광화학요법이 표피 Langerhans세포에 미치는 영향에 관한 형태학적 연구

광독성 약물인 psoralen을 투여한 후 장파장의 자외선(UVA)을 조사하는 광화학요법은

여러 피부질환에 이용되어 좋은 치료효과를 보이고 있다. 광독성 약물중 8-MOP는 건선 치

료에 주로 이용되고 있으며 TMP는 백반 치료에 많이 이용되고 있다.

Langerhans세포는 표피의 기저세포 상층부에 존재하는 수지상 세포로서 T임파구에 항원

을 소개하는 기능을 가지고 있으며 접촉과민반응의 유발, 이식편 거부반응 및 표피세포-

임파구반응등에서 중요한 역할을 한다. 그러나 Langerhans세포는 자외선 및 광화학요법에

의해 쉽게 손상받는다고 하며 자외선 조사후 나타나는 접촉과민반응의 감소 및 이식편

거부반응의 저하는 Langerhans세포손상과 일부 관계가 있다고 한다. 그러나 자외선 및 광

화학요법에 의해 Langerhans세포에 어떤 손상이 초래되는지는 저자에 따라 의견이 일치하

지 않고 있다. 또한 광화학요법을 반복 시행하면서 Langerhans세포의 변화를 장기간 관찰

한 보고와 TMP를 이용한 광화학요법이 Langerhans세포에 미치는 영향에 대한 보고가 없었

다. 이에 저자는 8-MOP 와 TMP를 이용한 광화학요법을 기니픽에 수주간 반복 시행하여 La

ngerhans세포의 수와 형태의 변화과정을 장기간 관찰하고 또한 8-MOP와 TMP가 Langerhans

세포에 미치는 영향에 차이가 있는지를 관찰하고자 본 실험에 착수하였다.

실험동물로 자성 백색 기니픽을 사용하여 배부의 털을 제거한 후 매주 2회씩 소량투여

군에는 8-MOP 또는 TMP 1.5mg/kg, 대량투여군에는 3mg/kg씩을 복강내 주사하고 1시간 후

에 UVA를 4J/cm**2 씩 조사하였으며 매주 1회 조직생검을 실시하였고 ATPase 염색을 하여

광학현미경으로 관찰하였다. 또한 일부 조직에서는 전자현미경 관찰을 시행하여 다음과

같은 결과를 얻었다.

1. 광독성약물 소량투여군의 Langerhans세포수는 광화학요법 실시 4주부터 감소하였고

광화학요법중지 후에는 점차 회복되어 광화학요법중지 4주 후에는 광화학요법 실시전과

비슷한 수준으로 회복되었다. 세포면적은 광화학요법 실시 1, 2, 3, 4주까지 큰 변화가

없었으나 광화학요법 실시 5주 후부터 증가하기 시작하여 광화학요법 중지 후에도 증가되

어 있다가 광화학요법 중지 4주 후에는 정상으로 회복되는 경향을 보였다. 기간에 따른

세포둘레의 변화는 세포면적의 변화와 비슷하였다.

2. 광독성약물 대량투여군의 Langerhans세포수는 광화학요법 실시 1주부터 급격히 감소

하여 3주까지 계속 감소하였고 광화학요법 중지 후에는 점차 회복되었으며, 세포면적은

광화학요법 실시 2주 후에 감소하였고 광화학요법 중지 3주 후에는 크게 증가하였다가 4

주 후에는 정상으로 회복되었다. 기간에 따른 세포둘레의 변화는 세포면적의 변화와 비슷

하였다.

3. 동량의 8-MOP와 TMP를 이용한 광화학요법이 Langerhans세포에 미치는 영향에는 차이

가 없었다.

4. 광화학요법실시 중에는 수지상돌기가 소실된 원형, 타원형 또는 방추형의 세포가 많

이 나타나다가 광화학요법 중지 후에는 수지상돌기가 잘 발달되고 정상세포 보다 2배 이

상 큰 거대세포가 많이 나타났으며 광화학요법 중지 4주 후에는 다시 정상형태의 세포로

회복되는 경향을 보였다. 이와같은 형태변화는 광독성약물 대량 투여군에서 잘 볼 수 있

었다.

5. 전자현미경 관찰결과 광화학요법을 받은 동물의 Langerhans세포는 퇴행성변화를 보

였다. 즉 미토콘드리아 및 골지체등 세포질내 소기관들의 감소를 보였고 또한 세포질내

많은 공포를 보였다.

