본 연구는 자가간호이행이 매우 중요한 위암환자의 교육프로그램 개발의 기초를 마련하기 위하여 2007년 4월∼6월까지 3차 병원에서 위절제술을 받은 위암환자 100명을 대상으로 자료를 수집하였고, 자가간호이행과 이에 영향을 주는 요인과의 상관관계를 규명하기 위하여 Becker의 건강신념모형의 요인들과 종속변수인 자가간호이행과의 상관관계를 분석하였다. 수집된 자료는 SPSS win 12.0 프로그램을 이용하여 대상자의 인구사회학적 특성과 질병관련 특성은 실수와 백분율로 구하고, 연구 변수들간의 상관관계는 t-test, ANOVA, Pearson correlation coefficient, Stepwise multiple regression으로 분석하였다.연구결과를 요약하면 다음과 같다.1) 위절제술환자의 자가간호이행 정도는 비교적 높은 수준(4.08±.54)으로 잘 수행되고 있었으나, 이 중 외래방문수행(4.87±.44)보다는 운동(3.99±.94)에 대한 자가간호이행이 더 낮게 나타났다.2) 위절제술 환자의 자가간호이행 영향요인 중에서 지각된 민감성/심각성 (3.50), 지각된 유익성(4.51)이 모두 중정도 이상이었고, 지각된 장애성(2.02)은 유익성보다 다소 낮게 나타났다. 한편 자기효능은 평균점이 79.07점(가능점수범위 10-100점)으로 중정도 이상으로 나타났다.3) 위절제술환자의 자가간호이행은 자기효능이 높고, 지각된 유익성이 높을수록 잘 수행되고 있었다. 그리고 자기효능은 지각된 유익성(p<.01)과 양의 관계를, 지각된 장애성(p<.05)과는 음의 관계로 유의한 상관관계를 갖는 것으로 나타나 지각된 유익성이 높고, 지각된 장애성이 낮을수록 자기효능이 높으며, 따라서 위절제술환자의 자가간호이행도 잘 수행하는 것으로 나타났다.4) 지각된 민감성/심각성은 지각된 장애성(p<.01)과 유의한 상관관계를 보였는데 질병에 대한 지각된 민감성/심각성이 높을수록 지각된 장애성도 높아지므로써 부정적인 영향을 초래할 수 있는 것으로 나타났다.5) 자기효능은 자가간호이행(p<.01), 지각된 유익성(p<.01), 지각된 장애성(p<.05)과, 지각된 유익성은 자가간호이행(p<.01)과, 또한 지각된 장애성은 지각된 민감성/심각성(p<.01)과 유의한 상관관계를 보였다.6) 위절제술환자 자가간호이행에는 자기효능이 가장 중요한 변수로(25.1%)로 나타났으며, 체중변화, 지각된 장애성이 유의한 변수로 포함되었다. 3개의 변수가 포함된 최종 모형의 설명력은 34.8%로 증가하였다.결론적으로 위절제술환자의 자가간호 이행을 증진시키기 위해서는 자기효능과 지각된 유익성을 높여주어야 하고, 지각된 장애성과 지각된 민감성/심각성은 위암환자의 자존감을 저하시키므로 위절제술환자 간호에 중요한 시사점을 나타내 준다고 본다.





[영문]

The purpose of this study was done to provide basic data for the development of patient education programs for patients with a gastrectomy and also data to provide a basis for the nursing care of these patients. The participants in the study were 100 patients who had a gastrectomy in one of several general hospitals in Seoul. The data collection period was from April 12 to June 4, 2007.In order to analyze the relationship between self-care and the factors which influence it Becker's Health Belief Model and self-care compliance were used in this work.Data are expressed as means±SD. Data were evaluated with one-way analysis of variance (ANOVA), Pearson correlation coefficient, and stepwise multiple regression followed by Student's t-test using SPSS PC ver. 11 program. Statistically significant differences were reported as *p<0.05, **p<0.01, or ***p<0.001. Data with values of p<0.05 were generally accepted as statistically significant.The study results are as follows:1) The average level of self-care of patients with a gastrectomy was 4.08±0.54 (5 point Likert scale).2) The mean scores for perceived sensitivity/severity, perceived benefits, perceived barriers, and self-efficacy were 3.50±0.91 (range: 1-5), 4.51±0.48, 2.02±0.69, and 79.07±13.33 (range: 10-100), respectively.3) The level of self-care of the patients showed significantly positive correlations with self-efficacy and perceived benefits. Also, self-efficacy was positively correlated with perceived benefits (p<.01). But, self-efficacy was negatively correlated with perceived barriers (p<.05), and a high level of self-care in patients with a gastrectomy.4) Perceived sensitivity/severity was significantly correlated with perceived barriers (p<.05). The more perceived sensitivity/severity increased, the more perceived barriers increased, showing negative influences in patients with a gastrectomy.5) Self-efficacy had a significant positive correlation with self-care in these patients (p<.01). Perceived benefits also had a significant positive correlation with level of self-care (p<.01) and perceived barriers had a significant positive correlation with perceived sensitivity/severity (p<.05).6) In the stepwise multiple regression analysis, the combination of three factors, self-efficacy, body weight change, and perceived barriers accounted for 34.8% of the variance in the level of self-care of patients with a gastrectomy. One of the three factors, self-efficacy was a very important factor, accounting for 25.1% of the variance in level of self-care.In conclusion, the results suggest that self-efficacy and perceived benefits are more important factors than perceived barriers and perceived sensitivity/severity for self-care in patients with a gastrectomy. The findings from this study are expected to provide concrete data to support nursing interventions to improve self-care in patients with stomach cancer.
