경장 영양 환자를 위한 간호 가이드라인 개발

중환자는 과대사 및 단백질 이화작용으로 인해 영양

요구량이 급속히 증가되는 반면, 영양 공급의 부족으로 영양결핍에 빠질 위험이 크다. 영양결핍의 문제를 해결하기 위해 중환자실에서 경장영양은 꼭 필요한 영양공급방법이다. 하지만 경장영양 환자의 4∼95%가 흡인성 폐렴을 경험하고 있으며 이로 인한 사망률 또한 17∼62%이다. 본 연구는 경장 영양 환자를 위한 간호 가이드라인을 제공함으로써 간호사들에게 실제적인 교육 자료를 개발하기 위한 방법론적 연구이다.본 연구의 과정은 2단계로 진행되었으며 제 1단계로 간호사의 경장 영양 환자 간호 지식수준과 지식요구를 조사하고 제 2단계로 제 1단계의 결과와 문헌고찰을 통해 경장 영양 환자를 위한 간호 가이드라인을 개발하였다.2007년 3월 1일부터 5일까지 서울 소재의 1개 대학병원의 중환자실에서 근무하는 간호사 30명을 대상으로 간호사의 경장 영양 환자 간호 지식수준과 지식요구를 조사하였다. 예비 간호 가이드라인 개발은 자료 분석을 통해 대상자의 각 항목별로 지식요구가 80%이상인 항목을 가이드라인 내용 개발 항목으로 선정하였다.선정된 항목은 간호대학 교수 2인과 병원의 간호국 부원장 1인, 중환자실 수간호사 1인, 중환자실 간호사 경력이 5년 이상인 간호사 10인 등 모두 14명의 전문가 집단을 대상으로 내용 타당도를 검증하였고 CVI를 산출하여 지수가 0.80이상인 항목을 선택하였다. CVI를 통해 선정된 예비간호가이드라인은 전문가 집단의 감수를 거쳐 수정 및 보완하여 4개 영역의 42개 항목으로 구성된 최종 간호 가이드라인을 완성하였다.연구의 결과를 요약하면 다음과 같다.1. 간호사의 경장 영양환자 간호 지식수준은 3점 만점에서 최고 2.6점, 최저 1.3점이며 평균 2.0점이었으며, 경장 영양환자 간호 지식 요구는 42개 항목 모두 80%이상의 요구가 있었다.2. 경장 영양 환자를 위한 예비 간호가이드라인 항목은 문헌의 내용을 기초로 항목을 선정하고 각 항목에 포함된 모든 내용을 바탕으로 예비 간호 가이드라인을 개발하였다. 경장 영양 환자 간호 가이드라인 항목은 경장 영양의 특성영역, 경장 영양 환자를 위한 간호 사정과 간호 중재, 간호평가의 4개 영역으로 구분하였으며 경장영양의 특성영역에 28개 항목, 간호사정 영역에 4개 항목, 간호중재 영역에 8개 항목, 간호평가영역에 2개 항목으로 구성하였다.3. 경장 영양 환자 예비 간호 가이드라인은 전문가의 내용 타당도를 검증하였으며 CVI를 산출하여 지수가 0.80 이상인 내용을 선정하였다.4. 최종 경장 영양 환자 간호 가이드라인은 4개 영역의 42개 항목으로 구체적인 내용은 다음과 같았다.경장 영양 특성 영역에는 경장 영양의 정의, 영양공급 방법에 따른 장점과 단점, 경장 영양 환자의 적응증, 경장 영양의 금기사항, 경장 영양 공급방법 형태에 따른 적응증과 각각의 장점과 단점, 경장 영양의 합병증에 대한 내용으로 구성하였으며, 간호사정 영역에는 신체 사정(assessment)과 영양상태 사정, 경장 영양의 합병증 사정 그리고 폐 흡인의 위험요인에 대한 내용을 구성하였고, 간호중재 영역에는 비위관 튜브 간호와 위루술 간호, 경피 내시경 위루술 간호, 누공 간호로 나누어 설명하였으며 구강간호, 커프(cuff)관리, 튜브 위치 확인방법, 환자 자세, 박테리아 감염예방법, 경장 영양의 합병증 관리, 경장 영양의 관찰사항, 퇴실관리의 내용이 포함되었다. 마지막으로 간호평가 영역에는 경장 영양 환자를 위한 영양 상태 평가와 합병증 평가의 내용으로 구성하였다.결론적으로, 본 연구를 통해 개발된 간호 가이드라인은 경장 영양 환자를 위한 간호 교육 방법의 한 방법으로, 중환자실에서 흔히 접하는 경장 영양 환자의 영양 상태와 합병증을 파악하는데 중요한 자료가 될 것이며, 최상의 간호 중재를 제공하고, 평가하여 영양결핍과 주요한 합병증인 흡인성 폐렴의 재발률 감소에 기여할 것으로 생각된다.