이상의 결과를 종합하면 Langerhans세포는 광화학요법에 의해 쉽게 손상받으며 그 손상

은 세포표면 표적의 손실이라기 보다는 세포자체의 손상으로 추측되었으며, 손상의 정도

는 광독성 약물의 투여용량에 비례하였다. 또한 8-MOP와 TMP론 이용한 광화학요법이 Lang

erhans세포에 미치는 영향에는 차이가 없었으며 광화학요법 중지 후에는 Langerhans세포

수 및 형태가 회복되었다. 회복기에 많이 보이는 거대 Langerhans세포는 광화학 요법으로

부터 살아남은 세포가 대상작용에 의해 커진 것으로 사료되었다.





Quantitative and Morphologic Studies on the Effect of Photochemotherapy on

Epidermal Langerhans Cell

During the past few years, photochemotherapy using psoralen and long-wave

ultraviolet light(PUVA) has proved to be considerably effective in the treatment of

psoriasis and several other conditions. The psoralens most widely uaed in

photochemotherapy are 8-methoxypsoralen (8-MOP), which is primarily used for the

treatment of posriasis and 4, 5', 8-trimethylpsoralen (TMP), which is primarily

used for the treatment of vitiligo.

Due to the fact that the skin is the only tissue that is naturally exposed to UV

radiation, several investigators have studied the effect of UV on the Langerhans

cell(LC) population and on LC-dependent immune functions of the epidermis. Recent

studies have demonstrated that epidermal LCs are a population of cells derived from

the bone marrow, and that they bear Fc and C3 receptors and Ia antigens on their

surface. Functionally, LCs are required as antigen-presenting cells for the

initiation of the immune response, i.e., contact hypersensitivity, skin graft

rejection, and also, they are required as stimulator cells in the epidermal

cell-lymphocyte reaction.

It is now evident that UV radiation from sunlight or artificial sources during

photochemotherapy causes various functional and morphological alterations in LCs.

However, there are some conflicting results concerning the exact mode of the LC

depletion process following UV irradiation.

The present study was undertaken in order to clarify the exact mode of the LC

depletion process following systemic PUVA therapy using 8-MOP and TMP. We have

studied the number and the morphological alterations of ATPase positive cells in

guinea pig epidermal sheets and have also performed electron microscopic

observations on them.

The following results were obtained.

1. In the small dose group(1.5mg/kg of psoralen), the number of LCs decreased at

4 and 5 weeks of PUVA, and the density of the LCs returned to normal by the 4th

week after the discontinuance of PUVA. The size and the circumference of LCs were

slightly decreased at 4 weeks of PUVA, then increased at 2 and 3 weeks following

the discontinuance of PUVA, and gradually returned to normal by 4 weeks.

2. In the large dose group (3mg/kg of psoralen), the number of LCs began to fall

dramatically in the first week of PUVA. The density of LCs gradually returned to

normal following the discontinuance of PUVA. The size and the circumference of LCs

decreased at 2 weeks of PUVA, and then markedly increased at 3 weeks after the

discontinuance of PUVA, and returned to normal at 4 weeks.

3. When the effects of 8-MOP and TMP on LC were compared at the same doses, no

significant differences were found between 8-MOP and TMP.

4. The morphological changes of LCs following PUVA were prominent in the large

dose group. During PUVA administration most cells were observed as round and

without dendrites. However, 2 and 3 weeks after the discontinuance of PUVA, we

observed many LCs which were approximately 2 times their normal size and had

elongated dendritic processes. By the 4th week after the discontinuance of PUVA,

such giant cells began to decrease in size.

5. On electron microscopy, many LCs showed degenerative changes, such as the

appearance of many intracytoplasmic vacuoles and the loss of mitochondria, golgi

apparatus and lysosomes in the small dose group. Similar degenerative changes were

noted in both LCs and keratinocytes, though these changes were much more prominent

in LCs. In the large dose group, the degenerative changes were so severe that we

could not find LCs at 3 weeks of PUVA.

In summary, our results indicate that LCs are sensitive to PUVA, the degree of

change is dose-related, and the restoration of LCs takes place by the 4th week

following the discontinuance of PUVA. It seems likely that moderate dose of PUVA

caused an actual decrease in their number due to cell damage. Alas, we suggest that

the appearance of giant LCs is a reactive process of the remaining LCs following

PUVA.
