출혈성 쇽을 일으킨 환자를 조기에 치료하여야 비가역적 쇽의 발생을 방지할 수 있다.

현재 권장되는 쇽의 치료 방법은 초기에는 등장성 수액을 투여하고, 수액소생 치료에도

불구하고 지속적으로 활력징후가 불안정하면 혈액제재를 사용하는 것이다. 등장성 수액을

이용한 소생에는 쇽을 교정하는 효과에 한계가 있으며 다량의 수액을 사용하게 되면 저

체온증, 혈액응고장애, 산혈증 등의 부작용이 발생한다. 쇽 교정을 위한 수액으로 사용되

는 등장성 수액을 대체하기 위해 고장성식염수의 투여가 연구되고 있다. 고장성식염수는

이론적으로 적은 양을 투여하여도 혈장량이 늘어나서 소량의 수액으로 쇽을 교정하는 소

생 효과를 보일 수 있고 조직의 부종을 억제하는 효과가 있다. 실험적으로 뇌 조직의 부

종을 억제하는 효과는 입증되었다. 그러나 고장성식염수를 이용한 쇽의 치료와 등장성 수

액을 이용한 쇽의 치료의 사망률을 비교한 실험에서 고장성식염수를 이용한 수액소생이

사망률을 떨어뜨린다는 가설은 확실한 검증이 되지 않은 상태이다. 본 연구의 목적은 출

혈성 쇽을 유발한 백서모델을 이용하여 고장성식염수와 생리식염수를 사용하여 수액소생

시 쇽을 치료하는 효과를 비교하는 데 있다. 이 연구에는 백서를 고장성식염수군과 생리

식염수군으로 나누어 실험하였다. 각 군 당 10마리의 백서의 동맥에 도자를 삽입하여 혈

압과 맥박을 감시하는 상태에서 실혈시켜 쇽을 유발하고 15분 동안 쇽 상태를 유지한 뒤,

정맥에 삽입된 도자를 통해 각각 생리식염수 또는 고장성식염수를 일정한 주사속도로 투

여하여 수액소생을 한 후, 평균혈압의 변화와 동맥혈의 산소포화도, pH, 염기부족, 젖산

농도 및 나트륨농도를 비교하였다. 실험의 결과로서 고장성식염수로 수액소생을 한 고장

성식염수군이 생리식염수를 이용하여 수액소생을 한 생리식염수군에 비하여 적은 양의 수

액 투여로도 혈압상승을 일으키는 쇽 치료효과를 보였다. 그러나 두 실험 군 사이의 산소

포화도 및 pH, 염기부족, 젖산농도의 의미 있는 차이는 없었다. 고장성식염수군에서 측정

한 혈액 내 나트륨농도는 생리적으로 안전한 범위에 들어있었다. 결론적으로 출혈성 쇽을

유발한 백서 모델에서 고장성식염수와 생리식염수를 이용한 수액소생의 효과를 비교한

결과, 고장성식염수를 이용한 수액소생이 생리식염수를 이용한 수액소생에 비하여 적은

양으로도 높은 혈압 상승 효과를 가지고 있었다.

The guidelines for fluid resuscitation of trauma patients with severe hemorrhagic

shock has advised aggressive fluid resuscitation with isotonic crystalloid

solutions followed by blood transfusion. However, the effect of resuscitation with

large amount of isotonic saline has been questioned because it cannot alway prevent

the patient go into refractory shock and infusion of large amount of isotonic

saline provokes various adverse consequences. Recently, there have been several

experimental and clinical reports that resuscitation with hypertonic saline has

advantages over resuscitation with isotonic saline in terms of hemodynamic and

metabolic effects.

This study is designed to examine whether hypertonic saline has advantageous

effects over isotonic saline to resuscitate early stage of hemorrhagic shock in a

controlled hemorrhagic model. Twenty Sprague-Dawley rats were used in this study.

The femoral artery was exposed by surgical dissection and a catheter was inserted

in the rat for continuous blood pressure monitoring and blood sampling for blood

gas analysis and chemistry. Another catheter was inserted via the right internal

jugular vein for fluid infusion. After preparation and induction of hemorrhagic

shock (40 mm Hg in systolic blood pressure) by bleeding via the catheter in the

femoral artery, rats were randomised into hypertonic saline group (rats

resuscitated with hypertonic saline) and isotonic saline group (rats resuscitated

with isotonic saline). Ten rats were enrolled in each group, respectively. Infusion

rate of hypertonic saline (16 ugtt) was pre-determined by a pilot study. Mean

arterial blood pressure, pH, lactate, base deficit, PaO₂, SaO₂, PaCO₂, sodium

concentration, potassium concentration, and total amount of fluid infused during

resuscitation were compared between two groups at baseline and after shock

resuscitation. At baseline, there was no difference in mean arterial blood

pressure, pH, lactate, base deficit, PaO₂, SaO₂, PaCO₂, sodium concentration,

and potassium concentration. Resuscitation with hypertonic saline induced a higher

rise in the mean arterial pressure than normal saline. The sodium concentration

level was higher in the hypertonic saline group after resuscitation, but it was

within a clinically acceptable range. Total amount of fluid to resuscitate the

shock was less in hypertonic saline group than in isotonic saline group. There was

no difference in pH, lactate, base deficit, PaO₂, SaO₂, PaCO₂, and potassium

concentration after resuscitation between two group. It is concluded that

hypertonic saline has an advantage to resuscitate hemorrhagic shock over isotonic

saline by raising blood pressure rapidly with small amount infusion in a controlled

hemorrhagic shock model of rat.