[영문]There is a need for rapid nutritional intake during serious cases of hyperactive protein catabolism, and the danger of falling into nutrient deficiency is great.Enteral nutrtion is an all-important alimentation method applied at intensive care units to solve problems of nutrient deficiency. However, 4-95% of enteral nutrition patients are experiencing aspiration pneumonia and the death ratio from this is 17-62%.This study was done using methodological research to develop actual education data for nurses in order to provide a nursing guideline for enteral nutrition patients.The process of this study proceeded in two stages and investigated nurses' enteral nutriton patient nursing knowledge level and knowledge request in the first step, and developed a nursing guideline for enteral nutrition patients by my second-stage through results of the first step and literature investigation. I investigated the enteral nutrition patient nursing knowledge level and knowledge request of 30 nurses who worked in intensive care units at a university hospital in Seoul from March 1 to March 5, 2007. The developing guideline uses data analysis and only inculudes measures with at least an 80% approval rating. Those who took part in the rating included 2 university professors, 1 nursing department staff member, and 1 ICU head nurse, 10 nurses with over five years experience in this field, for a total of 14 experts who selected measures with at least a 0.80 CVI index rating. Through the revisions and supervision of these experts, a guidelines of 42 measures in 4 areas was completed.The oucome of the research is as follows:1. Nurses' enteral nutrition patients nursing knowledge levels ranged from 1.3 to 2.6, out of a perfect score of 3 and had an average of 2.0, with 42 items having an approval rating of 80%.2. The guideline for enteral nutrition patients inculed measures based on documents. The measures were divied into for groups: 28 measures in the field of special reserch, 4 measures in the field nursing assesment, 8 in nursing intervention, and 2 in nursing evaluation.3. The guideline was verified by experts and measures with a CVI index rating of more than 0.80 were chosen.4. Finally, the enteral nutrition patient nursing guideline substances that are specific to 42 items of 4 areas as following are:Enteral nurtition special quality area composed by definition of enteral nutrition, strength and weakness by alimentation method, enteral nutrition patient's indication, enteral nutrition patient's contraindication, indication and each strength and weakness by enteral nutrition supply method form, contents about enteral nutrition's complication, composed physical exam and nourishment assessment, enteral nutrition's complication assessment and substance for risk factor of aspiration pneumonia in nursing assessment area, nursing intervention area divided and explained by nasogastric tube nursing and gastrostomy nursing, percutaneous endoscopic gastrostomy nursing, fistila nursing, oral care, cuff management, tube position confirmation method, patient position, bacteria infection preservative, complication management of enteral nutrition, monitoring item of enteral nutrition, and cheking government officials' contents were included.Finally, nursing evaluation area was composed of contents of nutritive conditions evaluation and complication evaluation for enteral nutrition patient.Conclusively, the nursing guideline developed through this study is a method of nursing education for enteral nutrition patients. The guideline fully takes into account the enteral nutrition patient's nutritive conditions as well as complications that may exist at intensive care units. I think the guideline is important in order to evaluate complications and provide the best nursing intervention. It may help contribute to a significant decrease in the reappearance rate of aspiration pneumonia.
