not be used at all. These are: fitting a regression model with treatment as a binary indicator covariate, analysing observed lipid profiles in treated subjects as if it was underlying lipid profiles, and excluding treated subjects from the analysis. In conclusion, we have demonstrated that non-parametric adjustment method based on averaging ordered residuals and addition of a sensible constant to the observed lipid profiles in treated subjects can effectively adjust the distorting effect of lipid-lowering drug and recover a marked loss in statistical power. Also, in genetic association studies of continuous traits that distortion arising from a treatment effect really matters, we proposed to use the appropriate methods that are more effective and straightforward to implement.
